Lemierre's syndrome and septicaemia caused solely by Arcanobacterium haemolyticum in a young immunocompetent patient.

We present a case of septicaemia caused by Arcanobacterium haemolyticum in a previously healthy 23-year-old man suffering from acute pharyngotonsillitis, who developed complicated Lemierre's syndrome. Three blood cultures (both aerobic and anaerobic) revealed the exclusive presence of A. haemolyticum. The presence of Fusobacterium necroforum was not essential for the development of this pathology. To our knowledge, this is the first reported case of Lemierre's syndrome caused solely by A. haemolyticum. We confirm that this organism must be considered a potential pathogen in immunocompetent pat

Bilateral mycotic cerebral abscess due to aspergillosis--a case report.

Aspergillosis of central nervous system is an uncommon infection mainly occurring in immunocompromised patient. It may be present in several forms: abscess, meningitis, mycotic aneurysm, infarction and in tumoral form. Here we report a case of cerebral aspergillosis presenting as bilateral frontal lobe abscess without evidence of any underlying systemic disorder or extracranial disease.

Complications of otitis media: an evolving state.

The advent of antibiotics in the 1930s and 1940s has led to a decline in the incidence of otitis media complications. However, otitis media and its complications are continuing to evolve owing to the emergence of antibiotic resistance and the introduction of pneumococcal vaccines. Modern high-resolution computed tomographic scans have allowed for earlier diagnosis when signs and symptoms are present, but a high index of suspicion must be maintained because the clinical presentation may be quite subtle. The intent of this article is to discuss the intracranial and extracranial complications of

Disseminated and cerebral infection due to Nocardia farcinica: diagnosis by blood culture and cure with antibiotics alone.

Systemic infections with Nocardia species continue to be a serious threat to immunosuppressed hosts. Diagnosis of these infections can be difficult despite their known tendency for cerebral and subcutaneous involvement. We describe a patient who presented with nonspecific constitutional symptoms and was found to have subcutaneous and cerebral abscesses due to Nocardia farcinica. In addition, a blood culture yielded the organism. The patient responded remarkably to oral therapy; resolution of the cerebral disease was observed on serial magnetic resonance images. We discuss the important clinica

MRI appearances of Listeria rhombencephalitis.

Listeria monocytogenes is a rare cause of central nervous system infection in the non-immunocomprised patient and 10% of these patients develop a rhombencephalitis. We present such a case and discuss the clinical, pathological and radiological features.

Using a ventricular reservoir to instill amphotericin B.

Intrathecal administration of amphotericin B is the best method of eradicating intracranial fungal infections. The Ommaya reservoir provides an easy and practical method for fungicidal medication administration. Treatment of coccidioidomycosis with amphotericin B may be accomplished via an Ommaya reservoir. Astute nursing care is essential to prevent complications associated with this procedure.

Mucor cerebral abscess associated with intravenous drug abuse.

We report on a case of a 26-year-old intravenous narcotic abuser with a primary cerebral mucormycotic abscess caused by Rhizopus oryzae. He was treated with a combination of intravenous and intraventricular amphotericin B and surgical drainage with a successful outcome. There was no evidence that his infection was acquired by the rhinocerebral route, it seems likely that he injected himself with a contaminated batch of narcotic or amphetamine. Mucormycosis presenting in this way has been described previously but this is only the second such case to survive. Early diagnosis and treatment is ess

[Cerebral abscess and Osler-Rendu disease. Apropos of 4 cases].

The authors report 4 cases of cerebral abscesses (C.A.) complicating Rendu Osler disease (hereditary haemorrhagic telangiectasia: H.H.T.). These cases and 43 others comparable found in literature enable to assess C.A. complicates H.H.T. by the way of pulmonary arteriovenous fistulas (P.A.V.F.). C.A. appears during known H.H.T. in only a quarter of cases; in all other cases C.A. reveals the disease. When the etiological search for metastatic C.A. fails to find any classical cause: O.R.L., stomatologic, cardiac, it seems important to suspect H.H.T., and research P.A.V.F. by chest X ray and pulmo

Epidemiology of central nervous system infections in Olmsted County, Minnesota, 1950-1981.

We identified all diagnosed cases of infections of the central nervous system (CNS), excluding poliomyelitis, in the population of Olmsted County, Minnesota, from 1950 to 1981 and described incidence, time trends, etiologic agents, and mortality for these infections. The adjusted incidence rate for bacterial meningitis was 8.6/100,000 person-years (with a case fatality ratio of 10%) and was highest in children less than five years of age; in this age-group, rates more than doubled from 1950 to 1981. The adjusted incidence rate of brain abscess was 1.1, with a case fatality ratio of 37%. The ad…

[Aspergillosis of the brain].

A case of aspergillosis of the brain in a 49-year old woman is described. In view of the fact that there were no inflammatory changes in other organs, the assumption was put forward that the fungus had penetrated into the brain by the otogenic way, or through the accessory nasal sinuses. The results of experimental infection of laboratory animals aimed at obtaining a model of meningoencephalitis are discussed. It was shown that spores of the Aspergillus fumigatus fungus in low concentrations introduced into the otic veins penetrated into the lungs where they caused an inflammatory process, and

Diagnosis and treatment of abscess of the central ganglia.

The authors present four cases of abscess of the thalamus and basal ganglia and review an additional 10 cases from the literature. These abscesses appear to be metastatic and are usually associated with a cardiac septal defect or a preceding intrathoracic infection. Fever, elevated white blood count, and/or meningismus, in combination with angiography demonstrating an avascular mass, help distinguish abscess from tumor in this location. The authors advocate aspiration rather than excision.
