Varicella zoster virus (VZV) causes varicella during acute infection and establishes latency in the sensory ganglia. Reactivation of VZV results in herpes zoster, a debilitating and painful disease. It is believed that VZV reactivates due to a decline in cell-mediated immunity; however, the roles that CD4 versus CD8 T cells play in the prevention of herpes zoster remain poorly understood. To address this question, we used a well-characterized model of VZV infection where rhesus macaques are intrabronchially infected with the homologous simian varicella virus (SVV). Latently infected rhesus mac

BACKGROUND: Thymic carcinomas are rare tumors with a challenging diagnosis. Our aim was to report our 17-year experience of these tumors and to highlight the challenges encountered and the main differential diagnoses ruled out.
METHODS: We studied 12 (92%) men and 1 (7.7%) woman with a mean age of 37 years (range 15-60 years). All patients were symptomatic, with chest pain representing the most frequent symptom. Radiology revealed anterior mediastinal masses in all cases, with either infiltration of the adjacent organs or pulmonary parenchymal metastases.
RESULTS: The diagnosis was made on s

Recent studies on acute myelogenous leukemia (AML) patients have revealed the existence of T-cell immunodeficiencies, characterized by peripheral T lymphocytes that are unable to interact with blasts, reduced thymic emigrants and oligoclonal restricted repertoires. These observations suggest that there is a profound thymic dysregulation, which is difficult to study in AML patients. Using the C1498 AML mouse model, we demonstrated that leukemia development was associated with thymus atrophy, which was defined by abnormal organ weight and reduced cellularity. In addition, we observed a dramatic

Although video-assisted thoracic surgery can be used for well-encapsulated thymomas, its role in invasive thymomas remains controversial. Between 1998 and 2009, 77 patients aged 22-76 years underwent thymomectomy by video-assisted thoracic surgery. Tumors <5 cm without major invasion on preoperative computed tomography were selected. There were 13 invasive thymomas (Masaoka stage III and IV). A modified dissection technique was employed to prevent breaching the tumor capsule and risking tumor seedling. Limited resection of the phrenic nerve, pericardium, perithymic fat, and a wedge of lung was

The objectives of this study were (I) to explore the prognosis of ocular myasthenia gravis (OMG) in patients with onset at age 70 years and above (i.e. senior persons); (2) to identify predictors of secondary generalization in this age group; and 3) to address the effects of immunotherapy on this population of patients. We performed a retrospective analysis of 39 patients with myasthenia gravis who presented with only ocular signs and symptoms after age 70 years. Generalized myasthenia gravis (GMG) developed in 12 OMG patients (31%). None of the GMG patients required ventilator assistance or a

BACKGROUND: In stage III thymoma, the involved organs are usually resected to achieve complete resection. However, inclusion of the phrenic nerve (PN) in the resection may compromise pulmonary function which may be critical for the patients with serious complications.
METHODS: We retrospectively analyzed 18 thymoma patients with Masaoka stage III thymoma that involved the PN to determine the effects of PN resection versus PN preservation on postoperative pulmonary function, survival, and relapse.
RESULTS: The PN was resected in 9 patients and preserved in 9 patients. The PN resection was avo

BACKGROUND: The treatment of locally advanced thymic tumors is not uniform. Recently, several centers have reported the results of induction chemotherapy followed by resection and then radiation. Our center adopted an alternative strategy and treated locally advanced thymic tumors with induction chemoradiotherapy in an effort to maximize the intensity of the induction therapy.
METHODS: A retrospective review was performed of 10 patients with locally advanced thymic tumors treated from 1997 to 2006. Seven patients were clinically staged as Masaoka stage III and 3 as stage IVA. The treatment pl

Patients with myasthenia gravis or other neuromuscular disorders are subject to increasing weakness of bulbar-innervated muscles, with resulting aspiration or catastrophic airway compromise. The available practical assessments of bulbar function in children are inadequate. We report our experience with the "slurp" test, a new bedside measure of bulbar function, in children with myasthenia gravis. Our experience suggests that the test is valuable for identifying patients with probable serious compromise of bulbar function, for monitoring such children during times of intercurrent illness, and f

The development of organ-specific autoimmune diseases in mice thymectomized on day 3 of life (d3tx mice) can be prevented by transferring CD4(+)CD25(+) T cells from syngeneic, normal adult mice. Using a d3tx model, we asked whether CD4(+)CD25(+) T cell deficiency contributes to glomerulonephritis (GN) in lupus-prone mice. New Zealand Mixed 2328 (NZM2328) mice spontaneously develop autoantibodies to dsDNA and female-dominant, fatal GN. After d3tx, both male and female NZM2328 mice developed 1) accelerated dsDNA autoantibody response, 2) early onset and severe proliferative GN with massive mesan

To evaluate clinical characteristics and outcome of myasthenia gravis (MG) in aged patients (> 60yrs), we retrospectively reviewed a continuous series of 122 myasthenic patients observed from January 1968 through December 1994. Patients with congenital, neonatal, or penicillamine-induced myasthenia were excluded. Twenty-five subjects (20%) were > 60yrs. The male/female ratio was 3:2; 20% of patients had an ocular form and 86% were seropositive. Mediastinum CT scan revealed thymic changes in 14%. During the first five years of disease, 60% of patients with ocular form progressed towards a gener

In advanced stage thymomas, recurrence is not uncommon but prognostic factors in recurrent thymoma have not been determined and standardized treatment for recurrence has not yet been established. A retrospective analysis was conducted on 24 thymoma patients who underwent treatment for recurrence in our institution to determine the prognostic factors for overall survival. Recurrence of thymoma appeared 11.6-109.6 months after the primary operation (34.6±25.7 months). Pleural disseminated recurrence was common (n=21) as the primary recurrent lesions. Single or combined modality therapy was perfo

Myasthenia gravis (MG) can be difficult to treat despite an available therapeutic armamentarium. Our aim was to analyze the factors leading to unsatisfactory outcome (UO). To this end we used the Myasthenia Gravis Foundation of America classification system. Forty one patients with autoimmune MG were followed prospectively from January 2003 to December 2007. Outcomes were assessed throughout follow-up and at a final visit. 'Unchanged', 'worse', 'exacerbation' and 'died of MG' post-intervention status were considered UOs. During follow-up, UO rates reached 54% and were related to undertreatment

Determining the appropriate surgery-based treatment for complicated anterior mediastinal malignancies (CAMM), especially those invading the superior vena cava (SVC) and its branches, remains a challenge for general thoracic surgeons. In this report, we summarize our experience and lessons regarding this issue in order to discuss a reasonable strategy for diagnosis and treatment of CAMM. Between January 2001 and April 2003, 15 patients with CAMM invading the SVC and/or its branches with or without invasion of other neighboring organs were surgically treated in our institution by a single surgeo

Mice are capable of rejecting H-2-incompatible bone marrow grafts after a single lethal exposure to X-rays. The onset of rejection begins 18-24 hr after transplantation and is completed by 96 hr. Maturation of this type of allograft reactivity does not occur until the 22nd day of life. In adult mice, the resistance to marrow allografts can be weakened by administration of cyclophosphamide or dead cultures of Corynebacterium parvum, but not heterologous anti-thymocyte serum. Sublethal exposures to X-rays 7 or 14 days before transplantation also weaken resistance. There is considerable interstra

Thoracic duct cells and spleen cells from normal Lewis rats were tested for their ability to restore the antibody response of X-irradiated hosts to diphtheria toxoid (DTAP) or DNP-diphtheria toxoid (DNP—DT) adsorbed to alum. Although spleen cells restore a measurable response to both DTAP and DNP—DT, thoracic duct cells do not. However, thoracic duct cells from donors primed to DTAP restore the primary anti-DNP response to DNP—DT. These findings indicate that lymphocytes which initiate the antibody response to the carrier play an important role in the initiation of the primary response to the

A brief review is presented of the evidence linking the development of certain types of neoplasms, and of the malignant lymphomas in particular, to chronic immunosuppression in animals and man and to the naturally occurring human immunologic deficiency states. The discussion then focuses on Hodgkin's disease and considers recent evidence concerning the relation between the clinical stage of the disease and its associated defect in cell-mediated immunity. Finally, the prior occurrence of infectious mononucleosis in some cases of Hodgkin's disease is considered in the context of the hypothesis t
