Adenosine in the treatment of maternal paroxysmal supraventricular tachycardia.

Paroxysmal supraventricular tachycardia is the most common sustained cardiac arrhythmia in pregnant women. Because nearly 50% of these supraventricular tachyarrhythmias fail to respond to vagal maneuvers, other therapies are used, including electrocardioversion and pharmacologic agents. Propranolol, verapamil, and adenosine have Food and Drug Administration-approved labeling for acute termination of supraventricular tachycardia. Verapamil has been the most commonly used agent in the general population but it has several shortcomings, such as its potential to cause or exacerbate systemic hypote…

Adenosine in the treatment of paroxysmal supraventricular tachycardia in children.

Intravenous bolus adenosine was given to four pediatric patients aged 1 month to 8 years who had paroxysmal supraventricular tachycardia that had not responded to conventional medical therapy. Adenosine (one to three doses) was successful in converting the arrhythmia to normal sinus rhythm in all four cases, and no side effects of the drug were noted.

[Effect of adenosine on carotid chemoreceptor activity in the rabbit].

The response of single carotid chemoreceptor afferent fibers upon adenosine acting on the carotid body (CB) was examined in 39 urethan-anesthetized rabbits. Totally 73 units with spontaneous discharge were recorded in our experiment. The results were as follows: (1) Of 55 units, 51 showed an increase in discharge frequency from 0.76 +/- 0.10 to 1.53 +/- 0.23 imp/s. A few new units were recruited concomitantly in response to intracarotid injection of adenosine (10 micrograms/kg). (2) Adding adenosine in the doses of 0.5, 1.5, 10, 50 and 100 micrograms/kg to the perfusate passing through the iso…

Serum adenosine deaminase in viral and bacterial pneumonia.

We measured the activity of serum adenosine deaminase (ADA) in paired sera from 171 military conscripts with radiographically verified pneumonia. Patient serum samples were selected on the basis of serologic analyses identifying as single etiologic agents Streptococcus pneumoniae in 29 patients, Haemophilus influenzae in 7, Mycoplasma pneumoniae in 43, adenovirus in 24, influenza A or B in 12, and parainfluenza in 5 patients. In 14 patients Neisseria meningitidis and in 31 Chlamydia spp were considered the main etiologic agent. Compared with a control group of 45 healthy men, the ADA activity …

Verapamil or adenosine for the immediate treatment of supraventricular tachycardia.

A comparison of verapamil with adenosine for the immediate treatment of supraventricular tachycardia was made from a retrospective review of 164 spontaneous episodes of paroxysmal tachycardia in 43 patients. Verapamil administered to 33 patients restored sinus rhythm in 91 of 112 episodes (81 per cent). Hypotension occurred in 9 per cent of episodes. Adenosine terminated 94 per cent of episodes of supraventricular tachycardia in 25 patients. The arrhythmia recurred shortly after adenosine restored sinus rhythm in 20 episodes. Transient side effects were common. Fifteen patients were treated wi…

Imidazodiazepinediones: a new class of adenosine receptor antagonists.

A series of imidazo[4,5-e][1-4]diazepine-5,8-diones were synthesized from hypoxanthines. Certain of these cyclic homologues of caffeine, theophylline, theobromine, 3-isobutyl-1-methylxanthine, and enprofylline were inhibitors of binding of adenosine analogues to rat brain A1 and A2 adenosine receptors and were antagonists of A2 adenosine receptors stimulatory to adenylate cyclase in rat PC12 cell membranes. Activity at adenosine receptors was lower than the corresponding xanthines, perhaps because imidazodiazepinediones contain a boat-shaped seven-membered ring rather than the planar heteroary…

Effects of adenosine on human coronary arterial circulation.

Adenosine is a potent vasodilator used extensively to study the coronary circulation of animals. Its use in humans, however, has been hampered by lack of knowledge about its effects on the human coronary circulation and by concern about its safety. We investigated in humans the effects of adenosine, administered by intracoronary bolus (2-16 micrograms), intracoronary infusion (10-240 micrograms/min), or intravenous infusion (35-140 micrograms/kg/min) on coronary and systemic hemodynamics and the electrocardiogram. Coronary blood flow velocity (CBFV) was measured with a 3F coronary Doppler cath…

Haemodynamic and metabolic effects of infused adenosine in man.

1. Haemodynamic and metabolic effects of intravenous infusion of adenosine, an endogenous vasodilator, were studied in healthy humans. 2. Catheters were inserted into pulmonary and brachial arteries and into the hepatic and subclavian veins. Cardiac output was determined according to the Fick principle, and splanchnic blood flow was measured by using extraction of Indocyanine Green. Skin blood flow was estimated by a laser Doppler technique, calf blood flow by venous occlusion plethysmography and skeletal muscle and adipose tissue blood flow by a local isotope clearance technique. 3. Adenosine…

Adenosine is required for ethanol-induced heterologous desensitization.

Recent evidence suggests that ethanol initially causes an increase in receptor-dependent cAMP levels, followed by heterologous desensitization of receptors coupled to GS after chronic exposure. Here we investigated the role of adenosine in mediating these responses. We found that ethanol caused accumulation of extracellular adenosine in NG108-15 and S49 lymphoma cells. This adenosine activated adenosine receptors to increase intracellular cAMP levels. The addition of adenosine deaminase, to degrade accumulated extracellular adenosine, or isobutyl-methylxanthine, an adenosine receptor antagonis…

Adenosine receptor mediated stimulation of ventilation in man.

We wanted to examine how adenosine stimulates ventilation in man. Bolus doses of adenosine were given i.v. in an antebrachial vein in multiples of 2.65 mg. The minute ventilation was increased by adenosine 5.3 to 15.9 mg (median values) from control 12.6 +/- 1.9 l min-1 to 42.5 +/- 4.7 l min-1 in a dose-dependent manner. The adenosine receptor antagonist theophylline, 58.3 +/- 3.3 (mean +/- SEM) mumol l-1 plasma, inhibited the response by approximately 25%. Dipyridamole 10 mg, an adenosine uptake blocker, enhanced the effect of adenosine by approximately 60%. The ventilation was not affected b…

Central gastric antisecretory action of adenosine in the rat.

The effects of intracerebroventricularly (i.c.v.) administered adenosine and some of its analogues on gastric secretion were studied in rats. The compounds inhibited the gastric output of acid, pepsin and fluid in pylorus-ligated rats in a dose-dependent manner with an order of potency: 5'-N-ethylcarboxamidoadenosine (NECA) greater than (-)N6-phenylisopropyladenosine (R-PIA) greater than (+)N6-phenylisopropyladenosine (S-PIA) greater than adenosine. Pretreatment with 10 and 30 mg/kg of theophylline i.v. or 5 mg/kg of 8-phenyltheophylline s.c. did not modify the antisecretory effect of 0.1 micr…

Respiratory effects of a long-acting analog of adenosine.

The effect on respiration, measured as integrated phrenic nerve activity, of an analog of adenosine, N6(L-2-phenylisopropyl)adenosine (PIA), was determined in 26 paralyzed, vagotomized and glomectomized cats whose end-tidal pCO2 was kept constant by means of a servocontrolled ventilator. Whether given intravenously or into the third cerebral ventricle, PIA caused a dose-related depression of respiration, involving both tidal activity and respiratory frequency. In a group of 6 cats, the effects of intraventricular PIA or its vehicle alone on medullary extracellular fluid (ECF) pH were also dete…

Stimulation of electrolyte secretion in rabbit colon by adenosine.

Serosal addition of adenosine after inhibition of adenosine deaminase with deoxycoformycin increases short-circuit current (Isc) and tissue conductance of isolated epithelia of rabbit descending colon. In the presence of Cl this increase in Isc results from a reversal of electrically neutral Cl absorption to rheogenic Cl secretion. When Cl is absent the stimulating effect of adenosine on Isc is reduced to one-third and appears to be brought about by HCO3 secretion. Under all conditions active Na transport remains unaltered. Adenosine-induced electrolyte secretion is markedly decreased by seros…

Adenosine deaminase is not required for the generation of superoxide anion.

Neutrophils and macrophages generate superoxide anion during the respiratory burst in response to various stimuli, including microorganisms. It has recently been proposed that an important source of superoxide anion during the respiratory burst that stimulates murine macrophages is the sequential metabolism of adenosine via adenosine deaminase and xanthine oxidase to uric acid. Thus, the immunodeficiency state associated with adenosine deaminase deficiency may be caused at least in part by a defect in superoxide anion generation. The ability to generate superoxide anion of stimulated neutrophi…

[Pathogenetic significance of adenosine deaminase in autoimmune diseases].

UNLABELLED: Adenosine deaminase (ADA: E.C.3.5.4.4.) is one of the catabolic enzymes of purine nucleotide. In 1972 Gibllet has discovered two cases of ADA deficiency combined with severe immunodeficiency (ADA-. SCID) with remarkable dysfunction in both T and B cell. Thereafter, it has been believed by many investigators that deficiency of purine metabolic enzyme had played important role in immune function mechanism. Rheumatoid arthritis (RA) is one of the autoimmune diseases which is very frequently seen, but its pathogenesis is not distinct yet. This study has shown how the catabolic pathway …
