Lumboperitoneal Shunt Malfunction Due to Misplacement of the Lumbar Catheter Into the Spinal Subdural Extra-arachnoid Space: A Case Report.

Lumboperitoneal (LP) shunting is an established treatment for idiopathic normal pressure hydrocephalus (iNPH). Although generally considered less invasive, LP shunts carry the risk of lumbar catheter malposition, including rare misplacement into the spinal subdural extra-arachnoid space (SSES), a potential cause of shunt malfunction. A 72-year-old woman presented with gait disturbance, cognitive decline, and urinary incontinence. Magnetic resonance imaging (MRI) revealed ventriculomegaly and disproportionately enlarged subarachnoid space (DESH). Her symptoms improved after a tap test, and an L

A novel ventriculoperitoneal shunt flow sensor based on electrically induced spatial variation in cerebrospinal fluid charge density.

Introduction: Ventriculoperitoneal (VP) shunts divert cerebrospinal fluid (CSF) out of cerebral ventricles in patients with hydrocephalus or elevated intracranial pressure (ICP). Despite high failure rates, there exist limited clinically viable solutions for long-term and continuous outpatient monitoring of CSF flow rate through VP shunts. We present a novel, low-power method for sensing analog CSF flow rate through a VP shunt premised on induced spatial electrical charge variation. Methods: Two geometric variants of the proposed sensing mechanism were prototyped: linear wire (P1) and cylindri

Primary pure and nonsecreting embryonal carcinoma of the anterior third ventricle: a case report.

Central nervous system germ cell tumors (GCTs) account for less than 5% of primary brain tumors in children and adolescents but continue to attract much attention. To the best of our knowledge, a primary pure and nonsecreting embryonal carcinoma of the anterior third ventricle has never been previously reported. A 15-year-old boy presented with signs of increased intracranial pressure for the past 2 weeks complicated by 2 episodes of generalized tonic-clonic seizures 1 day before admission. Neurological examination was normal, and funduscopic examination disclosed a grade II papilledema bilate

[Options for the symptomatic treatment of chronic neurological gait disorders].

Gait and mobility impairments are common and relevant in patients with chronic neurological disorders. It reduces the health-related quality of life and induces falls with morbidity. Symptomatic treatment options are therefore necessary in order to improve the health status of patients with neurological disorders.By means of a selective literature research focusing on studies with specific gait-related outcome measures. We discuss the differential treatment options for (1) hypokinetic gait disorders (Parkinson´s disease, Normal pressure hydrocephalus, vascular encephalopathy), (2) gait unstead

The Role of the Periaqueductal Gray Matter in Lower Urinary Tract Function.

The periaqueductal gray matter (PAG), as one of the mostly preserved evolutionary components of the brain, is an axial structure modulating various important functions of the organism, including autonomic, behavioral, pain, and micturition control. It has a critical role in urinary bladder physiology, with respect to storage and voiding of urine. The PAG has a columnar composition and has extensive connections with its cranially and caudally located components of the central nervous system (CNS). The PAG serves as the control tower of the detrusor and sphincter contractions. It serves as a bri

Treatment of Older Parkinson's Disease.

Parkinson's disease (PD) is the second most common neurodegenerative disorder after Alzheimer's disease. The prevalence of PD increases with age. The spectrum of clinical features, the rate of progression of the disease, the burden of nonmotor symptoms, and the response to medications are different in older patients with PD from the relatively younger patients. Management of symptoms of PD in older patients is challenging because of possible existence of several age-related systemic illness. While dealing with older patients, it is crucial not to attribute all the physical symptoms to PD. Thor

Tonsillar ectopia and headaches.

Tonsillar ectopia, encompassing slight descent of the cerebellar tonsils and Chiari I malformations, are disorders observed routinely in older children and adults and are believed to be an acquired form of the Chiari malformations. This entity is different from the other Chiari malformations in that hydrocephalus plays no role in its evolution. More likely it is a disorder of para-axial mesoderm, characterized by posterior fossa hypoplasia and content overcrowding, and not an embryologic anomaly of neuroectoderm. The prevalence of tonsillar ectopia to my knowledge has not been studied. The inc

[Two cases of symptomatic arachnoid cysts in elderly patients--a comparison and analysis with child cases].

A symptomatic arachnoid cyst in an elderly patient is rare. We report two cases of symptomatic arachnoid cysts in elderly patients. The first case is that of a 73-year-old woman complaining of headache and speech disturbance. She had an arachnoid cyst in the left interhemispheric fissure. This is rare. Only two interhemispheric arachnoid cysts in the elderly have ever been reported. The other case is that of a 64-year-old woman, having right hemiparesis and dementia and a cyst is shown in her left temporal lobe. Both patients underwent a cystectomy which resulted in the disappearance of their

An CSF anamalous molecular form of acetylcholinesterase in demented and non-demented subjects.

Analysis of 139 CSF samples from living subjects, using iso-electric focusing in polyacrylamide gels, demonstrated an anomalous molecular form of acetylcholinesterase (AChE). This form was present in 84 of 87 patients with a clinical diagnosis of Alzheimer's disease (AD), 28 of whom have died and in whom histopathological confirmation of AD was obtained. The abnormal AChE form was also present in 22 of 23 patients with clinical dementia not regarded as AD type. In the six patients who died this abnormal AChE form was found in three cases of multi-infarct dementia, one with cerebral glioma with

The surgical results of endoscopic third ventriculostomy in long-standing overt ventriculomegaly in adults with papilledema.

OBJECTIVE: Longstanding overt vetriculomegaly in adults (LOVA) is a type of chronic hydrocephalus presumed to begin during infancy, which manifests in adults after a long and slow clinical course. Only a quite small number of LOVA case series have been published, controversies regarding optimal management still exist. The authors describe a series of symptomatic LOVA patients with papilledema treated successfully using endoscopic third ventriculostomy (ETV) at a single institution. PATIENTS AND METHODS: In the past 7 years, 4 LOVA patients with papilledema were surgically treated using ETV. C

Cyclic vomiting syndrome: contribution to dysphagic infant death.

Vomiting involves the simultaneous violent contraction of abdominal and diaphragm muscles to produce a high pressure on the stomach. The heart right atrium forms a through path from IVC to SVC, so the high intra-abdominal pressure will drive blood from abdominal contents into the head. Normally internal viscous drags in organs will limit the volume leaving them during a single vomiting event. However, repetitive vomiting such as occurs in cyclic vomiting syndrome (CVS) may drive sufficient blood into head veins to produce extreme venous hypertension. Dysphagic infant death is essentially a hea

Resolution of neonatal posthemorrhagic ventricular dilation coincident with patent ductus arteriosus ligation: case report.

Preterm infants commonly present with a hemodynamically significant patent ductus arteriosus (hsPDA). The authors describe the case of a preterm infant with posthemorrhagic ventricular dilation, which resolved in a temporally coincident fashion to repair of hsPDA. The presence of a PDA with left-to-right shunting was confirmed at birth on echocardiogram and was unresponsive to repeated medical intervention. Initial cranial ultrasound revealed periventricular-intraventricular hemorrhage. Follow-up serial ultrasound showed resolving intraventricular hemorrhage and progressive bilateral hydroceph

Clinical Reasoning: A 75-Year-Old Man With Dementia, Incontinence, and Gait Dysfunction.

Idiopathic normal pressure hydrocephalus (iNPH) is a progressive neurological disorder characterized by gait apraxia, cognitive decline, and urinary incontinence. It can be difficult to diagnose iNPH as the symptoms may overlap with other neurodegenerative diseases including cervical spondylotic myelopathy. Cervical spondylotic myelopathy is a progressive degenerative disease in which compression of the cervical spinal cord causes gait disturbances and imbalance, loss of dexterity and strength in the hands, and, at late stages, urinary dysfunction. As with iNPH, increased age is associated wi

[Alzheimer's disease in Japan and other countries: review of epidemiological studies in the last 10 years].

Alzheimer disease (AD) is the most common type of dementia. In this review article, we summarized the current findings concerning the prevalence and incidence of AD in Japan and other countries; these findings were reported in the last 10 years. The prevalence of dementia among individuals aged more than 65 and more than 75 years was 2.4%-8.4% and 15.8%, respectively; this was determined according to the DSM-IV or DSM-III-R criteria. Further, the worldwide ratio of AD and vascular dementia (VaD) was 1.8-4.8, which was calculated according to the NINCDS-ADRDA and NINDS-AIREN criteria. In Japan,

Analysis of intracranial pressure during and after the infusion test in patients with communicating hydrocephalus.

The cerebrospinal fluid (CSF) infusion test is used to evaluate the dynamics of CSF circulation in patients with communicating hydrocephalus and is based on constant-rate infusion of the normal saline into cerebrospinal fluid space. The aim of the study was to refine methods of the analysis of intracranial pressure (ICP) recorded during and after the infusion test. The mathematical model of cerebrospinal fluid circulation was extended by the equation describing ICP decrease after the infusion. The nonlinear least-squares method of Levenberg-Marquardt was used to estimate the parameters describ

Sulfonylurea Receptor-1: A Novel Biomarker for Cerebral Edema in Severe Traumatic Brain Injury.

OBJECTIVES: Cerebral edema is a key poor prognosticator in traumatic brain injury. There are no biomarkers identifying patients at-risk, or guiding mechanistically-precise therapies. Sulfonylurea receptor-1-transient receptor potential cation channel M4 is upregulated only after brain injury, causing edema in animal studies. We hypothesized that sulfonylurea receptor-1 is measurable in human cerebrospinal fluid after severe traumatic brain injury and is an informative biomarker of edema and outcome. DESIGN: A total of 119 cerebrospinal fluid samples were collected from 28 severe traumatic bra

Feasibility of a Short-Stay Lumboperitoneal Shunt Pathway Based on Perioperative Optimization and Individualized Discharge Decision-Making: A Pilot Before-After Study.

Background: Lumboperitoneal (LP) shunt surgery is an established treatment for idiopathic normal pressure hydrocephalus (iNPH). In Japan, patients undergoing LP shunt surgery are often hospitalized for several days to more than one week after surgery, even in uncomplicated cases, reflecting concerns regarding early complications, cerebrospinal fluid overdrainage, and discharge readiness in older adults. This study evaluated the feasibility and short-term safety of a perioperative optimization pathway for planned short-stay hospitalization after LP shunt surgery. Methods: This single-center ret
