Reconstruction of the distal radius facet by a free vascularized osteochondral autograft: anatomic study and report of a patient.

PURPOSE: Large chondral defects of the distal radius after fractures present a reconstructive challenge. The purpose of this study was to present the anatomic findings from a cadaver of a vascularized osteochondral autograft taken from the third metatarsal appropriate for reconstructing the distal radius articular facet. A patient is presented in whom 70% of the scaphoid fossa was reconstructed with this technique. METHODS: The base of the third metatarsal was studied in the feet of 20 cadavers. The size and shape of the cartilage were measured. Additionally vessel distribution was recorded a

Traumatic posterior rotatory fracture-dislocation of lumbo-sacral spine.

Fracture-dislocation of the lumbo-sacral spine was an unusual injury and was divided into anterior, posterior and lateral types depending on the displacement of the cephalad portion of the spine over the caudal portion. According to the authors' knowledge, only 31 cases of traumatic fracture-dislocation of the lumbo-sacral spine were reported in the English literature. Only 3 previous reports referred to this injury with a posterior displacement, which was an even rarer injury. This was the fourth report of this type of injury.

[Four part valgus impacted fractures of the upper extremity of humerus: ilium graft reconstruction. Apropos of 8 cases].

PURPOSE OF THE STUDY: We present the results of an original technique for reconstruction of valgus-impacted humeral head using autologous iliac bone. MATERIAL AND METHODS: Between 1992 and 1993, 8 patients (average age 66 years, range 43 to 79 years) with four-part valgus impacted proximal humeral fractures were operated with the aim of preserving the humeral head. All fractures were treated with open reduction and were stabilized only with autologous bone. This procedure provided fixation without osteosynthesis. Using a deltopectoral approach the impacted segment of the humeral head was rais

Compound mechanism of fatal neck injury: A case report of a tiger attack in a zoo.

Fatal injuries caused by attacks by large wild cats are extremely rare in forensic medical practice in Europe. There are very few cases described in the forensic medical literature concerning incidents in zoos similar to the tiger attack on a 58-year-old male zoo employee that we present here. While preparing a runway for tigers, the man was attacked by a male Sumatran tiger. Another zoo employee was an eyewitness to the accident; in his testimony he described the sequence of events in detail. The autopsy showed the injuries typical of a tiger attack: traces of claws and canine teeth indicatin

Triradiate skin incision for dorsal approach to the wrist.

A new triaradiate skin incision is presented as an approach to the wrist. Centred over Lister's tubercle, it consists of one longitudinal proximal and two oblique distal rays, making three 120 degrees skin flaps. These provide an excellent triangular operating field to the distal radius, the radiocarpal joint and the carpus itself, allowing for most procedures in this region. There have been no complications in 32 patients, and the cosmetic appearance has been excellent in most of them.

Open Intra-articular Distal Femur Fracture with 21 cm Bone Loss Managed by Early Vascularized Fibula Grafting and Antibiotic Loaded Synthetic Calcium Sulfate Beads - A Case Report.

INTRODUCTION: Open fractures with massive bone defect presents as challenging clinical scenario in clinical practice. If neglected, this can result in infection, non-union or severe limb length discrepancy. Autogenous vascularized fibula grafting along with stable fixation in the form of internal fixation is a viable option in such situations. Synthetic calcium sulfate is a suitable antibiotic carrier to deliver antibiotics locally to prevent infection. We report the case of an adult male who had an open fracture of the distal femur with segmental bone loss of 21 cm, managed with autogenous va

Comparison of Differences in Complications and Revision After Conversion to Total Hip Arthroplasty from Plating vs. Nailing vs. Hemiarthroplasty.

BACKGROUND: Total hip arthroplasty (THA)after previous hip surgery increases the procedure complexity and complication rate. We investigated the complication rates following conversion to total hip arthroplasty from three hip surgeries, namely plate fixation of the intertrochanteric fracture, nailing of the trochanteric fracture, and hemiarthroplasty of the hip. METHODS: This retrospective study was conducted using the PearlDiver database (www.pearldiverinc.com, Colorado Springs) and identified all patients undergoing THA between 2010 and 2019. Among this group, we included all patients who h

[Centromedullary nailing of the femur for bone metastasis: clinical and radiological evaluation using the Tokuhashi score in 24 patients].

PURPOSE OF THE STUDY: Pluridisciplinary management of patients with metastasis to the femur is well defined, but the choice between palliative surgery or abstention must be decided on the basis of a few evaluated prognostic criteria. We report a series of 24 cases of metastasis to the weakened or fractured femur which was evaluated with the Tokuhashi score and treated by surgery. MATERIAL AND METHODS: Sixteen women and eight men, mean age 71 years (58-89) underwent centromedullary nailing of the femur. These patients had metastases from breast cancer (n = 13 of the 16 women). Twenty of the 24

[Biological internal fixation -- guidelines for the rehabilitation].

The process of fracture healing restores the biological and mechanical state of the bone tissue. In contrast to other tissues, bone has the unique capacity to heal through a real repair process resulting not in a scar but in a regular reconstitution of its original tissue structure. During the last two decades, the understanding of bone biology continuously evolved leading to the new concept that preservation of the viability of the bone fragments is the key to unimpaired fracture healing. The biological fracture management provides environmental conditions that allow the natural healing proce

Minimally Invasive Surgery for Traumatic High-Grade Lateral Spondylolisthesis of L1 with Multiple Spinal Fractures: Closed Reduction and Internal Fixation Using Percutaneous Pedicle Screws.

BACKGROUND: Traumatic high grade lateral spondylolisthesis at the thoracolumbar junction is an extremely severe injury caused by high-energy trauma, commonly resulting in polytrauma. The treatment of this pathology is challenging, and even death following surgery has been reported. Therefore, it is necessary to focus on making surgical invasion minimal. METHODS: A 53-year-old female fell from a height and presented with severe back pain and complete paralysis below L1. Computed tomography (CT) showed a Grade 4 traumatic lateral spondylolisthesis and severe comminution of L1, and mild compress

What are the ideal screw lengths in volar locking plate fixation for distal radius fractures?

PURPOSE: This study aimed to specify the optimal lengths of the distal locking screws (in a female population undergoing distal radius fracture fixation with a volar locking plate) to avoid damaging the dorsal extensor tendon compartments while preserving stability. METHODS: Twenty-five female adult patients underwent volar locking plate fixation with four 2.4 mm locking screws inserted distally. Our modified dorsal tangential fluoroscopic view (DTV) was taken perioperatively followed by postoperative CT scans to compare the accuracy in determining the distal screw lengths. RESULTS: Our modi

Femoral Shaft Fracture during Bungee Jump: A Case Report and Literature Review.

Bungee jumping is a recreational sport that is accepted to carry a level of risk. We present the case of a femoral fracture sustained during bungee jumping and examine the published literature on bungee jumping-related injuries. A previously well 31-year old female performed a 200ft bungee jump from a crane. The apparatus was performed as expected and documented on the bystander video footage. As the bungee-cord became taut for the second time, there was an audible crack with accompanying scream. A closed, neurovascularly-intact injury was sustained to her right thigh. Radiographs revealed a c

Thumb proximal phalanx reconstruction with nonvascularized corticocancellous olecranon bone graft.

Large segmental bone defects of the phalanges reportedly have been treated with free vascularized grafts from the hand, foot, or knee, or with nonvascularized grafts from the iliac crest. A nonvascularized structural corticocancellous graft from a local site would be advantageous. The olecranon has been used as a source of both cancellous and corticocancellous graft. The authors describe a unique case of the use of nonvascularized corticocancellous olecranon bone graft for structural purposes in a mutilating thumb injury. The patient injured the left thumb with a miter saw, resulting in a larg

Necessity for fibular fixation associated with distal tibia fractures.

INTRODUCTION: Intramedullary (IM) nailing is a well-accepted treatment for distal third tibia fractures in combination with injury to the fibula. However, the indications for operative stabilisation of the fibula remain controversial. METHODS: The authors performed a retrospective review on a consecutive series of patients who underwent intramedullary nailing of a non-comminuted distal third tibia fracture with or without fibular fixation at a Level I urban trauma centre. A review of surgical records identified 120 patients who initially were included in this study, while a total of 98 patien

The treatment of Lisfranc injuries.

Thirty-one patients with fracture-dislocations of the tarsometatarsal joint were examined to assess the functional end results after a mean follow-up of 2.9 years (range 20 to 56 months). Sixty-one percent were polytrauma patients; 39% suffered isolated fractures. Forty-five percent had associated lesions of the tarsal joint complex (Chopart and subtalar joint). According to the Baltimore Painful Foot Scoring System (PFS), 52% achieved an excellent or good result and 48% a fair or poor result. Of the four different treatment modalities, open reduction and temporary screw or K-wire fixation yie

The forgotten fragment: additional lesser tuberosity fixation of 4-part proximal humeral fractures-a biomechanical investigation.

BACKGROUND: Fractures of the proximal humerus are common. The most frequent surgical treatment option is open reduction and locking plate fixation. Multifragmentary fractures, including 3- and 4-part fractures, are especially challenging to treat because they correlate with an increased risk of fixation failure. In the past, several mechanisms of additional fixation were investigated, but none directly addressed the lesser tuberosity (LT). The goal of this study was to investigate the biomechanical impact of additional anterior fracture fixation in lateral locked plating (LLP) of 4-part proxim

Comparison of double screw fixation versus tension-band fixation after olecranon osteotomy for complex distal humerus fractures.

INTRODUCTION: Supra- and intercondylar elbow fractures are surgically challenging given the intra-articular comminution. Various surgical approaches have been described. Among these-olecranon osteotomy-provides optimal joint exposure but is associated with several complications. Tension band wiring (TBW) is the most commonly used fixation method. We hypothesized that double screw fixation would lead to fewer surgical revisions than TBW (all causes included) after olecranon osteotomy. METHODS: We carried out a single-center retrospective study of 39 patients, treated between January 2007 and A
