Orthopaedic manifestations of invasive group A streptococcal infections complicating primary varicella.

The incidence of invasive group A streptococcal (GAS) infections in primary varicella appears to be increasing. GAS infections complicating varicella range from cellulitis, abscess, and septic arthritis to life-threatening necrotizing fasciitis and pyomyositis in association with GAS toxic shock syndrome (TSS). Four patients admitted in 1 year to the Children's Hospital and Medical Center in Seattle, whose care included evaluation and treatment by the Orthopaedic service, are presented to illustrate this spectrum. Three had a delay in diagnosis, including discharge from previous emergency depa

The susceptibility of young adult Americans to vaccine-preventable infections. A national serosurvey of US Army recruits.

OBJECTIVE: Due to recent resurgences of measles, mumps, and rubella among young US adults, we sought to generate antibody prevalence data for national and military immunization policy evaluations. DESIGN: We used a questionnaire and serological survey of Army recruits to assess antibody status to measles, mumps, rubella, and varicella by enzyme-linked immunosorbent assay and to poliovirus types 1, 2, and 3 by microneutralization assay. SETTING: Basic training reception centers at Fort Benning, Ga., and Fort Jackson, SC. PATIENTS: The study included 1547 US Army recruits who were inducted du

Acyclovir treatment of varicella in otherwise healthy children.

STUDY OBJECTIVE: To determine whether acyclovir administered orally affects the duration and severity of varicella in otherwise normal children. DESIGN: Randomized, placebo-controlled, double-blind trial. SETTING: Patients' residence and university hospital clinic. PATIENTS: One hundred five children between 5 and 16 years of age with laboratory-confirmed varicella entered the study. Of the 102 who were included in the final analysis, 50 received acyclovir and 52 received placebo. INTERVENTION: Placebo or acyclovir was given orally four times daily, for 5 to 7 days. The acyclovir dose was

Biologic and immunologic characterization of the soluble skin-test antigen of varicella-zoster virus.

A sensitive method for assay of the potency of the skin-test antigen of varicella-zoster virus was established. Potency was determined by a reverse passive hemagglutination test, and the results correlated well with the cutaneous reaction of sensitized guinea pigs and an immune human adult to the antigen. Cutaneous reactivity was not induced by repeated inoculation of the antigen, but humoral neutralizing antibody was detected in most guinea pigs when a large amount of antigen was used. No cutaneous cross-reactivity between varicella-zoster virus and herpes simplex virus was noted; however, po

The diagnostic value of varicella-zoster virus (VZV)-specific antibodies in the early phase of varicella in immunocompromized children.

Varicella-zoster virus (VZV)-specific antibodies were tested by indirect membrane immunofluorescent technique in sera of 32 immunocompromized children during the first week after onset of varicella. VZV-specific IgM antibodies were found in sera of all but four patients. Three of the four IgM negative patients were followed up, and it was demonstrated that all of them gave a retarded antibody response. They had severe leukopenia and had several relapses of their VZV-infection. The results indicate (a) that the demonstration of VZV-specific IgM antibodies is of diagnostic value even in atypical

Varicella dendritic keratitis.

A 7-year-old boy and a 3-year-old girl suffered from unilateral disciform keratitis and iritis associated with varicella. While they were treated with topical corticosteroid, idoxuridine, and atropine drops, dendritic lesions typical of herpes zoster appeared four months after the onset of eruptive skin lesions. Using the direct immunofluorescent method, we showed varicella-zoster virus antigen in the epithelial cells scraped from the dendritic lesion.

Congenital varicella associated with multiple defects.

Only two previous reports in the medical literature record the association of multiple congenital defects in the baby and varicella in the mother during the first trimester of pregnancy. The case is reported of a female infant born to a mother who contracted varicella in the 11th week of pregnancy. The infant was premature, small for dates, and had skin and localized muscular defects and respiratory difficulty. Subsequently she was found to be retarded. She failed to thrive and was subject to frequent infections. Further investigation revealed a unilateral diaphragmatic weakness, scoliosis and

Evaluation of selected parameters of immune response to rubella virus, hepatitis B virus and varicella-zoster virus infections in children born to mothers after kidney or liver transplantation.

BACKGROUND: The immune status of children exposed prenatally to immunosuppressants is not fully understood. MATERIAL AND METHODS: A single- … to 1 year) and children aged >1 year. Immunoenzymatic tests were used to analyze the titers of the chickenpox virus (VZV-IgG), rubella (RuV-IgG) and hepatitis B virus (HBV, HBsAb). The studied differences were compared depending on the age group and the immunosuppressive regimen used by the pregnant mother. RESULTS: In neonates born to mothers after liver …

Sudden, unexpected death of a 15-year-old boy due to pancarditis: A case report and possible etiopathogenesis.

BACKGROUND: Generally, rheumatic heart disease is, today, sporadic in developed countries, even though it continues to be a major health haz … Blood specimens were also processed through PCR, in order to reveal the presence of Enteroviruses, Chickenpox virus, Epstein Barr virus. Also chemical-toxicological examinations for the detection of the main medications and drugs were performed on blood samples. RESULTS: The anamnesis, collected before the autopsy, revealed an acute pharyngitis few weeks b …

Varicella-zoster virus-derived major histocompatibility complex class I-restricted peptide affinity is a determining factor in the HLA risk profile for the development of postherpetic neuralgia.

UNLABELLED: Postherpetic neuralgia (PHN) is the most common complication of herpes zoster and is typified by a lingering pain that can last … ent of the studied HLA allele. IMPORTANCE: Varicella-zoster virus can cause two distinct diseases: chickenpox (varicella) and shingles (herpes zoster). Varicella is a common disease in young children, while herpes zoster is more frequent in older individuals. A common complication of herpes zoster is postherpetic neuralgia, a persistent and debilitating pai …
