Results of latissimus dorsi tendon transfer for irreparable cuff tears.

Rupture of the supraspinatus and infraspinatus tendon (and teres minor) can cause loss of active external rotation (ER), entailing severe functional disability in daily activities. Latissimus dorsi tendon transfer (LDTT), proposed by Gerber in 1988, appears to be the best adapted solution in these cases of irreparable posterior and superior cuff tears. Between 2001 and 2004, 30 patients were operated on by the technique described by Gerber, with the transfer fixed anteriorly to the subscapularis tendon and laterally to the greater tuberosity by transosseous suture. One patient, subsequently re

Editorial Commentary: Bio-Inductive Collagen Implant Augmentation Shows Long-Term Cost-Effectiveness, But Clinical Patient Outcomes and Careful Patient Selection Must Guide the Path Forward.

Arthroscopic rotator cuff repairs (ARCR) are fraught with low healing rates despite improvements in surgical techniques and constructs. Several studies have emerged showing significant improvements in failure to heal rates when incorporating bioinductive collagen implants (BCI) in the short term. Structural integrity following ARCR is paramount, as retear places exorbitant costs on the health care system and long-term studies have established that clinical outcomes are significantly worse in patients with structural retear. The up-front costs of biologic augmentation is cost-prohibitive in amb

Polypropylene mesh reinforcement does not improve acute biomechanical properties in superior capsular reconstruction: an ovine model study.

BACKGROUND: Superior capsular reconstruction (SCR) is a treatment option for irreparable rotator cuff tears (IRCTs), yet graft failure remains a significant limitation. While fascia (FA) grafts are commonly used, postoperative outcomes can be suboptimal, potentially contributing to higher mechanical failure rates. Polypropylene mesh (PM) augmentation has shown promise in improving soft-tissue repair strength, but its acute biomechanical efficacy in SCR remains insufficiently investigated. This study aimed to evaluate whether PM augmentation enhances the biomechanical stability of FA grafts in

Clinical Outcomes and Complications of Superior Capsular Reconstruction Versus Reverse Shoulder Arthroplasty for Massive Rotator Cuff Tears: A Meta-Analysis.

BACKGROUND: Irreparable rotator cuff tears without advanced arthritis are difficult to manage. Superior capsular reconstruction (SCR) preserves the native joint, while reverse total shoulder arthroplasty (rTSA) offers reliable function but with higher implant-related risks, especially in younger patients. This study compared outcomes and complications between SCR and rTSA. METHODS: This study was conducted following Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) 2020 guidelines. PubMed, Scopus, and Ovid Medline were searched from inception to May 2, 2025, for stud

Acromial Cupping with Lodged Bioinductive Collagen Implant Following Rotator Cuff Repair: A Case Report.

CASE: We report a 39-year-old man with a supraspinatus retear treated with a Regeneten implant. Six months postoperatively, magnetic resonance imaging (MRI) revealed a foreign body lodged at the inferior acromion margin, causing acromial cupping, remodeling, and fibrosis. At the 12-month follow-up, the patient presented symmetrical strength and full range of motion with no pain. CONCLUSION: We report the first case of Regeneten implant migration with acromial lodging. Despite MRI findings, the patient remained asymptomatic with no functional limitations. Close postoperative follow-up is essen

Unravelling the Role of PIEZO1 in Adipogenesis of Fibrogenic/Adipogenic Progenitors for Muscle Fat Infiltration After Rotator Cuff Tear.

BACKGROUND: Muscular fatty infiltration originated from fibrogenic/adipogenic progenitors (FAPs) is a common issue following rotator cuff tear (RCT) that impairs shoulder function. RCT disrupted the biomechanical equilibrium of the shoulder and decreased the mechanical stimuli transmitted to the rotator cuff. Whether mechanical stimuli participate in mediating muscular fatty infiltration after RCT remains unknown. The current study aimed to explore how mechanical environment changes caused by RCT affect muscular fatty infiltration and to identify the potential therapeutic modality. METHODS: H

Effects of seven types of exercise in the treatment of rotator cuff-related shoulder pain (RCRSP): a systematic review and Bayesian network meta-analysis.

BACKGROUND: Rotator cuff-related shoulder pain (RCRSP) is the most common cause of shoulder pain and represents one of the most common musculoskeletal conditions affecting the shoulder. Despite ample evidence supporting the effectiveness of exercise therapy for RCRSP, no consensus exists regarding the optimal exercise intervention. This systematic review aims to evaluate and compare the efficacy of specific versus conventional exercise interventions for improving pain and dysfunction in patients with RCRSP. METHODS: The MEDLINE, Embase, Cochrane Library, CINAHL, SPORTiscus, and Web of Science

Comparative Analysis of Bone Mineral Density of the Lumbar Spine, Hip, and Proximal Humerus in Patients with Unilateral Rotator Cuff Tears.

BACKGROUND: The proximal humerus, a common site for osteoporotic fractures, is frequently overlooked in osteoporosis evaluations. This study aimed to evaluate the relationship between the conventional bone mineral density (BMD) measurement (at the lumbar spine and femur) and the BMD measurement at both proximal humeri (the asymptomatic side and the side with a rotator cuff tear [RCT]) in patients with unilateral RCT. Furthermore, we investigated clinical features indicative of osteoporosis in RCT patients and assessed the utility of proximal humerus BMD measurements. METHODS: From April 2020

Arthroscopic distal clavicle resection versus conservation in patients with combined rotator cuff tears and acromioclavicular joint osteoarthritis.

PURPOSE: The most common cause of shoulder pain originating from the acromioclavicular (AC) joint is osteoarthritis, causing pain and disability. Operative Management of AC arthritis includes arthroscopic distal clavicle resection (DCR) and open clavicle resection. This study was conducted to evaluate the outcomes of isolated rotator cuff repair with conservative treatment of ACJ arthritis versus the combined resection of the distal clavicle with the repair of a rotator cuff tear, in cases with acromioclavicular arthritis. METHODS: A total of 46 patients with unilateral or bilateral combined

Does improved external rotation following reverse shoulder arthroplasty impact clinical outcomes in patients with rotator cuff pathology and external rotation less than 0°?

BACKGROUND: The purpose of this study was to (1) evaluate whether improved external rotation (ER) in patients with preoperative ER <0° impacts their clinical outcomes following reverse shoulder arthroplasty (RSA) for rotator cuff (RC) pathology and (2) describe the differences in preoperative factors and postoperative outcomes in this patient population. Our hypothesis was that clinical outcomes would not be affected by improvement in ER using a lateralized glenosphere design. METHODS: We retrospectively reviewed 55 patients with preoperative ER <0° who underwent primary RSA for RC pathology

Compensatory Movement Patterns Are Based on Abnormal Activity of the Biceps Brachii and Posterior Deltoid Muscles in Patients with Symptomatic Rotator Cuff Tears.

BACKGROUND: Abnormal movement patterns due to compensatory mechanisms have been reported in patients with rotator cuff tears. The long head of the biceps tendon may especially be overactive and a source of pain and could induce abnormal muscle activation in these patients. It is still unknown why some patients with a rotator cuff tear develop complaints and others do not. QUESTIONS/PURPOSES: (1) Which shoulder muscles show a different activation pattern on electromyography (EMG) while performing the Functional Impairment Test-Hand and Neck/Shoulder/Arm (FIT-HaNSA) in patients with a symptomat

Calcifying tendinitis of the long head of the biceps brachii and superior labrum: a report of one case and literature review.

Calcifying tendinitis of the shoulder is characterized with the formation of the calcium deposits in tendon, which mostly involves supraspinatus. The formation of the calcium deposits may be related to the excessive mechanical stimulation and non-tenocytes generation from tendon stem cell. The disease is usually associated with pain, especially when activating the shoulder. We describe one case of calcifying tendinitis involving the long head of the biceps brachii and superior labrum. The related literature and researches are also reviewed. After reviewing the past articles, there are no simil
