Mucormycosis in immunocompetent patients: a case-series of patients with maxillary sinus involvement and a critical review of the literature.

OBJECTIVES: To review the current literature on mucormycosis in immunocompentent/otherwise healthy individuals, to which five new cases with maxillary sinus involvement have been added. METHODS: We searched in the PudMed database all articles in the English language related to human infections caused by fungi of the order Mucorales, in immunocompetent/otherwise healthy patients, starting from January 1978 to June 2009. In addition, we updated the literature by reporting five new cases diagnosed and treated at the oral medicine unit of our institution. RESULTS: The literature review showed at

Bacteriologic evaluation of sinus aspirates taken by balloon catheter devices in chronic rhinosinusitis: preliminary study.

PURPOSE: Chronic rhinosinusitis (CRS) is known to be a polymicrobial infection involving both aerobes and Gram-positive and Gram-negative anaerobes. Accurate bacterial evaluation by adequate culture methods can justify subsequent antimicrobial strategies. METHODS: Two specimens were obtained from each of 10 patients undergoing catheter-based Balloon Sinuplasty™, one from the middle meatus (endoscopic approach) and the other from the sinus (catheter-based approach). RESULTS: The bacterial culture from the middle meatus was positive in 9 of 10 patients, including 6 different aerobes without an

Clinical outcomes of sinus floor augmentation for implant placement using autogenous bone or bone substitutes: a systematic review.

BACKGROUND: To date, there are still no clear cut guidelines for the use of autogenous bone or bone substitutes. AIM: The aim of the present review was to analyze the current literature in order to determine whether there are advantages of using autogenous bone (AB) over bone substitutes (BS) in sinus floor augmentation. The focused question was: is AB superior to BS for sinus floor augmentation in partially dentate or edentulous patients in terms of implant survival, patient morbidity, sinusitis, graft loss, costs, and risk of disease transmission? MATERIALS AND METHODS: The analysis was li

Correlation between maxillary sinus floor topography and related root position of posterior teeth using panoramic and cross-sectional computed tomography imaging.

OBJECTIVE: This study correlated the topographic relationship of the maxillary sinus floor to the roots of posterior teeth imaged by panoramic radiography and cross-sectional computed tomography (CT). STUDY DESIGN: Paired panoramic radiographs and CT images of maxillae from 80 subjects were analyzed. A total of 422 maxillary roots were classified according to their topographic relationship to the maxillary sinus and measured according to their projection lengths on the sinus cavity for both imaging modalities. Correlations between the 2 radiographic techniques were examined statistically. RE

Evaluation of the Proximity of the Maxillary Teeth Root Apices to the Maxillary Sinus Floor in Romanian Subjects: A Cone-Beam Computed Tomography Study.

Background/Objectives: Among the paranasal sinuses, the maxillary antrum holds unique clinical relevance due to its proximity to the alveolar process of the maxilla, which houses the teeth. This study aimed to evaluate the position of the root apices of the maxillary canines and posterior teeth relative to the maxillary sinus floor in Romanian subjects. Methods: Data for the study were retrospectively obtained from cone-beam computed tomography (CBCT) scans. The evaluation considered the pattern of proximity to the sinus floor for each tooth type, comparisons of the sinus relationships of teet

What Is the Most Effective Rehabilitation Method for Posterior Maxillas With 4 to 8 mm of Residual Alveolar Bone Height Below the Maxillary Sinus With Implant-Supported Prostheses? A Frequentist Network Meta-Analysis.

PURPOSE: The most effective rehabilitation method for patients with edentulous posterior maxillas with an intermediate (4 to 8 mm) residual bone height (RBH) below the maxillary sinus is unclear. Evidence derived from conventional meta-analysis is limited because of the lack of head-to-head studies. This network meta-analysis (NMA) was performed to identify the most effective method to treat patients with intermediate posterior RBH. MATERIALS AND METHODS: An NMA of randomized controlled clinical trials (RCTs) was conducted to assess various rehabilitation methods using implant-supported prost

Angiomatous Nasal Polyps in a 43-Year-Old Female From North-Central Nigeria: A Case Report.

Angiomatous nasal polyps (ANPs) are uncommon variants of nasal polyps, which, like their more common counterparts, are a significant and common cause of morbidity for patients, regardless of demographic variations. As a group, nasal polyps are non-neoplastic and commonly arise within the maxillary antrum, sphenoid, and ethmoid sinuses, protruding via the ostia into the nasal cavity. Their precise cause is unknown, but several factors related to genetics, local infections, family history, and environment have been implicated in their etiopathogenesis. ANPs are commonly diagnosed in the younger

Radiologic overview of sinonasal lesions.

Sinonasal tumors are often malignant and comprise approximately 3% of all head and neck malignancies. Half of these tumors arise in the nasal cavity, and other common locations of origin include the ethmoid and maxillary sinuses. Some unique clinical features are anosmia and altered phonation but the most common general features include headache, epistaxis, and diplopia. CT and MRI may be used to assess tumor location, invasion of adjacent tissue, presence of metastasis, internal tumor heterogeneity, and contrast enhancement. Local invasion of the tumor beyond the sinonasal tract can impact ad

Mucormycosis causing oral and nasal osteonecrosis in a case of acute myeloid leukemia.

AIMS: Mucormycosis is a rare and aggressive fungal infection with a high mortality rate because of its rapidly progressive and destructive nature. The oral cavity is often affected under opportunistic conditions. We report a 34-year-old woman diagnosed with acute myeloid leukemia complained of slight swelling on the right side of her face with toothache and gingival swelling. An incisional biopsy was performed, and the specimen analysis revealed broad aseptate hyphae with a ribbon-like appearance, which is characteristic of opportunistic Mucorales infection. METHODS AND RESULTS: The oral lesi

Inflammatory Pseudotumor in the Nasal Cavity and Sinuses: A Case Report and Associated Literature Review.

Inflammatory pseudotumor (IP) is a clinically aggressive but histologically benign condition of unknown cause. It rarely appears in the nasal cavity and sinuses. Here, we describe a 24-year-old female with the main symptom of right nasal obstruction. Examinations showed dilation in the right maxillary sinus and a pale neoplasm in the nasal cavity. The neoplasm was completely excised under endoscope. Postoperative pathology showed significant proliferation of plasma cells and lymphocytes, indicating the presence of IP. No recurrence was found during 20 months of follow-up. Only 28 cases (10 mal

Metastases to nasal cavity and paranasal sinuses.

The sinonasal cavities are rare locations for metastases. Metastases to these locations are usually solitary and produce similar symptoms to those of a primary sinonasal tumor. Nasal obstruction and epistaxis are the most frequent symptoms. The maxillary sinus is most frequently involved. The most common primary tumor sites to spread to this region originate in the kidney, breast, thyroid, and prostate, although any malignancy could potentially lead to a metastasis to the paranasal sinuses. The patient's prognosis is usually poor because of the fact that the sinonasal metastasis is usually ass

Sinolith: a rare isolated sphenoid sinus lesion.

Sinolith is a calculus in the paranasal sinuses. It has been also known as antrolith, rhinolith, antral calculi, antral stone, or antral rhinolith. The pathogenesis of calculi formation within a paranasal sinus is still not known. Chronic infection, foreign material, poorly draining sinus, and fungal infection are the main predisposing factors. Isolated sphenoid sinus lesions are rare, and most of them are inflammatory diseases. The main symptom of sphenoid sinus lesions is headache. Headache may be the only symptom of sphenoid sinus lesions. Sinolith is mostly encountered in the maxillary sin

Esthesioneuroblastoma arising from the middle meatus.

A 35-year-old female presented with 13-year history of unilateral recurrent nasal mass, epistaxis and facial pain. Nasal examination revealed a pale glistening mass in the right nasal cavity. On probing, mass was insensitive to touch and bled on handling. Computed tomographic scan showed a mass filling the right nasal cavity, ipsilateral maxillary and ethmoid sinuses. Diagnosis of pansinusitis polyposis was made.Transnasal endoscopy-assisted excision of the mass was done, and the diagnosis of olfactory neuroblastoma was established by histopathology and confirmed by immunohistochemistry. Resurgence of tuberculosis: a rare case of primary orbitomaxillary tuberculoma.

BACKGROUND: Extrapulmonary sites of tuberculosis, which represent 15% of all sites, most commonly involve lymph nodes and bones. Facial bones and paranasal sinuses are rarely involved in the process. Tuberculosis of maxilla is an extremely rare condition, which usually presents with involvement of anterior wall of the maxillary sinus. Symptoms may include nasal discharge with partial nasal obstruction and mild pain. It still remains an underdiagnosed entity. CASE REPORT: We report a case of tuberculoma of the whole left maxilla extending into the orbit, with no previous history of pulmonary t

Identification of an invasive infection of R. oryzae in a haematological patient using a molecular technique.

We report a case of cerebral mucormycosis in a 28-year-old male who was affected by chronic myeloid leukaemia and underwent allogeneic bone marrow transplantation. Nine months post-transplantation, he was admitted to the hospital with fever, bilateral eyelid oedema and neutropenia. X-ray analysis showed numerous areas of pulmonary parenchymal thickening, and a computed tomography scan of the brain showed inflammation of the frontal, maxillary, ethmoidal and sphenoidal sinuses and diffuse swelling of the periorbital tissues. Sinus cultures were taken, and based on its characteristic rhizoid str

Lessons to be learned from a complicated case of rhino-cerebral mucormycosis in a renal allograft recipient.

Fungal infections still represent a serious complication after organ transplantation. Early diagnosis and aggressive treatment are crucial. Because of the many diagnostic problems involved, we present a case of mucormycosis--primarily affecting the paranasal sinuses with later intracranial extension--in a highly immunized recipient of a third renal transplant. Although fungal infection was suspected from various imaging techniques, only the detection of typical fungal hyphae in the infected tissue was diagnostic. Neither the blood tests and cerebrospinal fluid examinations performed nor cultur

[Cementoma of the skull base].

Cementomas result from the odontotic epithelial-mesenchymal complex and are located mainly in the area of large and small molar teeth of the mandible. There was an extremely rare cement location in the analyzed clinical cases. In the first case, cementoma of the frontal sinus spread into the ethmoidal sinus and eye socket in a female patient aged 38 years. The tumor is manifested by right frontalorbital pain and exophthalmos. There were no problems in removing the tumor and repairing the defect of the skull base. The second case was an extensive tumor in a 11-year-old child, which involves the

Carotid artery-sygmoid sinus fistula: a rare complication of gunshot wound on the base of the cranium.

Vascular lesions without clinical manifestation may occur in cranial-facial wounds produced by bullets that course the base of the cranium. This work describes a rare kind of vascular complication in cranial-facial gunshot wound. The authors present the case of a patient, the victim of a cranium-maxillary gunshot wound. Carotid angiography revealed a carotid-sygmoid sinus fistula that filled the sygmoid and transverse sinuses, concomitant to the arterial angiographic phase. A direct communication between the external carotid artery and the sygmoid sinus was disclosed. We are not aware of any o

Malignant sinonasal epithelioid schwannoma.

Malignant schwannomas are rare neoplasms that are seldom found in the head and neck. Few cases have been reported involving paranasal sinuses and none of them was of the "epithelioid" type. In this report, an unusual case of epithelioid malignant schwannoma involving the maxillary sinus, nasal cavity and orbit is presented. The patient was a 27-year-old male with a history of headache, nasal obstruction and epistaxis. Histologically, the tumour had a biphasic pattern with spindle and epithelioid elements which led to a differential diagnosis with malignant melanoma. It had also to be distingui
