[Infective complications of the central nervous system (CNS) in addicts to parenterally administered drugs].

Given the high rate of infection by HIV in the APAD in our country, the description of localized infectious problems in the CNS secondary to drug-addiction should always take into account opportunistic infections or tumours occurring there. The initial clinical evaluation should highlight or rule out the presence of clinical indications (specially polyadenia and oral candidiasis) which suggest a clinical condition of immunodepression. Septic embolization due to bacteremia and fungemia is common among drug-addicts, sometimes causing, although not very often in comparison with other sites, vario

Isolation of Acanthamoeba from a cerebral abscess.

A 55-year-old diabetic aboriginal woman presented with a two-week history of fever, altered mental state and convulsions. On the basis of computed tomographic scanning a diagnosis of cerebral abscess was made. The pus that was drained produced no bacterial growth but, on microscopy, amoebic cysts were observed. Special cultures produced a growth of Acanthamoeba. The patient appeared to respond to drainage of the abscess and antiprotozoal therapy. Unfortunately, she developed necrotizing enteritis which led ultimately to her death. Antibiotic sensitivity and pathogenicity testing suggest that t

Left and right cerebral hemisphere differences in the occurrence of epilepsy.

Differences in function between the cerebral hemispheres are well documented for normal subjects and, in diseased states, these are used to lateralize and localize a dysfunction. However, the difference in frequency of occurrence of left vs. right hemisphere lesions and the greater likelihood of epilepsy occurring on the left have received scant attention. In the present study patients with a diagnosis of meningioma or cerebral abscess were identified from the EEG computer file. After operation the meningioma patients with predominantly left-sided tumours showed a statistically significant gre

CT diagnosis of intracranial cystic abnormalities in children.

The introduction of computed axial tomography has been of tremendous importance in the evaluation of intracranial pathology in children. The broad spectrum of cystic intracranial lesions of congenital, infectious, traumatic, vascular and neoplastic origin are discussed. Their CT findings, pathology and pathogenesis are emphasized.

Scedosporium apiospermum: A Rare Cause of Aggressive Orbital Apex Syndrome.

Orbital apex syndrome (OAS) is a localized orbital cellulitis at the orbital apex that can cause vision loss from optic neuropathy and ophthalmoplegia involving multiple cranial nerves. Herein, we report a rare and rapidly progressive case of OAS secondary to fungal pansinusitis caused by Scedosporium apiospermum in an immunocompromised patient following the extraction of abscessed teeth. A 48-year-old man with diabetes mellitus who had failed to adhere to his treatment presented with complaints of a right-sided headache and toothache for two weeks, with nausea and vomiting for two days prior

Endoscopic and intrathecal management of intraventricular fungal infections: a case report of Cladophialophora bantiana and systematic review.

BACKGROUND: Intraventricular fungal infections are rare and associated with high morbidity and mortality. Diagnosis is often delayed due to nonspecific clinical and radiological findings, and treatment is complicated by limited cerebrospinal fluid (CSF) penetration of systemic antifungal agents. Neuroendoscopic visualization and intrathecal antifungal therapy have been reported only sporadically, and standardized management strategies are lacking. CASE PRESENTATION: We report a 75-year-old male who developed a right temporal lobe abscess caused by Cladophialophora bantiana, confirmed by histop

Unusually high frequency of pituitary abscess in the post-COVID-19 era: insights from a single-center experience.

OBJECTIVE: Pituitary abscess (PA) is a rare but life-threatening condition, accounting for < 1% of pituitary lesions. At a single center, 140 endoscopic transsphenoidal surgeries for pituitary lesion resection were performed since 2018, with 13 patients diagnosed with PA at an unusually high incidence rate of 9.2%. The aim of this study was to characterize the diagnostic criteria, symptoms, microbiological profile, radiological features, and endocrinological characteristics of PA, while exploring potential reasons for the increased frequency of PA in the post-COVID-19 era. METHODS: A single-c

Uncommon Streptococcus Constellatus Meningitis Leading to Pulmonary Abscess and Brainstem Infarct in an Immunocompetent Patient.

BACKGROUND Except for neonates, streptococci other than Streptococcus pneumoniae are a rare cause of acute bacterial meningitis. Streptococcus constellatus is a member of the Streptococcus anginosus group of gram-positive streptococci. It is a commensal microbe of the mucosae of the oral cavity, gastrointestinal tract, and urogenital tract. Rarely, it becomes pathogenic and causes contiguous or distant infections after mucosal damage. This report describes a 19-year-old immunocompetent man who developed bacterial meningitis, lung abscess, and brainstem infarct secondary to Streptococcus conste

Hereditary haemorrhagic telangiectasia manifesting as subdural empyema.

Hereditary haemorrhagic telangiectasia (HHT) also known as Osler-Weber-Rendu disease is a rare autosomal dominant condition causing vascular dysplasia. Cerebral abscess formation, secondary to paradoxical septic emboli via HHT-derived pulmonary arteriovenous malformations (pAVMs) in this context is well documented. Herein, we present the first case of subdural empyema with this aetiology.

HIV/AIDS presenting with stroke-like features caused by cerebral Nocardia abscesses: a case report.

BACKGROUND: Immunosuppression in Human Immunodeficiency Virus can predispose to opportunistic infections of the central nervous system and can be life threatening without early recognition and management. This can be delayed in undiagnosed Human Immunodeficiency Virus. The present article is the only case report in the literature to describe a first presentation of Acquired Immune Deficiency Syndrome as cerebral Nocardia abscesses that were initially treated as a stroke. CASE PRESENTATION: A previously well 59 year old Caucasian man presented with sudden onset of left sided hemiparesis and se

How should eosinophilic cystitis be treated in patients with chronic granulomatous disease?

BACKGROUND: Chronic granulomatous disease (CGD) is a primary immunodeficiency resulting from the absence or malfunction of oxidative mechanism in phagocytic cells. The disease is due to a mutation in one of four genes that encode subunits of the nicotinamide adenine dinucleotide phosphate (NADPH) oxidase complex. Affected patients experience severe infections and granuloma formation due to exuberant inflammatory responses. Some evidence suggests that eosinophilic cystitis (EC) is included in the spectrum of inflammatory manifestations. EC is an inflammatory disease, rare in childhood, which ma

Intracranial actinomycosis: varied clinical and radiologic presentations in two cases.

Two patients with primary actinomycotic brain infection are presented here. The first case had 2 predisposing factors, cardiac septal defect and chronic mastoiditis, whereas the second patient was a chronic smoker, belonging to a desert region. Both the patients were successfully managed with surgical debridement and prolonged administration of antibiotics.

An unusual form of listerial CNS infection.

Listeria monocytogenes is a known cause of meningitis, but cerebral abscess formation is exceedingly rare. We describe a patient who presented with an unsteady gait and a small ring-enhancing lesion in the right parietal lobe. Pathologic evaluation demonstrated an abscess with associated microglial nodules, and culture revealed Listeria. In spite of the rarity of this entity, immunosuppression has been noted to be a predisposing factor. Our patient had diabetes mellitus and chronic obstructive pulmonary disease with recent prednisone administration, both very common ailments in the population
