Cross-power spectral density analysis of pursuit tracking. Evaluation of central and peripheral pathology.

Twenty-one normal subjects were evaluated for pursuit tracking performance at frequencies of .2, .4, .8, 1.2 and 1.6 Hz. Measures of phase (lead or lag of target), gain (amplitude of response) and spectral purity (distortion of smooth tracking) were obtained by a cross-power spectrum analysis of the stimulus input and the eye movement output. It was determined that normal subjects could quite adequately track a target moving at .4 Hz. After establishing this parameter for normals, 43 patients with central and peripheral pathology related to the vestibulo-oculomotor system were tested at this f

Tear production after vestibular schwannoma surgery and intermediate nerve function.

OBJECTIVE: To evaluate the function of the intermediate nerve, the authors objectively quantified tear production before and after vestibular schwannoma (VS) surgery. METHODS: This prospective cohort study comprised 111 patients who underwent resection of unilateral VS. Tear production was objectively assessed in both eyes in millimeters using the Schirmer test. The difference between tear production of the eye on the nontumor side and that of the tumor side was assessed, before and after surgery. Sixty randomly selected patients had a second postoperative Schirmer test to assess change in te

Big Stimulus, Little Ears: Safety in Administering Vestibular-Evoked Myogenic Potentials in Children.

BACKGROUND: Cervical and ocular vestibular-evoked myogenic potentials (VEMPs) have become common clinical vestibular assessments. However, VEMP testing requires high intensity stimuli, raising concerns regarding safety with children, where sound pressure levels may be higher due to their smaller ear canal volumes. PURPOSE: The purpose of this study was to estimate the range of peak-to-peak equivalent sound pressure levels (peSPLs) in child and adult ears in response to high intensity stimuli (i.e., 100 dB normal hearing level [nHL]) commonly used for VEMP testing and make a determination of w

Prevalence of Asymptomatic Superior Semicircular Canal Dehiscence on High Resolution CT scan in Omani Population.

OBJECTIVES: Incidental detection of superior semicircular canal dehiscence (SSCD) in patients without symptoms may lead to misinterpretation if not correlated with clinical presentation, examination findings, and vestibular function testing. This study aimed to assess the rate of radiological SSCD in the asymptomatic Omani population, as the first study of its kind in the country and the Gulf region. METHODS: A retrospective review was conducted of all asymptomatic patients who underwent high-resolution computed tomography (HRCT) of the temporal bones at a tertiary care facility between 2017

Expanding the mutational spectrum of LZTR1 in schwannomatosis.

Schwannomatosis is characterized by the development of multiple non-vestibular, non-intradermal schwannomas. Constitutional inactivating variants in two genes, SMARCB1 and, very recently, LZTR1, have been reported. We performed exome sequencing of 13 schwannomatosis patients from 11 families without SMARCB1 deleterious variants. We identified four individuals with heterozygous loss-of-function variants in LZTR1. Sequencing of the germline of 60 additional patients identified 18 additional heterozygous variants in LZTR1. We identified LZTR1 variants in 43% and 30% of familial (three of the seve

[Assessment of time to stimulate the function of the type of AAI in the distant observation in patients with sick sinus syndrome].

UNLABELLED: For the first time in 1966 vestibular stimulation has been applied. This idea is implemented in a patient with sinus node dysfunction and normal AV conduction. The aim of the study was to evaluate the time of AAI stimulation until the first reimplantation of stimulating system and to determine the cause of it. MATERIAL AND METHODS: Work is retrospective in nature. The analysis included 251 patients who were subjected to AAI pacemaker implantation in the period from December 1988 to January 2009. The group consisted of 150 women and 101 men. The average age of patients at the time

[Additive treatment for central vestibular vertigo].

We have observed in our own practice that numerous patients with primary symptoms of vertigo exhibit cervical segmental muscular imbalance and increased tension in the masticatory musculature. This is frequently associated with functional blockades, especially in the joints of the head and upper cervical spine. Particularly important are special receptors of the small vertebral joints and muscle insertions at the cervicocranial transition. Evidence indicates that there are neuroanatomic structures between these receptors and the central vestibular and cochlear core area of the brain stem, whic

Dynamic balance function in phasic cervical dystonia following Botulinum toxin therapy.

Previous research by our group revealed normal dynamic balance function in pure tonic cervical dystonia (CD) with impaired equilibrium in phasic CD patients investigated at least 3 months following Botulinum toxin (BtxA) treatment. The current study was performed to determine whether impaired dynamic equilibrium in phasic CD is influenced by symptomatic treatment with BtxA. Dynamic balance was tested in 20 patients with phasic CD on a dynamic platform with a cylindrical curved base (stabilometer) 4 weeks following BtxA treatment. Balance was assessed by the linear displacement of the platform

Facial sensibility in adolescents with and without clefts 1 year after undergoing Le Fort I osteotomy.

Static two-point discrimination, pressure, and vibratory threshold values were measured at standard coordinates in the area corresponding to the infraorbital nerve distribution in 59 adolescents 1 year after undergoing LeFort I osteotomy. The patients were divided into three groups: those born with unilateral cleft lip and palate (n = 30; mean age, 18 years; SD = 2); those with bilateral cleft lip and palate (n = 12; mean age, 19 years; SD = 3); and those without a cleft (n = 17; mean age, 19 years; SD = 3). One year after the osteotomy, none of the patients complained of altered sensibility i

Effects of transcutaneous electrical nerve stimulation on motion sickness induced by rotary chair: a crossover study.

OBJECTIVES: Motion sickness (MS) is evoked by the conflict among somatosensory, visual, and vestibular input. Some of the MS symptoms and signs are mediated by activation of the autonomic nervous system (ANS). Transcutaneous electrical nerve stimulation (TENS), a maneuver used for pain control, was found to influence cardiovascular responses through ANS reflex, and to enhance motor function, visuospatial abilities, postural control, and cognitive function. The purpose of the present study is to investigate the effects of TENS on MS. SUBJECTS AND DESIGN: Fifteen (15) healthy young men particip

Delayed blink R1 latency in a patient with trigeminal neuralgia due to a contralateral vestibular schwannoma: An illustrative case.

BACKGROUND: Although the blink reflex (BR) is effective in objectively evaluating trigeminal neuropathy, few studies have demonstrated its effect on trigeminal neuralgia (TN). The authors report a patient with TN due to contralateral vestibular schwannoma (VS) functionally diagnosed by delayed R1 latency of the BR. CASE DESCRIPTION: A 36-year-old man presented with left-sided deafness and paroxysmal facial pain in the right V1-3 area. Magnetic resonance imaging (MRI) showed a solid cystic mass compressing the right pons and left brainstem at the left cerebellopontine angle. Although preoperat

Video-head impulse test results in patients with Menière's disease related to duration and stage of disease.

BACKGROUND: The video-head impulse test employs the vestibulo-ocular reflex (VOR) to assess vestibular function. To this day, no consensus has been reached among scientists in terms of whether or not vHIT results change in MD patients as the disease progresses. OBJECTIVE: To assess whether the vHIT is more often abnormal in later stages of MD compared to earlier stages. METHODS: We retrospectively analyzed patients with 'definite' MD who had undergone a vHIT and caloric test between 2012 and 2015. Patients were evaluated based on duration of disease in years (≤1, >1≤5, >5≤10, >10) and stage

Subclinical variability in visual function modulates visual dependence - independent of age.

Paradoxically visual dependence is reported to increase with age, contributing to falls risk, whereas visual function typically declines. This study assesses the relationship between age, objective and subjective measures of visual function and visual dependence, in healthy young and older adults. Forty-four healthy Young (YA; n = 32; 18 males, aged 26.2 ± 5.3 yrs.) and Older (OA; n = 12; 3 males, aged 62.4 ± 6.7 yrs.) adults were assessed for objective (visual acuity, contrast sensitivity, depth perception, and lower peripheral vision), and subjective visual function (VFQ-25) along with motio

Comprehensive Multimodal Diagnostics and Rehabilitation in Persistent Post-Concussion Syndrome Following Motor Vehicle Collision.

Motor vehicle collisions (MVCs) are among the most common mechanisms of mild traumatic brain injury (mTBI), and a meaningful proportion of affected individuals develop persistent post-concussion syndrome (PPCS) characterized by prolonged multidomain dysfunction. Conventional neuroimaging frequently fails to capture the underlying pathophysiology, leaving clinicians without objective diagnostic evidence. We describe a 57-year-old woman who sustained mTBI in a rear-end MVC in March 2023. Head computed tomography (CT) performed at the emergency department and a subsequent brain magnetic resonance

A controlled and sustained local gentamicin delivery system for inner ear applications.

OBJECTIVE: Intratympanic gentamicin injection (ITGI) has gained acceptance worldwide for the treatment of Ménière's disease. Reports assessing the efficacy of ITGI suffer from high variability between patients. This variability may be due to ITGI, which does not permit a sustained diffusion of gentamicin across the round window membrane. The present study investigates the effectiveness of a sustained local hydrogel system on the delivery of gentamicin into the inner ear for the treatment of Ménière's disease. METHODS: A matrix of hydrogel loaded with/without gentamicin was explored in vivo. G
