RDoC-informed explainable AI as a paradigm for multilevel Alzheimer's disease diagnosis and progression prediction: a systematic review.

Explainable Artificial Intelligence (XAI) is gaining popularity in early diagnosis and monitoring of dementia. Herein, we recommend the incorporation of the National Institute of Mental Health's Research Domain Criteria (NIMH-RDoC) framework with XAI-informed diagnostic protocols to help establish diagnosis at early stages of Alzheimer's disease (AD). RDoC has a dimensional structure that extends across units of analysis from genes and molecules to circuits, physiology, behavior, and introspection. By restructuring diverse features as inputs including apolipoprotein E (APOE) genotype, amyloid

Napping, sleep apnea, and cognitive function: cross-sectional results from the U.S. POINTER-zzz sleep ancillary study.

BACKGROUND: The relationship between the duration of napping and cognitive function is unclear. We examine this association by analyzing baseline data from the US POINTER-zzz Sleep Ancillary Study. METHODS: This study evaluates sleep assessments obtained via home apnea-screening device, wrist actigraphy, and sleep diaries among 686 older adult participants. Naps were defined as sleep periods with at least 30 continuous minutes outside of the primary period of sleep or the longest sleep duration. We conducted linear regression analyses to test the association between napping minutes and baseli

Biomarkers of the blood-brain barrier and the neurovascular unit and their relationship with cognitive decline and cognitive frailty.

Aging is accompanied by an increasing prevalence of frailty and age-related cognitive decline (CD). Cognitive frailty (CF), characterized by physical frailty and cognitive impairment, has been associated with oxidative stress, neuroinflammation, mitochondrial dysfunction, and cardiovascular impairment. However, its biological foundations remain only partially understood. This review synthesizes evidence on biomarkers associated with the blood-brain barrier (BBB) and neurovascular unit (NVU) in CD and CF. These biomarkers are associated with endothelial injury, pericyte damage, BBB permeability

Resilience Through Social Connectedness and Cognition: Is Theory of Mind a Form of Enrichment for Older adults?

OBJECTIVE: Social connectedness is a modifiable lifestyle factor that delays age-related cognitive decline. Using cross-sectional, longitudinal, and experimental approaches, we examined whether theory of mind-inferring what others think or feel-is a potential mechanism underlying this relationship. METHODS: In Study 1, 305 community-dwelling older adults participating in two different, but related, studies completed comprehensive measures of general cognition, theory of mind, and personal social networks. We examined whether theory of mind mediated the relationship between older adults' socia

Correlation between serum YKL-40 and VILIP 1 levels and brain volume in dementia patients.

AIM: Alzheimer's disease (AD) is a chronic, progressive cognitive disorder characterized by prominent episodic memory impairment. The contribution of neuronal damage and neuroinflammation to this process has been investigated by measuring various substances. Two of the most promising substances in serum and plasma studies are visinin-like protein 1 (VILIP-1) and tyrosine (Y), lysine (K), leucine (L)-40 (YKL-40). These markers may lead to early diagnosis and new treatment options. METHODS: Serum VILIP-1 and YKL-40 levels were analyzed in 33 probable AD patients and 23 healthy controls. Cranial

Music and hypnosis for well-being in retirement homes: A pilot study.

Anxiety, pain and poor well-being are major issues in elderly individuals. Complementary interventions, such as music and hypnosis, are increasingly used to address these issues. The objectives of this study were to assess immediate changes in anxiety, pain and well-being during personalized prerecorded music and hypnosis interventions compared to control sessions, and to explore participants' subjective experiences. We employed a multiple time series model with daily measurements with older people living in retirement homes in rural areas (n = 8). The Edmonton Symptom Assessment Scale (ESAS)

Association of neuroticism with incident dementia and cognitive function: 26-year follow-up of EPIC-Norfolk study.

BACKGROUND: Neuroticism is a stable personality trait associated with increased vulnerability to mental and physical disorders. This study examined whether neuroticism is associated with the risk of dementia, particularly across the adult life course and over long-term follow-up. METHODS: We analysed data from 19,678 dementia-free participants (mean [standard deviation, SD] age, 60.8 [9.3] years) who had neuroticism assessed between 1996 and 2000. Incident dementia was identified via linked hospital inpatient, mental health and mortality records through 2022. Cox proportional hazards models e

Cognitive profiles among dementia-free older adults and their associations with neuroimaging markers.

Prior studies have demonstrated the existence of cognitively-defined subgroups among dementia free older adults, however, it is unclear whether such subgroups are characterized by distinct neuroimaging measures of brain function and structure. To address this gap, the current study used latent profile analysis (LPA) to identify cognitively-defined subgroups in a sample of 167 (mean age = 69 years) dementia-free older adults with cognitive testing, amyloid PET, and multimodal brain MRI scans. The cognitive test scores covered the domains of episodic memory, executive function, language, and vis

Alzheimer's disease-related cortical proteins modify the association of brain insulin signaling with cognitive decline.

BackgroundBrain insulin signaling has been associated with both Alzheimer's disease (AD) pathology and cognitive decline, but the mechanisms remain unclear.ObjectiveTo examine whether AD-related cortically-expressed proteins modify the association of brain insulin signaling and cognitive decline.MethodsParticipants included 116 autopsied members of the Religious Orders Study (58 with diabetes matched to 58 without, by age at death, sex, and education) who had both postmortem brain (prefrontal cortex) insulin signaling (by ELISA and immunohistochemistry, including RAC-alpha serine/threonine-pro

The need for details for object discrimination and mnemonic discrimination.

Both mnemonic discrimination and object recognition tasks rely on the utilization of subtle visual details. Successful mnemonic discrimination utilizes the subtle details of an event to orthogonalize highly similar episodic memories, whereas object discrimination is a memory-free skill that utilizes details in order to distinguish between similar stimuli simultaneously presented. Additionally, neuroimaging studies have implicated areas of the medial temporal lobe as important for both mnemonic discrimination and object discrimination, further suggesting that performance on these two tasks are

Demographic and clinical predictors of response and remission in the treatment of major depressive disorder with ketamine and esketamine: A systematic review.

Accumulating evidence supports the efficacy of (es)ketamine in the treatment of major depressive disorder (MDD), particularly treatment-resistant depression (TRD). Yet around 50% of the individuals with TRD do not respond to (es)ketamine. Elucidating predictors of response and remission could improve treatment outcomes at the individual level by defining subpopulations that are most likely to benefit from (es)ketamine. This systematic review outlines the predictive value of demographic and clinical characteristics for treatment outcomes of (es)ketamine in MDD. A systematic literature search wa

Somatostatin and the pathophysiology of Alzheimer's disease.

Among the central features of Alzheimer's disease (AD) progression are altered levels of the neuropeptide somatostatin (SST), and the colocalisation of SST-positive interneurons (SST-INs) with amyloid-β plaques, leading to cell death. In this theoretical review, I propose a molecular model for the pathogenesis of AD based on SST-IN hypofunction and hyperactivity. Namely, hypofunctional and hyperactive SST-INs struggle to control hyperactivity in medial regions in early stages, leading to axonal Aβ production through excessive presynaptic GABAB inhibition, GABAB1a/APP complex downregulation and

Cognitive, behavioral and psychiatric symptoms in patients with spinal cord injury: a scoping review.

Spinal Cord Injury (SCI) is a condition where the spinal cord is damaged and experiences partial or complete loss of motor and/or sensory function, which is typically less than normal. After SCI, patients may exhibit more severe psychiatric symptoms and experience cognitive impairments, including reduced speed and attention processing capacity, as well as difficulties with executive function and episodic memory retention. Among the behavioral and psychiatric symptoms, depression, anxiety, substance use disorder, and posttraumatic stress disorder are the most common. This review aims to investi

Measurement of resistance-area product by transcranial Doppler: An alternative tool for cognitive screening in hypertensive on drug treatment?

INTRODUCTION: Arterial hypertrophy and remodeling are adaptive responses present in systemic arterial hypertension that can result in silent ischemia and neurodegeneration, compromising brain connections and cognitive performance (CP). However, CP is affected differently over time, so traditional screening methods may become less sensitive in assessing certain cognitive domains. The study aimed to evaluate whether cerebrovascular hemodynamic parameters can serve as a tool for cognitive screening in hypertensive without clinically manifest cognitive decline. METHODS: Participants were allocate

Craniopharyngioma-An update on metabolic and cognitive complications and new therapy.

Craniopharyngiomas (CPs) are rare primary brain epithelial tumors arising in the suprasellar region from remnants of Rathke's pouch. About 50% originate at the level of the third ventricle floor, including the hypothalamus (HT). CPs are characterized by a low proliferation rate and symptoms due to mass effect and local infiltration and are managed primarily with surgery and radiotherapy. Gross total removal of a CP will reduce the recurrence rate but increases the risk of HT damage. Today, subtotal resection is the goal and will reduce the risk of HT damage. There are two histological subtypes

Cognitive, Disability, and Treatment Outcome Implications of Symptom-Based Phenotyping in Late-Life Depression.

OBJECTIVE: Late-life depression is associated with substantial heterogeneity in clinical presentation, disability, and response to antidepressant treatment. We examined whether self-report of severity of common symptoms, including anhedonia, apathy, rumination, worry, insomnia, and fatigue were associated with differences in presentation and response to treatment. We also examined whether these symptoms improved during treatment with escitalopram. DESIGN: Eighty-nine older adults completed baseline assessments, neuropsychological testing and providing self-reported symptom and disability scal

The functional role of the precuneus.

Recent advancements in computational approaches and neuroimaging techniques have refined our understanding of the precuneus. While previously believed to be largely a visual processing region, the importance of the precuneus in complex cognitive functions has been previously less familiar due to a lack of focal lesions in this deeply seated region, but also a poor understanding of its true underlying anatomy. Fortunately, recent studies have revealed significant information on the structural and functional connectivity of this region, and this data has provided a more detailed mechanistic unde
