OBJECTIVES/STUDY DESIGN: Administration of group B streptococcal (GBS) antibiotic prophylaxis to women in labor has dramatically reduced the incidence of GBS neonatal disease, but there is little information on its impact on neonatal infections caused by other organisms. We conducted a nested case-control study to define the association between maternal intrapartum antibiotics and risk of neonatal non-GBS infection.
RESULTS: In our study population, 114 of 13,224 infants had 115 non-GBS infections. The incidence of non-GBS neonatal infections fell during the study period, ranging from an atta

Vaginal colonization by Group B streptococci (GBS) during pregnancy is associated with lige-threatening neonatal infections acquired during passage through the birth canal. Factors associated with an increased risk of GBS transmission to the neonate include prematurity, premature spontaneous rupture of the membranes before 37 weeks of gestational age, prolonged (> 12 h) rupture of the membranes at full term, fever in the mother, multiple pregnancy, and a history of GBS infection. A study was conducted to evaluate the performance characteristics of a rapid GBS screening test comparatively with

PURPOSE: To assess the effect of group B streptococcal (GBS) meningitis on retinal blood flow (RetBF) and choroidal blood flow (ChBF) autoregulation in sedated newborn piglets (1 to 5 days of age).
METHODS: Fourteen study animals injected with 0.5 ml heat-killed GBS (10(9)) were compared to 10 control animals injected with 0.5 ml saline. The site of injection for both groups was the cerebral lateral ventricles. RetBF and ChBF were measured by radioactive microspheres (141Ce, 51Cr, 113Sn, 85Sr, 95Nb, 46Sc) over a mean arterial blood pressure (MABP) range of 20 to 150 mm Hg. Hypertension and hy

A 21 year old primigravida with a twin pregnancy developed Pasteurella multocida chorioamnionitis. Infection occurred at 27 weeks gestational age after prolonged rupture of membranes. The twin in the separate sac presenting proximal to the cervix suffered infection and died shortly after birth whereas the other twin was not infected. The bacterium is believed to have caused ascending infection from asymptomatic colonization of the vaginal tract.

BACKGROUND: In-vitro and clinical efficacy of a combination therapy consisting of 3 antibiotic agents was to be assessed in neonatal septicemia.
METHODS: From 1980 to 1987, 152 newborns with septicemia as proven by blood culture were treated with an initial antibiotic regimen consisting of azlocillin (150 mg/kg bw), cefotaxime (100 mg/kg bw), and tobramycin (5 mg/kg bw).
RESULTS: According to the microbiologic testing, antimicrobic therapy was effective in each of the 152 organisms: 101/152 bacteria were susceptible to all 3 agents; resistance to 1 or 2 of the antibiotics was evident in 33/1

Aspiration or ingestion of contaminated amniotic fluid or vaginal secretions has been suggested as a cause of systemic group B streptococcal (GBS) infection in the neonate. Suckling rat studies disagree on whether systemic disease will develop after an oral challenge of GBS. Our goal was to determine if systemic GBS disease would occur following oral colonization in the suckling rat and the effect of bacterial, host and environmental factors. Suckling rat littermates received oral inoculation on one of the first four days of life with varying doses and strains of GBS. Studies confirmed gastric

In a histopathological study, from a homogeneous Swedish population with a better than average socio-economic standard, placentas from 161 singleton infants small for gestational age (SGA) and 322 singleton infants appropriate for gestational age (AGA) were examined for the presence of inflammation in the placental parenchymal membranes, cord and decidua. Acute chorioamnionitis including funiculitis was found in 13.4% of the total material, in 17% of term AGA-infants (GA, greater than or equal to 38 weeks), in 13% of preterm AGA-infants (less than or equal to 37 weeks) and in 6.8% of SGA-infan

Three patients with vulvovaginitis caused by Trichomonas vaginalis, which was refractory to conventional treatment with metronidazole are described. The T vaginalis strain isolated from one patient was resistant to metronidazole (minimum inhibitory concentration (MIC) more than 100 mg/l) under aerobic conditions, although under anaerobic conditions it was as susceptible as a normal reference strain. The effect of the concomitant use of other medication and the influence of other vaginal pathogens on the efficacy of metronidazole are highlighted.

Chorioamnionitis is a frequent cause of premature labour and delivery, as well as of maternal and neonatal mortality. Group B streptococcus (GBS) has emerged over the past decade as a common pathogen in the etiology of neonatal sepsis. The case of chorioamnionitis reported here is unusual for three reasons: the premature labour was associated with intact membrane and amniotic fluid infected with GBS; all 5 infants were contaminated with GBS and all infants survived. Chorioamniotitis with intact membranes raises some questions regarding the antepartum use of steroids (potent anti-inflammatory a

Predisposing factors and clinical presentations associated with early-onset group B streptococcal (GBS) infections in 60 infants were reviewed. Over a three-year period, the incidence of these infections in The Royal Women's Hospital, Melbourne, was 2.0 per 1000 births. The clinical presentation ranged from fulminating sepsis to asymptomatic bacteraemia. In some cases, there was evidence of intrauterine fetal infection, despite intact membranes and a lack of clinical evidence of maternal infection. However, the over-all incidence of fever, before and after delivery, among the mothers of these

OBJECTIVE: In women with Group B Streptococcus (GBS) vaginal colonization who report a beta-lactams allergy, the prevention of neonatal GBS early-onset disease (EOD) mainly relies on maternal antibiotic prophylaxis with lincosamides during labor. With approximately 30% of GBS being resistant to macrolides or lincosamides, this antibiotic prophylaxis may be inappropriate, leading to a risk of EOD. We aim to measure the proportion of inappropriate antibiotic prophylaxis despite the existence of an antibiotic susceptibility testing, and its possible impact on the newborn.
METHODS: Single-center

OBJECTIVE: The impact of maternal group B Streptococcus (GBS) colonization on outcomes of expectant management in late preterm prelabor rupture of membranes (PPROM) is debated. This study examined maternal and neonatal infectious outcomes in women with late PPROM at 34-36.6 weeks of gestation, stratified by GBS colonization status.
METHODS: This retrospective cohort study included 412 women admitted for expectant management of late PPROM. Outcomes were compared between GBS-positive (n = 73) and GBS-negative (n = 339) groups. Co-primary outcomes were clinical chorioamnionitis and latency durat

INTRODUCTION: Group B Streptococcus (GBS) causes infections in women during pregnancy and puerperium and invasive infections in newborns. The genes lmb, cylE, scpB, and hvgA are involved with increased virulence of GBS, and hypervirulent clones have been identified in different regions. In addition, increasing resistance of GBS to macrolides and lincosamides has been reported, so knowing the patterns of antibiotic resistance may be necessary to prevent and treat GBS infections. This study aimed to identify virulence genes and antibiotic resistance associated with GBS colonization in pregnant w

BACKGROUND: In non-pregnant adults, the incidence of invasive Group B Streptococcus (GBS) disease is continuously increasing. Elderly and immunocompromised persons are at increased risk of infection. GBS commonly colonizes the vaginal tract, though data on colonization in the elderly are scarce. It is unknown whether the prevalence of GBS colonization is increasing in parallel to the observed rise of invasive infection. We conducted a three-year (2017-2019) prospective observational cross-sectional study in two teaching hospitals in Switzerland to determine the rate of GBS vaginal colonization
