[Diagnostic and therapeutic approach to progressive sensorineural hearing loss].

Progressive sensorineural hearing loss (PSNHL) is an important clinical entity that can develop rapidly and evolve to deafness. The causes of PSNHL can be divided into hereditary, developmental, infectious, autoimmune, metabolic, vascular, neoplastic, toxic and degenerative, even if the origin of the hearing impairment often remains unknown and a diagnosis of idiopathic PSNHL is made. In order to establish the etiology of the hypoacusia or to rule out any known etiology of PSNHL and to diagnose an idiopathic form, a detailed family, physiological and medical history has to be obtained, includi

Portable eye-tracking as a reliable assessment of oculomotor, cognitive and reaction time function: Normative data for 18-45 year old.

Eye movements measured by high precision eye-tracking technology represent a sensitive, objective, and non-invasive method to probe functional neural pathways. Oculomotor tests (e.g., saccades and smooth pursuit), tests that involve cognitive processing (e.g., antisaccade and predictive saccade), and reaction time tests have increasingly been showing utility in the diagnosis and monitoring of mild traumatic brain injury (mTBI) in research settings. Currently, the adoption of these tests into clinical practice is hampered by a lack of a normative data set. The goal of this study was to construc

Utility of the Frequency Tuning Measure of oVEMP in Differentiating Meniere's Disease from BPPV.

BACKGROUND: Utility of frequency tuning of ocular vestibular evoked myogenic potential (oVEMP) for evaluation of utricular function in individuals with Meniere's disease is a recent development. However, there is dearth of studies regarding its utility in differential diagnosis of Meniere's disease from other vestibular pathologies. PURPOSE: The present study aimed at investigating the feasibility of frequency tuning of oVEMP in discriminating Meniere's disease from benign paroxysmal positional vertigo (BPPV). RESEARCH DESIGN: Static group comparison. STUDY SAMPLE: oVEMPs were acquired from

Unertan syndrome: a case series demonstrating human devolution.

A large family with six individuals exhibiting the Unertan syndrome (UTS) was identified residing in southern Turkey. All of the individuals had mental impairments and walked on all four extremities. The practice of intra-familial marriages suggested that the UTS may be an autosomal recessive disorder, similar to previously described cases. The inferior portions of the cerebellum and vermis were absent as evidenced by MRI and CT scans. The height and head circumference of those affected were within normal ranges. Barany's test suggested normal vestibular system function. The subjects could not

Assessment of dynamic stability and identification of key tasks and parameters in patients with unilateral and bilateral vestibulopathy: a laboratory-based study.

Chronic imbalance is the cardinal symptom in bilateral vestibulopathy patients (BV), and in a subset of symptomatic unilateral vestibulopathy patients (UV), leading to a significant impact on their daily lives. Despite these profound effects, such as the risk of falls, the mechanism of imbalance remains complex, posing challenges both for monitoring patients' functional status and for evaluating rehabilitation therapies. The aim of this study was to assess the dynamic stability of patients with BV and UV during multiple motor tasks and to provide a summary of the most relevant tasks and biomec

Noisy galvanic vestibular stimulation and static balance in parkinson's disease: a multimodal resting‑state fMRI feasibility study.

BACKGROUND: Postural instability in Parkinson's disease (PD) is a major contributor to falls and functional decline, yet remains poorly responsive to dopaminergic therapy. Noisy galvanic vestibular stimulation (nGVS), a non-invasive neuromodulation technique, has shown potential to enhance postural control by augmenting multisensory integration. However, the neural mechanisms and temporal dynamics underlying its effects remain incompletely understood. OBJECTIVE: To evaluate the behavioral and neurophysiological effects of a single session of nGVS in patients with mild-to-moderate PD using sta

The Effects of Long Duration Spaceflight on Sensorimotor Control and Cognition.

Astronauts returning from spaceflight typically show transient declines in mobility and balance. Other sensorimotor behaviors and cognitive function have not been investigated as much. Here, we tested whether spaceflight affects performance on various sensorimotor and cognitive tasks during and after missions to the International Space Station (ISS). We obtained mobility (Functional Mobility Test), balance (Sensory Organization Test-5), bimanual coordination (bimanual Purdue Pegboard), cognitive-motor dual-tasking and various other cognitive measures (Digit Symbol Substitution Test, Cube Rotat

[Frequency-specific analysis of the hVOR Prior of Cochlear Implant Operation].

Dizziness is one of the most common postoperative complications after a cochlear-implant (CI) surgery. With our prospective, matched-paired controlled study, we could demonstrate that patients with distinctive sensorineural hearing loss--even without any complaints of dizziness--already have a reduced horizontal vestibular-ocular-reflex (hVOR). Compared to controls, CI patients presented with a significantly reduced gain. 9 out of 17 CI patients showed physiological results in rotatory testing and video head thrust testing. One patient presented with pathological results in both tests. Remarka

Betahistine dihydrochloride in the treatment of peripheral vestibular vertigo.

The present study compares the efficacy and safety of betahistine dihydrochloride to that of a placebo in recurrent vertigo resulting from Meniere's disease (MD) or in paroxysmal positional vertigo (PPV) of probable vascular origin. The design was double-blind, multicentre and parallel-group randomised. Eleven Italian centres enrolled 144 patients: 75 of the patients were treated with betahistine (41 MD/34 PPV) and 69 with placebos (40 MD/29 PPV). The betahistine dosage was 16 mg twice per day for 3 months. Compared to the placebo, betahistine had a significant effect on the frequency, intensi

Cerebellopontine angle meningiomas: postoperative outcomes in a modern cohort.

OBJECT: Tumors of the cerebellopontine angle (CPA) have always proven difficult for neurosurgeons to optimally manage. Studies investigating the natural history and treatment of vestibular schwannomas have dominated the literature in this regard. Distinguishing meningiomas from schwannomas in this location carries particular importance as each tumor type has certain prognostic and surgical considerations. In this study, the authors have characterized the outcomes of 34 patients surgically treated for CPA meningiomas and have investigated various factors that may affect postoperative neurologic

Reflex control of the spine and posture: a review of the literature from a chiropractic perspective.

OBJECTIVE: This review details the anatomy and interactions of the postural and somatosensory reflexes. We attempt to identify the important role the nervous system plays in maintaining reflex control of the spine and posture. We also review, illustrate, and discuss how the human vertebral column develops, functions, and adapts to Earth's gravity in an upright position. We identify functional characteristics of the postural reflexes by reporting previous observations of subjects during periods of microgravity or weightlessness. BACKGROUND: Historically, chiropractic has centered around the co

Clinical observation on acute low-frequency hearing loss without vertigo: the role of cochlear hydrops analysis masking procedure as initial prognostic parameter.

OBJECTIVES: Even though it is currently not possible to prove a pathological diagnosis for inner ear disease, acute low-frequency hearing loss (ALFHL) without vertigo could be caused by inner ear hydrops because progression into the clinical spectrum of endolymphatic hydrops (EH) frequently occur among patients with the initial clinical presentation. Therefore, audiological measures representative of inner ear hydrops, such as the cochlear hydrops analysis masking procedure (CHAMP) test, may be used to predict the prognosis of ALFHL without vertigo. To test this hypothesis, we prospectively in

Semi-quantitative evaluation of endolymphatic hydrops by bilateral intratympanic gadolinium-based contrast agent (GBCA) administration with MRI for Meniere's disease.

CONCLUSION: Bilateral intratympanic administration of a gadolinium-based contrast agent (GBCA) in MRI was successfully performed and proved to be beneficial in the semi-quantitative evaluation of endolymphatic hydrops. Such image-based diagnosis will lead to re-revaluation and reclassification of the diagnostic criteria for Meniere's disease (MD). OBJECTIVE: To visualize endolymphatic hydrops semi-quantitatively in patients with MD, by using bilateral intratympanic GBCA administration with MRI.
