A Grading System for Transnasal Flexible Laryngoscopy.

OBJECTIVE: Transnasal flexible laryngoscopy (TFL) is becoming more popular in laryngology clinical practice. There has not been any grading system for TFL to help the physician document and communicate the laryngeal view yet. In this study, we aimed to classify the laryngeal view based on the visualization of the glottic aperture with TFL performed on conscious patients. METHODS: The TFL videos of 920 randomized patients were evaluated by three blind observers experienced with laryngology. The laryngeal view, consisting of the basic anatomic landmarks of the glottis, arytenoids, and epiglotti

Exploring the Neural Bases of Primary Muscle Tension Dysphonia: A Case Study Using Functional Magnetic Resonance Imaging.

UNLABELLED: Primary muscle tension dysphonia (pMTD) is a voice disorder that occurs in the absence of laryngeal pathology. Dysregulated activity of the paralaryngeal muscles is considered the proximal cause; however, the central origin of this aberrant laryngeal muscle activation is unclear. The Trait Theory (Roy and Bless, 2000a,b) proposed that specific personality traits can predispose one to laryngeal motor inhibition and pMTD, and this inhibition is mediated by a hyperactive "behavioral inhibition system (BIS)" composed of limbic system structures (and associated prefrontal connections).

Vocal nodules and edema may be due to vibration-induced rises in capillary pressure.

HYPOTHESIS: Vocal fold vibration may physically raise intravascular pressure to levels high enough to damage capillaries and result in leakage of erythrocytes. This type of injury is commonly seen in benign vocal fold lesions and is not well explained. STUDY DESIGN: Theoretical, retrospective. METHODS: The relationship of intravascular pressure to vibration frequency and amplitude is derived and confirmed with a physical blood vessel model, then applied to published human measurements to estimate human intravascular pressures. RESULTS: Vocal fold intravascular pressure is predicted to have

Prosodic disturbance in aphasia: vowel length in Thai.

The aim of this study is to determine to what extent a phonologically contrastive function of the prosodic feature of length is resistant to impairment in aphasia. The language chosen for investigation is Thai, a language which contrasts short and long vowels. Subjects included two Broca aphasics, one transcortical motor aphasic, one Wernicke aphasic, one conduction aphasic, one nonaphasic right-brain-damaged patient, one nonaphasic cerebellar dysarthric patient, and five normal controls. The subjects read a list of words containing short and long vowels. Vowel durations were measured from spe

Motor endplate-expressing cartilage-muscle implants for reconstruction of a denervated hemilarynx.

OBJECTIVE: Tissue engineering of the larynx requires a complex, multiple tissue layer design. Additionally, spontaneous reinnervation of the larynx after recurrent laryngeal nerve (RLN) injury is often disorganized, resulting in subpar function. This study investigates use of tissue-engineered cartilage and motor endplate-expressing (MEE) tissue-engineered skeletal muscle implants for laryngeal reconstruction and the promotion of organized reinnervation after RLN injury. METHODS: F344 rat primary muscle progenitor cells (MPCs) were isolated. Three-dimensional muscle constructs were created by

Apraxia of speech and phonological errors in the diagnosis of nonfluent/agrammatic and logopenic variants of primary progressive aphasia.

PURPOSE: The International Consensus Criteria for the diagnosis of primary progressive aphasia (PPA; Gorno-Tempini et al., 2011) propose apraxia of speech (AOS) as 1 of 2 core features of nonfluent/agrammatic PPA and propose phonological errors or absence of motor speech disorder as features of logopenic PPA. We investigated the sensitivity and specificity of AOS and phonological errors as markers for these variants and also investigated the relationship between AOS, phonological errors, and findings on C-labeled Pittsburgh Compound B (PiB)-positron emission tomography (PET) imaging associated

Glottic closure patterns: type I thyroplasty versus type I thyroplasty with arytenoid adduction.

OBJECTIVES/HYPOTHESIS: The goal of laryngeal framework surgery in patients with unilateral vocal fold paralysis is to improve glottic closure by medializing the paralyzed vocal fold. Type I thyroplasty (Th) and arytenoid adduction (AA) are two of the most commonly performed procedures. Two of the main rationales for performing an AA are to improve closure of the posterior glottis and correct vertical height discrepancy. The purpose of this study was to evaluate if AA with Th yields better posterior glottic closure and vertical height equality than Th alone. STUDY DESIGN: Retrospective. METHO
