Intracranial migration of bone dust after intraventricular neuroendoscopy complicating acute hydrocephalus and removal of bone dust: case report.

OBJECTIVE: To present the first case of neuroendoscopic removal of migrated intraventricular bone dust and gel foam after intraventricular endoscopic surgery. CLINICAL PRESENTATION: A 37-year-old man was admitted with a 2-year history of headache. Brain computed tomography (CT) scan and magnetic resonance imaging revealed a cavum vergae cyst. The patient was operated on by stereotactically guided endoscopic cyst fenestration with no intraoperative complications. Postoperative CT scan demonstrated regression of the cyst with no other pathological findings. Because of a postoperative fever, a l

Surgical retrieval of embolised atrial septal occluder device from pulmonary artery: pathophysiology and role of the intraoperative transoesophageal echocardiography.

Atrial septal defect is usually closed in the cardiac catheterisation laboratory using atrial septal occluder (ASO) device. One of the complications associated with the procedure is embolisation of the device into the pulmonary artery. We are reporting two cases wherein the pulmonary embolisation of ASO device occurred during the procedure in one patient and in the early post-procedure period in another; both were retrieved surgically. We are also describing the haemodynamic consequences of this complication and the role of intraoperative transoesophageal echocardiography during surgical retri

[An uncommon use of argon plasma coagulation: endoscopic transection of self-expanding metallic stents].

Migration, distal malposition or fracture of self-expanding metallic biliary or esophageal stents rarely occur but may lead to stent dysfunction, mucosal trauma secondary to impaction or friction on the digestive wall. We report our experience of using argon plasma to endoscopically cut three metallic biliary and esophageal self-expanding stents to restore permeability or to treat impaction of the stent after migration, malposition or fracture.

A wandering defibrillator lead.

A 53-year-old woman was admitted because of sudden onset of severe chest pain and palpitations. Her medical history revealed an out-of-hospital cardiac arrest due to ventricular tachycardia caused by arrhythmogenic right ventricular cardiomyopathy for which an implantable cardioverter-defibrillator was implanted with epicardial patches. On 2D echocardiography, a mobile piece of lead was seen in the right atrium and right ventricle, loosely attached to the free wall of the right ventricule; 3D reconstruction confirmed this suggestion. Remarkably, the lead disappeared while performing the echoca

A comparison of covered and uncovered Wallstents for the management of distal malignant biliary obstruction.

BACKGROUND: The efficacy and complications of covered self-expandable metal stents (SEMS) remain controversial. OBJECTIVE: We conducted this study to compare the efficacies and complication rates of covered and uncovered SEMS in malignant biliary obstruction. DESIGN: Data on patients who underwent endoscopic retrograde biliary drainage with covered or uncovered Wallstents for the palliation of unresectable distal malignant biliary obstruction were collected. Stent patency, complications, and causes of stent occlusion were analyzed. SETTING: We collected the data retrospectively. PATIENTS:

Repair of a ruptured globe using topical anesthesia.

We present a 76-year-old patient who had ocular trauma with dehiscence of the wound and scleral rupture with a prolapsed iris, ciliary body, intraocular lens, and vitreous after uneventful cataract surgery with a self-sealing sclerocorneal tunnel incision. General anesthesia was not possible because the patient had a history of lung cancer with extensive emphysema and unstable coronary disease. Local retrobulbar or peribulbar anesthesia was not considered because of the risk for further extrusion of intraocular contents. Topical anesthesia was applied with a 10.0 mm x 2.5 mm cellulose sponge s

Aortic pseudoaneurysm from aortic penetration with a bird's nest vena cava filter.

This is a case report of a 29-year-old woman with an infected aortic pseudoaneurysm. Two years previously a bird's nest vena cava filter was placed after complex gastric surgery. Imaging studies and operative findings showed that the pseudoaneurysm was caused by penetration of the aorta by a prong of the vena cava filter.

Complications of hepatic artery infusion: a review of 4580 reported cases.

PURPOSE: The resurgence of hepatic artery infusion (HAI) for the treatment of colorectal liver metastases has been dampened by concern over its complications. We have reviewed the incidence and frequency of complications associated with HAI and discussed the factors associated with these complications. METHODS: A PUBMED search was conducted from 1950-2001 using various combinations of these keywords: hepatic arterial infusion, colorectal carcinoma, complications, and trials. The main inclusion criterion was the reporting of HAI complications. The main exclusion criterion was duplicated patien

Epicardial electrode insertion by means of video-assisted thoracic surgery technique.

BACKGROUND: The number of patients who depend on pacemaker (PM) insertion is steadily increasing. Slovenia has two centers for PM insertion, in which 250 such procedures are performed every year. Thus, with regard to the number of procedures per year per million inhabitants, Slovenia holds a middle position in the list of countries. METHOD: We report a patient with cardiomyopathy, complete A-V block (A-V block III) and permanent transvenous pacing lead replacements. The resulting thrombosis of the superior vena cava compelled us to insert an epicardial permanent pacing lead. Video-assisted th

Percutaneous techniques for managing maldeployed or migrated stents.

PURPOSE: To report our experience in the percutaneous management of dislocated endovascular stents. METHODS: During a 6-year period, 28 (2.7%) patients with a maldeployed or migrated endovascular stent (20 Palmaz, 5 Wallstent, 2 Memotherm, and 1 AVE) were recognized among 1021 patients undergoing noncardiac vascular stent procedures. Percutaneous stent management was performed using balloon catheters, gooseneck snares, grasping forceps, or additional stent implantation on the basis of the stent type, degree of expansion, mechanical characteristics, and location of the stent. RESULTS: Three s

A review of 560 Hickman catheter insertions.

Indwelling, cuffed, tunnelled, central venous (Hickman) catheters are increasingly being used for venous access and the administration of chemotherapy for oncological patients. This paper reviews the technical problems associated with the percutaneous insertion of these catheters and the complications arising from their use. Five hundred and sixty catheters were inserted; 31.3% had complications at insertion, most commonly precipitation of an arrhythmia (13.9%). Arterial puncture occurred in 3.8% and pneumothorax in 1.6%. Catheters remained in place for a median period of 91 days. Forty percen

Migration of jugular or subclavian venous catheters into inferior tributaries of the brachiocephalic veins or into the azygos vein, with possible complications.

Six children are reported in whom a central venous catheter from the internal jugular or the subclavian vein migrated to an anomalous position: the left superior intercostal vein in the first two cases, the thymic vein in the third, and the azygos vein in the last three. Resultant complications in five cases were: extravasation of the infusate in the first and third case; local vascular stenosis or complete vascular occlusion in the second and fifth case; and obstruction of the azygos arch due to local thrombosis and possible stenosis, with a likely extravasation of the infusate, in the sixth

Coated and uncoated self-expanding metal stents for malignant stenosis in the upper GI tract: preliminary clinical experiences with Wallstents.

OBJECTIVES: The clinical feasibility of self-expanding metal stents with respect to the technical success, complications, and reintervention rate should be tested. METHODS: Five coated and 26 uncoated prototype Wallstents, especially designed for stenosis of the upper GI tract, were implanted in 23 patients. All patients with dysphagia suffered from inoperable tumor stenosis of the esophagus or the cardia. Stent implantation was performed under slight i.v. sedoanalgesia. RESULTS: Technical success was achieved in all 31 implanted stents. Forty-eight hr after implantation, dysphagia was impro

Spontaneous fracture of the outlet catheter of a totally implanted catheter system (Port-A-Cath).

A case of spontaneous fracture of the outlet catheter of a totally implanted catheter system (Port-A-Cath) is presented. The outlet catheter was fractured at the entrance into the left subclavian vein twenty-one weeks after insertion and the distal part was embolized in the right ventricle. The embolized catheter fragment was retrieved by a 'goose-neck' snare via the right femoral vein. The awareness of a possible spontaneous fracture of the outlet catheter of a totally implanted catheter system (Port-A-Cath) is important to prevent accidental spillage of potent cytotoxic substances.

Percutaneous inferior vena caval filters: follow-up of seven designs in 320 patients.

Three hundred twenty-four percutaneous inferior vena caval (IVC) filters of different designs were placed in 320 patients from April 1985 through June 1992. No acute mortality or substantial morbidity was attributed to filter placement. Radiologic or pathologic follow-up data were obtained in 227 (71%) patients (230 filters); clinical follow-up data only were obtained in 50 (16%) patients (50 filters). One hundred twenty (43%) patients died; post-filter-placement pulmonary emboli (PE) were related to the cause of death in eight. At IVC filter imaging studies, 26 of 137 (19%) filters demonstrat

A strategy for atrial septal defect closure in small children that eliminates long-term wall erosion risk.

OBJECTIVES: To evaluate feasibility, efficacy, and safety of an approach to atrial septal defect (ASD) occlusion in children ≤20 kg that eliminates cardiac wall erosion risk. BACKGROUND: Cardiac wall erosion is a potentially catastrophic complication of ASD device closure. The HELEX Septal Occluder (HSO) is a compliant device with no reports of erosion. The HSO is technically difficult to deploy in smaller children and cannot be used to close larger defects. To eliminate wall erosion risk, we use the HSO when feasible and surgery for larger defects. METHODS: Retrospective review of ASD proce
