Endoscopic management of duodenal fistula in a patient operated after abdominal injury.

Post-traumatic duodenal injuries constitute a relatively rare group among this type of lesions reported in the abdominal structures. In the vast majority of cases, a post-traumatic duodenal injury is accompanied by damage to other important organs. The surgical management of duodenum injuries poses a high risk of life-threatening complications with duodenal fistula among the most common. In some cases, the combination of basic and advanced surgical procedures and intensive conservative treatment is insufficient to treat the complication. The progress in endoscopic techniques and the applicatio

Predictors of blunt abdominal aortic injury in trauma patients and mortality analysis.

OBJECTIVE: Blunt abdominal aortic injury (BAAI) occurs in less than 0.1% of blunt traumas. A previous multi-institutional study found an associated mortality rate of 39%. We sought to identify risk factors for BAAI and risk factors for mortality in patients with BAAI using a large national database. We hypothesized that an Injury Severity Score of 25 or greater, and thoracic trauma would both increase the risk of mortality in patients with BAAI. METHODS: The Trauma Quality Improvement Program (2010-2016) was queried for individuals with blunt trauma. Patients with and without BAAI were compar

The summary of experience of abdominal vascular injury related to posterior lumbar surgery.

PURPOSE: To study the clinical and pathophysiologic characteristics and summarize the experience of treatment of abdominal vascular injury related to lumbar surgery. METHODS: We analyzed patients who suffered abdominal vascular injury during lumbar surgery in our hospital retrospectively and reviewed related literature in the PUBMED database from 2002 to 2017. Combined with the existing treatment options and outcomes, we investigated further and summarized our findings. RESULTS: With the data from our hospital, four cases of injuries were included, i.e., left common iliac artery and vein (CI

Risk factors for and the prevention of acute kidney injury after abdominal surgery.

Postoperative acute kidney injury in patients undergoing abdominal surgery is not rare and often results in bad outcomes for patients. The incidence of postoperative acute kidney injury is hard to evaluate reliably due to its non-unified definitions in different studies. Risk factors for acute kidney injury specific to abdominal surgery include preoperative renal insufficiency, intraabdominal hypertension, blood transfusion, bowel preparation, perioperative dehydration, contrast agent and nephrotoxic drug use. Among these, preoperative renal insufficiency is the strongest predictor of acute ki

Spontaneous reduction of ileoileal adult intussusception after blunt abdominal injury.

This is the first case report of an adult who had spontaneous reduction of ileoileal intussusception occurred after punching to the abdomen. A 40-year-old man was brought to our emergency department by ground ambulance due to pounding and punching a few hours ago. Physical examination showed multiple dermabrasions on his face, abdomen, and lower extremities. All other examinations were unremarkable except that of mild abdominal pain. Laboratory results gave no clues. On abdominal x-ray, paucity of intestinal gas, pseudomass and surrounding gas appearances were visible. No nausea or vomiting oc

[The morphological characteristic of an experimental rupture of the liver for the simulation of the blunt abdominal injury].

The strength and the energy of a striking blow with a clenched fist may be as high as 300 kgf (3,000 H) and 824.4 J respectively. Pressing a human body against a hard surface leads to a deep chest deflection while the rupture of the liver is associated with a lower blow strength and energy (2,000 H and 141.5 J respectively). In such situations, the shockproof mechanisms of the injury prevail giving rise to antishock peripheral and central ruptures of the liver due to the general deformation and displacement of this organ in the direction of the blow. In the absence of barriers preventing the i

Blunt abdominal aortic injury: a Western Trauma Association multicenter study.

BACKGROUND: Blunt abdominal aortic injury (BAAI) is a rare injury. The objective of the current study was to examine the presentation and management of BAAI at a multi-institutional level. METHODS: The Western Trauma Association Multi-Center Trials conducted a study of BAAI from 1996 to 2011. Data collected included demographics, injury mechanism, associated injuries, interventions, and complications. RESULTS: Of 392,315 blunt trauma patients, 113 (0.03%) presented with BAAI at 12 major trauma centers (67% male; median age, 38 years; range, 6-88; median Injury Severity Score [ISS], 34; range

Gastric intramural and portal venous gas following blunt abdominal injury.

INTRODUCTION: Gastric emphysema or pneumatosis is a rare finding. Early endoscopy and urgent laparotomy is advised in post-trauma patients. CASE PRESENTATION: A 29 year old man presented with blunt abdominal injury following a high-speed motorbike crash He complained of abdominal pain and abdomen was distended. CT abdomen revealed air in the gastric wall with disruption of gastric mucosa. He had normal white cell counts, bleeding parameters and blood gases. He was treated conservatively with nasogastric decompression, intravenous analgesics and antibiotics with which he recovered well. CONCL

Laparoscopic repair of traumatic abdominal wall hernia from handlebar injury.

A 14-year-old boy was seen at an outside hospital after falling over the handlebar of his bicycle and was discharged home. He was subsequently seen in our emergency department with complaints of persistent abdominal pain. A computed tomography scan of the abdomen revealed disruption of the muscles of the upper right abdominal wall containing the hepatic flexure of the colon, with a small amount of intraperitoneal free fluid noted. The patient underwent laparoscopic exploration using 3 ports (2-5 mm and 1-12 mm) and 2 separate stab incisions. The traumatic abdominal wall hernia was repaired wit

[A seatbelt sign following a car accident: look for internal abdominal injury].

We present three patients, a 55-year-old man, a 69-year-old woman and a 25-year-old man, with a seatbelt sign following a car accident. All 3 patients exhibited various injuries that may occur in the case of a blunt trauma, for example rib fractures. In one patient, however, symptoms of internal abdominal injury occurred several days after the accident. The presence of a seatbelt sign is associated with an increased risk of internal abdominal injury. We therefore advise a CT scan of the abdomen in patients who present with a seatbelt sign, even if abdominal ultrasound does not reveal signs of

Normotensive ischemic acute kidney injury as a manifestation of intra-abdominal hypertension.

Intra-abdominal hypertension (IAH), leading to abdominal compartment syndrome (ACS), is a frequent cause of acute kidney injury (AKI) in surgical and trauma intensive care units not commonly recognized by nephrologists. Multiple organ systems are often affected and frequently culminate in disastrous outcomes. The diagnosis of AKI as a manifestation of IAH requires a high index of clinical suspicion, especially, because laboratory and imaging studies are unreliable. Early recognition and treatment of the condition is associated with good clinical outcome. We report a typical case of normotensiv

An abdominal aortic rupture due to seatbelt blunt injury: report of a case.

A 66-year-old man, who was a passenger in a car involved in a low-speed head-on motor vehicle accident, was rushed to our hospital. His abdomen was tender and distended. An enhanced computed tomography scan showed a massive retroperitoneal hematoma, and its three-dimensional imaging revealed an active leak of the contrast medium from the aortic bifurcation. He went into shock, and was immediately transferred to the operating theater. Through a median laparotomy, a ruptured site measuring 5 mm in diameter was found at the aortic bifurcation and it was closed with sutures under a proximal aortic

[Complete avulsion of ureter caused by abdominal blunt injury: a case report].

A 21-year-old woman who had been injured in a traffic accident appeared with abdominal pain and macroscopic hematuria. Computed tomography (CT) performed 6 hours after the injury showed extravasation of contrast medium in the right retroperitoneal space. Retrograde pyelography (RP) showed the interruption of right ureter at the site of ureteropelvic junction. We performed an abdominal operation 15 hours after the injury under the diagnosis of right ureteral avulsion. We observed a completely separated right ureter at the ureteropelvic junction, and performed an end to end anastomosis. The pati

Intestinal stenosis from mesenteric injury after blunt abdominal trauma.

We report a case in which blunt abdominal trauma resulted in injury to the mesentery with subsequent ischemic stricture of the adjacent small bowel. We present CT images at the time of trauma and 5 weeks later when clinical signs of intestinal obstruction occurred. We include images of enteroclysis and angiography of this uncommon sequela of blunt abdominal trauma. At surgery, a stenotic small bowel loop was found adjacent to a healed defect in the mesentery. Histological examination of the resected segment showed mucosal and submucosal ischemia with mucosal ulceration, mural inflammation, and

Seat belt injury presenting as bilateral abdominal wall herniations in a child.

The constellation of abdominal injuries caused by seat belts in automobile accidents usually consists of intraabdominal solid organ and hollow viscus injuries. We present a two-and-one-half-year-old patient with an unusual constellation of seat belt injuries that presented as bilateral abdominal herniations. We feel that an abdominal hernia caused by a seat belt injury should alert the clinician to the probability of other severe intraabdominal injuries. The uniqueness of this case is compounded by the way in which the seat belt was misused.
