Impact of sella floor reconstruction on Rathke Cleft Cyst recurrence: a systematic review and meta-analysis.

BACKGROUND: The optimal surgical technique for managing Rathke's Cleft Cyst (RCC) remains unclear. Leaving the sellar defect open (marsupial … cal recurrence. Cyst fenestration while maintaining an open sellar floor (marsupialisation into the sphenoid sinus) is associated with a significantly lower risk of recurrence at over 4 years follow-up. Intraoperative CSF leaks were less strongly associated with cyst recurrence, suggesting that watertight reconstruction, rather than the leak itself, is the prima …

Primary yolk sac tumor of pterygopalatine fossa with loss of vision: A case report.

INTRODUCTION: Primary yolk sac tumor (YST) is an infrequently-diagnosed malignant extragonadal germ cell tumors. It is likely to recur locally and may present with widespread metastases once diagnosed. Primary YST of the head is uncommon but can cause severe complications, such as loss of vision once the tumor mass invades the optic nerve. PATIENT CONCERNS: A 20-month-old boy presented to the general clinic of the local children's hospital with a complaint of swelling of left face for 1 year and proptosis of the left eye for over 2 weeks as stated by his parents. Initially, he did have some v

Disseminated Aspergillus flavus infection presenting as spondylodiscitis: a fatal tuberculosis mimicker in an immunocompetent host - case report and literature review.

INTRODUCTION: Disseminated invasive aspergillosis is a rare but life-threatening fungal infection that predominantly affects immunocompromised individuals. Vertebral involvement is an uncommon manifestation, and diagnosis in immunocompetent patients is particularly challenging because of its nonspecific clinical and radiological features, often resulting in delayed diagnosis and inappropriate therapy. We report a fatal case of disseminated Aspergillus flavus aspergillosis presenting as spondylodiscitis in an immunocompetent host, highlighting the diagnostic challenges and therapeutic implicati

Use of Intraoperative Tumor Consistency Grading to Guide Resection Technique of Pituitary Neuroendocrine Tumors.

BACKGROUND AND OBJECTIVES: Pituitary neuroendocrine tumors (PitNETs) are common intracranial neoplasms for which the endoscopic endonasal trans-sphenoidal approach has become the gold standard for surgical management. Resection can be further categorized as extracapsular resection (ER), during which the tumor pseudocapsule is dissected from surrounding structures and majority of tumor is delivered en bloc , or intracapsular resection (IR) during which the tumor is debulked, suctioned, and removed from within the pseudocapsule. This study assessed the relationship between PitNET consistency and

Assessment of low-dose paranasal sinus CT imaging using a new deep learning image reconstruction technique in children compared to adaptive statistical iterative reconstruction V (ASiR-V).

PURPOSE: To compare the effects of deep learning image reconstruction (DLIR) and adaptive statistical iterative reconstruction V (ASiR-V) on image quality in low-dose computed tomography (CT) of paranasal sinuses in children. METHODS: Low-dose CT scans of the paranasal sinuses in 25 pediatric patients were retrospectively evaluated. The raw data were reconstructed with three levels of DLIR (high, H; medium, M; and low, L), filtered back projection (FBP), and ASiR-V (30% and 50%). Image noise was measured in both soft tissue and bone windows, and the signal-to-noise ratios (SNRs) and contrast-

[Study of small dural window exposure strategy for preventing intraoperative acute brain herniation during removal of huge intracranial meningiomas].

OBJECTIVE: To investigate and assess the value of small dural window exposure strategy on preventing brain herniation during the operation for huge intracranial meningiomas. METHODS: Different classical operative approaches were used for the removal of intracranial meningiomas situated in different locations. When the dura was found to be under high tension, it was imperative that measures should be taken to prevent damage to the brain tissue in the important cortical areas as a result of acute intraoperative brain swelling and herniation. The strategy to be taken was to make a small dural wi

Orbit-associated tumors: navigation and control of resection using intraoperative computed tomography.

OBJECT Treatment of skull base lesions is complex and usually requires a multidisciplinary approach. In meningioma, which is the most common tumor entity in this region, resection is considered to be the most important therapeutic step to avoid tumor recurrence. However, resection of skull base lesions with orbital or optic nerve involvement poses a challenge due to their anatomical structure and their proximity to eloquent areas. Therefore the main goal of surgery should be to achieve the maximum extent of resection while preserving neurological function. In the postoperative course, medical

[A case of multiple anomalies of cerebral vessels--fenestration of the middle cerebral artery aneurysm of the anterior communicating artery and arteriovenous malformation on the frontopolar region (author's transl)].

A case associated with multiple cerebral vascular anomalies, which consisted of fenestration of the middle cerebral artery, arteriovenous malformation and aneurysm of the anterior communicating artery, was reported. A 48 year-old male has been suffering from the left paralysis and mental disorder after the initial attack of subarachnoid hemorrhage, and the second attack resulted in the deterioration of the symptoms. He was admitted to our clinic on October 28, 1974. On neurological examination, mental disorders, such as disorientation, emotional incontinence, amnesia and acalculia, hemiplegia

Endoscopic endonasal resection of suprasellar meningiomas: the importance of case selection and experience in determining extent of resection, visual improvement, and complications.

OBJECTIVE: Suprasellar meningiomas have been resected via various open cranial approaches. During the past 2 decades, the endoscopic endonasal approach has been shown to be an option in selected patients. We wished to examine the learning curve for parameters such as extent of resection, visual outcome, and complications. METHODS: We retrospectively reviewed a consecutive series of patients in whom suprasellar meningiomas were resected via an endonasal endoscopic approach between 2005 and 2013 at our institution. After June 2008, our surgical technique matured. Using this time point, we divid

Impact of intraoperative MRI-guided transsphenoidal surgery on endocrine function and hormone substitution therapy in patients with pituitary adenoma.

BACKGROUND: Pituitary adenomas are rare with an incidence of 0.4-8.2 per 105 inhabitants. Symptoms range from headaches to pituitary insufficiency or excessive output of hormones with associated disease. Except for prolactinomas, surgery is recommended as the first line and most effective treatment for the majority of these tumours. One of the refinements of surgical therapy introduced was intraoperative magnetic resonance imaging (iMRI). OBJECTIVE: The aim of this study was to analyse the postoperative pituitary function and the general outcome of patients treated for non-functioning and GH-

Value of visual evoked potential monitoring during trans-sphenoidal pituitary surgery.

The visual outcome of 22 patients undergoing trans-sphenoidal excision of pituitary macroadenomas with intraoperative flash visual evoked potential (VEP) monitoring (Group A), was compared with a non-randomized group of 14 patients who had undergone similar operations without VEP monitoring (Group B). Tumour size, preoperative visual acuity, peripheral fields, and latencies and amplitudes of P1 and P2 were analysed to ascertain the best predictor of postoperative visual function. It was found that patients in Group A had a significantly greater improvement in field defects than those in Group

Delayed onset of hyponatremia after transsphenoidal surgery for pituitary adenomas.

Hyponatremia is rarely reported as a delayed complication of transsphenoidal resection of pituitary adenoma. Usually attributed to the syndrome of inappropriate secretion of antidiuretic hormone (SIADH), hyponatremia causes nonspecific symptoms, often after hospital discharge. To clarify the frequency, presentation, and outcome of this poorly understood complication, we reviewed our database of 2297 patients who underwent transsphenoidal pituitary surgery between February 1971 and June 1993. Of 53 patients (2.3%) treated for symptomatic hyponatremia, 11 were excluded (2 received arginine vasop

Effect of thrombin concentration on the adhesion strength and clinical application of fibrin glue-soaked sponge.

Fibrin glue-soaked gelatin sponge (FGGS) has been used for tissue sealing in neurosurgical practice, but too rapid clotting of fibrin glue occasionally prevents good fixation of FGGS. Dilution of thrombin may provide adequate manipulation time between mixing fibrinogen and thrombin on gelatin sponge and application into the tissue defects. The present study characterized the effect of thrombin dilution on the adhesion strength of FGGS and retrospectively assessed the clinical usage of the dilution for filling dead space or sealing arachnoid defect in 255 cases who underwent transsphenoidal sur

[Early trans-sphenoid decompression in indirect traumatic optic neuropathy].

BACKGROUND: Indirect traumatic optic neuropathy represents a severe, potential vision-threatening disease process that requires close interdisciplinary cooperation for treatment. In general, any therapy has been discussed controversially in the literature. Based upon experiences with spinal cord trauma, high-dose steroid therapy is recommended initially to minimize secondary consequences following orbital trauma. The usefulness of surgical decompression of the optic nerve has been confirmed in several studies but is still not recommended in principle. PATIENTS AND METHOD: In nine consecutive

Evidence of meningioma infiltration into cranial nerves: clinical implications for cavernous sinus meningiomas.

Anatomical and biological studies of cavernous sinus meningiomas help us understand the biological heterogeneity of these tumors. The question of whether cavernous sinus meningiomas infiltrate cranial nerves is clinically important because of the effect on treatment planning. In the authors' experience of treating 36 patients with cavernous sinus meningiomas, tumor invasion into a cranial nerve was documented in two patients in whom a cranial nerve was resected during the cavernous sinus dissection. In both patients, histological examination using hematoxylin and eosin and bodian stains showed
