PURPOSE: To determine the efficacy and postoperative complications of complex deep lamellar endothelial keratoplasty (DLEK) when used for th … ve visual acuity to a functional level in most patients. DLEK can be used successfully in eyes with aphakia and other anterior segment abnormalities without a high risk of graft dislocation or failure. Mild postoperative complications usually occurred early after DLEK and did not affect visual outcomes and graft survival if managed appropriately.

METHODS: Children born in four of the western counties of Sweden who were diagnosed with congenital cataracts formed a cohort (n=137). We implanted sulcus-fixated posterior chamber intraocular lenses (PCL) in three cataractous, six aphakic, and four eyes during penetrating keratoplasty due to corneal opacity in the absence of capsular and zonular support. This technique has been successfully performed in all cases and produced good visual outcome in 12 eyes (93%). In four eyes which had penetrating keratoplasty and the fixation of PCL, one developed vitreous hemorrhage and one developed corneal graft rejection.

Several transscleral suturing techniques require intricate surgical maneuvers and are associated with an increased risk for surgical trauma.
