Squamous cell carcinoma of the vulva and adjacent lesions treated at Nagoya University Hospital from 1965 to 1997.

Japan has a lower incidence of vulvar squamous cell carcinoma (VSCC) than Western nations. To pin-point the reasons for this, we reviewed biopsy samples from all cases treated at Nagoya University Hospital over the past 33 years in order to investigate the background lesions for VSCC. Two of 36 VSCC patients had adjacent or coexisting lichen sclerosus (LS), 5 had squamous cell hyperplasia (SCH), and 16 had vulvar intraepithelial neoplasia (VIN). There were 8 cases in which these lesions were thought to be the origin of the VSCC, 1 in which keratinizing squamous cell carcinoma (KSC) was seen in

Carcinoma of the vulva in young women.

OBJECTIVE: To determine if young women with carcinoma of the vulva have a different risk factor history and outcome compared with older women. METHODS: We conducted a retrospective review of the medical records of 78 women treated at the Medical College of Georgia for squamous carcinoma of the vulva during 1979-1993. Women younger than 45 years were compared with those 45 and over for historic risk factors, treatment modality, and outcome. RESULTS: Over the study interval, the average presenting age of these patients decreased from 69 to 55 years. Women under 45 were found to have a stronger

[Study on the localization of human papillomavirus DNA in the vulvar disease by means of in situ hybridization].

We investigated human papillomavirus (HPV) DNA of vulvar diseases obtained from 18 cases of condyloma acuminatum, 3 cases of hyperplastic dystrophy without atypia, 4 cases of lichen sclerosus, 4 cases of Bowenoid papulosis, one case of Bowen disease and 8 cases of squamous cell cancer by in situ hybridization with biotinylated HPV 6, 11, 16 and 18 DNA probes. The results of in situ hybridization analysis showed that HPV 6/11 was positive in 94.4% (17/18) of condyloma acuminatum. In 17 cases of HPV 6/11 positive condyloma acuminatum, 3 cases were positive for HPV 16 and one for HPV 18, respecti

Vulvar pruritus--a symptom, not a disease.

Because the underlying causes are so varied, vulvar pruritus must be understood as a symptom and not as a disease entity in itself. Successful treatment depends on proper identification of the underlying disorder.

Quadrivalent Human Papillomavirus Vaccine and High-Grade Vulvovaginal Lesions.

IMPORTANCE: Human papillomavirus (HPV) vaccination has been associated with reduced risk of high-grade cervical lesions. However, evidence on its association with high-grade vulvovaginal lesions remains scarce. OBJECTIVE: To evaluate the association between quadrivalent HPV vaccination and high-grade vulvovaginal lesions and assess the population-level incidence reduction among birth cohorts eligible for various vaccination programs. DESIGN, SETTING, AND PARTICIPANTS: This population-based cohort study included women who were born between 1985 and 1998 and resided in Sweden between 2006 and

The potential impact of ACOG guideline changes on genital melanoma detection rates in females: a retrospective cross-sectional analysis and implications for clinical practice.

Genital melanoma (GM) is a rare but aggressive cancer that accounts for less than 5% of vaginal cancers and 0.2-0.8% of all melanomas in the United States. The 2018 American College of Obstetricians and Gynecologists (ACOG) guidelines reduced routine pelvic exams for women aged 65 and older, raising concerns about later-stage GM diagnosis, worse survival rates, and potential underreporting. This study investigated the impact of these guideline changes on female GM detection rates through a cross-sectional analysis of female patients aged 18 and older in the All of Us database from 2012 to 2023

Retrospective analysis of the diagnostic effectiveness of the sentinel lymph node biopsy (SLNB) in vulvar cancer.

OBJECTIVES: Inguinal lymphadenectomy used in the treatment of vulvar cancer often results in complications, such as lymphoedema or abnormal wound healing. Aim of this study was assessment of the diagnostic effectiveness of the sentinel lymph node biopsy (SLNB) procedure in patients treated due to vulvar cancer. MATERIAL AND METHODS: Eighty-four patients diagnosed with squamous cell vulvar carcinoma (FIGO I-IV) underwent preoperative lymphoscintigraphy with technetium 99 to map sentinel lymph node. During surgery sentinel lymph nodes were identified and resected, followed by complete bilateral

Is there a role for magnetic resonance imaging in the evaluation of inguinal lymph node metastases in patients with vulva carcinoma?

OBJECTIVE: To study the accuracy of magnetic resonance imaging (MRI) in lymph node detection in patients with vulva carcinoma. METHODS: Sixty patients with diagnosed vulva carcinoma underwent MRI examination for preoperative evaluation of lymph nodes. MRI images were read independently and retrospectively by two radiologists, both unaware of physical examination and surgery findings. The following characteristics of each lymph node with a short-axis diameter of >or=8 mm were recorded: size (axial, sagittal and coronal); aspect (homogeneous, with fatty center or partial fat); margin (smooth, l

Evaluation of different treatment modalities for vulvar intraepithelial neoplasia (VIN): CO(2) laser vaporization, photodynamic therapy, excision and vulvectomy.

OBJECTIVES: To evaluate various treatment modalities for vulvar intraepithelial neoplasia (VIN) in relation to possible risk factors for recurrence. METHODS: Retrospective review of 93 patients with VIN treated by CO(2) laser vaporization, photodynamic therapy with aminolevulinic acid (PDT), excision or vulvectomy. RESULTS: 40.4% of the 47 patients with laser vaporization, 48.1% of 27 patients with PDT, 42% of 12 patients with local excision and none of the 7 patients treated by vulvectomy experienced a relapse within a mean follow-up of 53.7 months. The risk for recurrence significantly inc

The value of routine follow-up in patients treated for carcinoma of the vulva.

BACKGROUND: Vulvar carcinoma patients traditionally are offered follow-up after their primary treatment because earlier diagnosis of recurrent disease is believed to improve chances for curative treatment. The objective of the current study was to determine the value of a strict routine follow-up protocol for the detection of recurrences in a large series of patients who were treated for carcinoma of the vulva. METHODS: Clinicopathologic data for patients with primary squamous cell carcinoma of the vulva who were treated between January 1990 and July 2000 were prospectively stored in a databa

Does long-term follow-up have a role for node negative squamous carcinoma of the vulva? The Gateshead experience.

A retrospective review was performed of 138 cases of squamous vulval cancer referred to Gateshead between 1986 and 1997, with a median follow-up of 48 months. Eighteen recurrences were detected, 11 within one year of surgery. All nine patients with groin/distant recurrence (including 4 presenting initially with local recurrence only) died of vulval cancer. Vulval pain, bleeding or other symptoms heralded all recurrences. Routine review was ineffective in detecting recurrence. Eight cases were detected by general practitioners, three by specialists, and one was self-diagnosed. Six of these had

Radiation treatment in recurrent squamous cell cancer of the vulva.

PURPOSE: To evaluate the treatment and outcome of recurrent vulvar cancer. METHODS AND MATERIALS: In a retrospective review of 26 women referred to the department of radiation oncology between 1982 and 1995, patient records were analyzed with respect to the findings at original surgery, the time to locoregional recurrence, the location of the recurrence, and the subsequent management and outcome. RESULTS: Sixteen recurrences were managed with a combination of surgery and radiotherapy, and the remainder with radiation treatment, combined with chemotherapy in some cases. The overall survival f

Long-term impact of postoperative radiotherapy in carcinoma of the vulva FIGO I/II.

PURPOSE: Between 1953 and 1978, postoperative radiotherapy was used as an adjuvant therapy for carcinoma of the vulva that had not been treated with radical vulvectomy. We evaluated long-term results and possible prognostic factors. METHODS AND MATERIALS: Ninety-two patients were treated. Surgical procedures were simple vulvectomy, electrocoagulation, or local excision. Radiotherapy doses to the vulva ranged from 0 to 90 Gy. All patients received radiotherapy to the inguinal lymph nodes, ranging from 30 to 60 Gy. Thirty-year retrospective follow-up was done evaluating the records and statisti

Vulvar papillomatosis, aceto-white lesions, and normal-looking vulvar mucosa evaluated by microscopy and human papillomavirus analysis.

BACKGROUND AND OBJECTIVES: To assess the presence of human papillomavirus (HPV) DNA in vulvar papillomatosis, since some women with this complaint have symptoms associated with HPV infection, such as itching, burning, and dyspareunia. GOAL: To reassure the patients that they do not have a transmissible viral disease, by excluding a HPV origin of their condition. STUDY DESIGN: Vulvar biopsy specimens from 22 females with vulvar papillomatosis, from 10 females with prominent aceto-white vulvar lesions, and from 14 healthy controls were analyzed histologically for signs of HPV infection and by

Predicting the course of disease in recurrent vulvar cancer - A subset analysis of the AGO-CaRE-1 study.

OBJECTIVE: In vulvar cancer (VSCC), the course of disease with regard to localization of recurrence and relation of different recurrence sites is poorly described. METHODS: The AGO CaRE-1 study is a retrospective survey of treatment patterns and prognostic factors in vulvar cancer. Patients (pts) with primary VSCC, FIGO stage ≥1B treated in Germany from 1998 to 2008 were included in a centralized database (n = 1618). In the current subgroup analysis, different sites of primary recurrence and their impact on disease course and survival were analyzed using multistate and competing risks methods
