Nortriptyline in the treatment of panic disorder and agoraphobia with panic attacks.

Thirty-four consecutive patients with panic disorder or agoraphobia with panic attacks were treated with nortriptyline at the LAC-USC Medical Center's Anxiety Disorders Clinic. Fourteen (67%) of the 21 completers totally lost their panic attacks, five (24%) showed partial improvement, and two (10%) showed no improvement. The relationship of treatment outcome to pretreatment and posttreatment measures of depression is discussed, in addition to the potential role of nortriptyline in treating panic attacks in clinical practice.

Psychotherapeutic approaches to the treatment of panic attacks, hypochondriasis and agoraphobia.

Various theoretical orientations suggest different treatment approaches to the agoraphobic syndrome. In this paper a self-psychology formulation is applied to the treatment of panic attacks, hypochondriasis and agoraphobia. When these symptoms are viewed as manifestations of self-fragmentation, their treatment can be conceptualized as a process of improving self-cohesiveness. The initial management of symptoms sets in motion a self-object transference which is the foundation of treatment. As therapy proceeds, the defences against affect which are associated with a primary defect in anxiety reg…

Tritiated imipramine binding to platelets is decreased in patients with agoraphobia.

Controversy exists regarding the relationship between anxiety states and major depression. We studied the binding of tritiated imipramine to platelet membranes in order to determine if patients with agoraphobia and panic attacks differed from depressed subjects or healthy volunteers on this biological parameter. Mean (+/- SD) Bmax and Kd values were significantly lower in patients with agoraphobia and panic attacks (787 +/- 276 fmole/mg protein and 0.35 +/- 0.14 nM, respectively) than in healthy volunteers (1237 +/- 201 fmole/mg protein and 0.71 +/- 0.37 nM, respectively). In addition, patient…

A multimodal approach to the treatment of agoraphobia: four case studies.

It is argued that while in vivo exposure methods may be the treatments of choice for the agoraphobic patient, other approaches may be required for complete or significant symptom-removal. Four females, each meeting the DSM-III criteria for a diagnosis of agoraphobic disorder, were treated with in vivo exposure. However, for three of the four patients symptom removal occurred only when other methods were employed on an as-required basis for each particular patient. Two of the four patients were asymptomatic both at post-treatment and follow-ups. Over the course of the treatment, the remaining t

Paradoxical intention in the treatment of agoraphobia and other anxiety disorders.

Following a description of the paradoxical intention procedure and a brief review of the general behavioral literature relevant to this technique, the authors focus on the role of paradoxical treatment programs for agoraphobic clients. The cognitive and exposure components of paradoxical intention are discussed and the procedure is compared and contrasted with other cognitive behavioral methods on one hand, and with flooding on the other. Finally, the authors provide several ideas for future research.

The outcome of behavior therapy for agoraphobia in relation to marital adjustment.

After intensively treating 18 agoraphobic patients with 13 1/2 hours of exposure in vivo, we examined the effects on both patients and their spouses over a period of six months, using questionnaire measures of symptoms and marital adjustment. Those patients whose marriages were rated as unsatisfactory before treatment improved less during treatment, and were significantly more likely to relapse during follow-up, than those patients with satisfactory marriages. The marriages of nine patients appeared to be adversely influenced by their symptomatic improvement, and two distinct types of marital

Avoidance Moderates Cognitive Behavioral Therapy for Panic Disorder and Agoraphobia.

Cognitive behavioral therapy (CBT), a well-validated treatment for panic disorder, includes interoceptive exposures and possibly in vivo exposures to agoraphobic situations. Testing predictors and moderators of CBT outcomes can improve treatment efficacy. Sixty-six individuals with panic disorder with or without agoraphobia were randomized to panic control therapy (PCT) (n = 32) or PCT and in vivo exposures to agoraphobic situations (PCT + IV) (n = 34). Secondary analyses using multilevel models with repeated measures design revealed that individuals who displayed more interoceptive avoidance …

Alpha absolute power measurement in panic disorder with agoraphobia patients.

BACKGROUND: Panic attacks are thought to be a result from a dysfunctional coordination of cortical and brainstem sensory information leading to heightened amygdala activity with subsequent neuroendocrine, autonomic and behavioral activation. Prefrontal areas may be responsible for inhibitory top-down control processes and alpha synchronization seems to reflect this modulation. The objective of this study was to measure frontal absolute alpha-power with qEEG in 24 subjects with panic disorder and agoraphobia (PDA) compared to 21 healthy controls. METHODS: qEEG data were acquired while particip…

Case study: seizure disorder presenting as panic disorder with agoraphobia.

Seizure disorders can produce anxiety that is almost indistinguishable from psychiatric disorders. There are few reports of adolescents with seizure disorders that produce fear. The first case of an adolescent female who presented with panic disorder and agoraphobia which was a consequence of seizure activity is reported. Careful diagnostic evaluation and correlation with video electroencephalography were important in distinguishing seizure activity from panic disorder.

[The psychoimmunological network og panic disorders, agoraphobia and allergic reactions].

While treating panic and agoraphobia patients with behaviour therapy, a high frequency of allergic reaction of the IgE-mediated type I was observed. Panic disorder, agoraphobia, allergic disorder, and vasomotor reactions are briefly discussed in the framework of psycho-endocrino-immunological research. A pilot study had shown a high correlation between panic disorder with and without agoraphobia and allergic reaction. A controlled study was then planned to test the hypothesized psychoimmunological relationship. 100 allergic patients, 79 panic/agoraphobic patients, and 66 controls underwent psy…
