Protective effects of magnolol, an active compound of Houpu Sanwu Tang, on intestinal barrier function in sepsis: Mechanisms of PPARG activation and inhibition of JAK-STAT and NF-κB signaling.

BACKGROUND: Sepsis, a dysregulated host response to infection, frequently leads to catastrophic intestinal barrier failure, a key driver of mortality. Magnolol, a bioactive compound from Magnolia officinalis, has shown pleiotropic therapeutic effects, but its role in sepsis-induced intestinal injury remains unclear. Here, we investigate the protective mechanisms of magnolol in sepsis, focusing on its modulation of inflammatory signaling. METHODS: We employed both in vitro (LPS-stimulated Caco-2 cells) and in vivo (cecal ligation and puncture model of sepsis) systems. The effects of magnolol o

Decreased AMP-kinase function in pulmonary artery smooth muscle cells contributes to pulmonary artery remodeling in persistent pulmonary hypertension of the newborn.

Pulmonary vascular remodeling contributes to persistent pulmonary hypertension of the newborn (PPHN); the mechanisms remain unknown. 5'-AMP-activated protein kinase (AMPK) is a critical regulator of energy balance and metabolism. We investigated the hypothesis that decreased AMPK function in pulmonary artery smooth muscle cells (PASMCs) leads to impaired mitochondrial capacity to perform oxidative phosphorylation and altered notch ligand expression, which together promote vascular remodeling in PPHN. Studies were performed in fetal lambs with PPHN induced by prenatal ductus arteriosus constric

Effect of Sacubitril-Valsartan on Secondary Mitral Regurgitation and Left Ventricular Reverse Remodeling in Patients With Heart Failure With Reduced Ejection Fraction: A 2D and 3D Echocardiographic Study.

BACKGROUND: Heart failure (HF) with reduced ejection fraction (HFrEF) is characterized by progressive left ventricular (LV) remodeling and is often complicated by secondary (functional) mitral regurgitation (MR). Sacubitril-valsartan (S/V), an angiotensin receptor-neprilysin inhibitor (ARNI), has demonstrated superiority over enalapril in reducing morbidity and mortality in HFrEF. Its mechanism of action includes promoting LV reverse remodeling (LVRR), but the comprehensive effect on both LVRR and secondary MR, particularly using advanced three-dimensional (3D) echocardiography, requires furth

Transient EZH2 Suppression by Tazemetostat during In Vitro Expansion Maintains T-Cell Stemness and Improves Adoptive T-Cell Therapy.

The histone methyltransferase enhancer of zeste homolog 2 (EZH2) plays important roles in T-cell differentiation, proliferation, and function. Previous studies have demonstrated that genetic deletion of EZH2 in CD8+ or total T cells impairs their antiviral and antitumor activities, cytokine production, and ability to expand upon rechallenge. Contrary to the detrimental role of deleting T cell-intrinsic EZH2, in this study, we demonstrated that transient inhibition of EZH2 in T cells prior to the phenotypic onset of exhaustion with a clinically approved inhibitor, tazemetostat (Taz), delayed th

Effects of Sacubitril/Valsartan on Survival in Patients with Heart Failure and Significant Valvular Heart Disease.

Although the benefits of sacubitril/valsartan in heart failure with reduced ejection fraction (HFrEF) are well established, patients with hemodynamically significant mitral regurgitation (MR) were excluded from pivotal trials. We aimed to assess the effects of sacubitril/valsartan on survival in patients with HFrEF and concomitant significant MR. All patients from a single center who underwent echocardiography between June 2008 and December 2020, with a left ventricular ejection fraction (LVEF) of less than 40% and hemodynamically significant MR were recruited. Patients were categorized accord

Therapeutic Effects of Heart Failure Medical Therapies on Standardized Kidney Outcomes: Comprehensive Individual Participant-Level Analysis of 6 Randomized Clinical Trials.

BACKGROUND: Kidney outcomes have been variably defined using nonstandardized composite end points in key heart failure trials, thus introducing complexity in their interpretation and cross-trial comparability. We examined the effects of steroidal mineralocorticoid receptor antagonists, the angiotensin receptor-neprilysin inhibitor sacubitril/valsartan, and SGLT2 (sodium-glucose cotransporter-2) inhibitors on composite kidney end points using uniform definitions in 6 contemporary heart failure trials. METHODS: Individual participant-level data from trials of steroidal mineralocorticoid recepto

Real-world data on the efficacy and safety of hedgehog pathway inhibitors in patients with basal cell carcinoma: Experience of a tertiary Australian centre.

BACKGROUND: Basal cell carcinomas (BCCs) are the most common cancers worldwide. Although most BCCs are amenable to local treatment, there are limited therapeutic options for surgically unresectable locally advanced and metastatic BCCs. Activation of the sonic hedgehog signalling pathway plays a significant role in the development of most BCCs. Hedgehog pathway inhibitors (HPIs) can be used to inhibit this pathway. Efficacy and safety data on HPI use in Australia is scarce. OBJECTIVES: This study aims to present the effectiveness and safety of HPI at a tertiary dermatology referral centre. ME

SGLT-2 Inhibitors on Top of Current Pharmacological Treatments for Heart Failure: A Comparative Review on Outcomes and Cost Effectiveness.

Heart failure (HF) represents a major global health and economic burden with still unacceptably high morbidity and mortality rates. In recent decades, novel therapeutic opportunities with a significant impact on HF outcomes have been introduced in addition to angiotensin-converting enzyme (ACE) inhibitors, β-blockers, and mineralocorticoid receptor antagonists. These include drugs such as ivabradine, sacubitril-valsartan, and sodium-glucose cotransporter-2 (SGLT-2) inhibitors. The availability of an extremely large pharmacological armamentarium to face this chronic global disease highlights th
