[Pellagra secondary to chronic alcoholism in Costa Rica].

Introduction: pellagra is a disease caused by a deficiency of niacin (vitamin B3) or tryptophan, its precursor. Case report: we present the case of a 67-year-old Costa Rican male patient, alcoholic and homeless, with chronic ulcers on the lower limbs, photosensitive dermatitis, gait disturbance, mild cognitive impairment, and dysthymia. A diagnosis of pellagra was made based on clinical findings, which was later confirmed by a positive response to vitamin supplementation. Discussion: a review of pellagra, a condition that is increasingly underdiagnosed, was conducted; in this case, it presente

Alcoholism and Immobility Induced Rhabdomyolysis Culminating in Hemodialysis.

Rhabdomyolysis is characterised by muscle breakdown and the release of myoglobin. It is a potentially serious condition that can lead to acute kidney injury (AKI). Factors, such as ischemia, trauma, muscle compression and drug toxicity, can trigger muscle breakdown. Treatment involves aggressive fluid resuscitation to maintain urine output and prevent renal injury. Severe cases with AKI may require temporary renal replacement therapy, such as haemodialysis. It has also been proposed that dialysis can speed up recovery by removing myoglobin that is secreted into the circulation by injured muscl

Brain-derived neurotrophic factor among patients with alcoholism.

BACKGROUND: Brain-derived neurotrophic factor (BDNF) is involved in neurogenesis and in the protection against oxidative damage and neuronal apoptosis. After exercise, there is an increased expression of this myokine, especially in skeletal muscle and brain. Low BDNF levels have been described in neurodegenerative diseases. Alcoholics show both muscle atrophy and brain atrophy. Thus, this study was performed in order to analyze serum BDNF levels among alcoholics and their associations with brain atrophy and muscle strength. METHODS: Serum BDNF values were determined to 82 male alcoholics and

Opioid Receptor Antagonists in the Treatment of Alcoholism.

OBJECTIVES: On the basis of the recent advances in drug therapy of alcoholism, we conducted a review on opioid receptor antagonist drugs with approved indication for the treatment of alcoholism, such as naltrexone and nalmefene. METHODS: We reviewed over 100 publications on peptides and opioid receptors, as well as studies conducted in experimental animals and in humans on the effect of opioid receptor antagonists on alcohol consumption in the treatment of alcoholism. We also reviewed the pharmacological characteristics of naltrexone and nalmefene, and the usefulness of these drugs in clinica

Genetic risk prediction and neurobiological understanding of alcoholism.

We have used a translational Convergent Functional Genomics (CFG) approach to discover genes involved in alcoholism, by gene-level integration of genome-wide association study (GWAS) data from a German alcohol dependence cohort with other genetic and gene expression data, from human and animal model studies, similar to our previous work in bipolar disorder and schizophrenia. A panel of all the nominally significant P-value SNPs in the top candidate genes discovered by CFG  (n=135 genes, 713 SNPs) was used to generate a genetic  risk prediction score (GRPS), which showed a trend towards signifi

Executive functioning in chronic alcoholism and Korsakoff syndrome.

Korsakoff syndrome (KS) is characterized by dense anterograde and retrograde amnesia. There is often a temporal gradient to the retrograde amnesia, with earlier memories more readily recalled than recent memories. Executive functioning has also been found to be impaired in KS. However, research comparing executive functioning between chronic alcoholics (AL) and patients with KS has been relatively sparse to date. In a group comparison design, executive functioning in 15 KS patients and 16 chronic alcoholic patients was assessed using the Behavioural Assessment of the Dysexecutive Syndrome test

An overview of the genetic susceptibility to alcoholism.

AIMS: Alcoholism is a multifactorial, genetically influenced disorder. It is a major health and social issue, a highly frequent disease and a cause of premature death. It is also the most expensive addictive disorder due to morbidity, mortality, societal and legal problems. Besides their involvement in alcohol-related fatalities, forensic scientists are also required to assess driving and working ability as well as permanent invalidity due to alcohol-related conditions. Greater knowledge of the genetic basis of alcoholism could improve prevention by identifying specific risk factors and mechan

Disulfiram in severe alcoholism--an open controlled study.

BACKGROUND: Disulfiram is used to a great extent in Denmark to treat alcoholism but the evidence is limited. AIM: To study the effect of supervised disulfiram treatment in alcohol dependence. Subjects were recruited from a psychiatric emergency ward following alcohol withdrawal treatment. METHODS: A total of n=39 patients were openly randomized to either disulfiram 800 mg twice a week for 26 weeks (n=19) or no disulfiram (n=20). All patients were also treated with cognitive behavioural therapy (CBT) in groups. RESULTS: The rate of abstinence was 20% and 26% in the control and disulfiram gro

[Potentialities of neuropsychological methods for investigation of alcoholism].

Results from a sample of 411 alcohol dependent patients and healthy controls showed diagnostic potentialities of neuropsychological methods for investigation aimed at discovering of disturbances of higher psychic functions indicative of organic brain lesions. The proposed standardized neuropsychological technique is a sufficiently sensitive instrument not only for revealing minimal brain deficiency linked to chronic alcoholic intoxication but also for the evaluation of its intensity, diffuseness, localization of organic brain lesion. The complex analysis of above-listed indicators allows to de

Brain metabolism of ethanol and alcoholism: an update.

It has long been suggested that some of the neuropharmacological, neurochemical and behavioural effects of ethanol are mediated by its first metabolite, acetaldehyde. In spite of the well documented psychoactivity of acetaldehyde, the precise role of this compound in alcohol abuse remains a matter of intense debate among scientists devoted to the study of alcoholism. Very frequently, the main drawback has been related to the presence of adequate levels of acetaldehyde or its derivatives inside the brain after ethanol ingestion. Since penetration into the central nervous system from blood of pe

Relationship between pathological gambling, alcoholism and drug addiction.

The aim of this survey was to evaluate the distribution of pathological gamblers treated in an alcohol or drug addiction treatment program run by the Italian National Health Service providing assistance to alcohol and drug abusers in Venice (North east Italy) from September 1 to December 31, 2001. Each drug- or alcohol-dependent patient retained for treatment for at least one month was administrated an anonymous precoded questionnaire to collect personal and socio-demographic features. The South Oaks Gambling Screen (SOGS) was used to measure pathological gambling and the Symptom Checklist-90-

[Alcoholism--how it affects health and working capacity].

Alcoholism is a growing medical and public health issue both in adult and in younger populations. It is a multi-aetiological phenomenon influenced by genetic, psychological, cultural and other factors. Alcoholic beverages have traditionally been prepared from various ingredients, such as grapes, malt, and rice. Drinking prevalence has varied and is more pronounced in women and the youth. Alcoholism is shown to be of neurophysiologic origin and may lead to the impairment of all human body systems. The most frequent cause of death in alcoholics are the diseases of the cardiovascular system. Alco

Disulfiram treatment for alcoholism in severe mental illness.

Controlled research has shown that supervised disulfiram is an effective treatment for alcoholism. Despite this, little is known about the effects of disulfiram in persons with alcoholism and severe mental illness. We conducted comprehensive chart reviews on 33 patients with alcoholism and severe mental illness (70% schizophrenia or schizoaffective disorder) who had been prescribed disulfiram. Twenty-one percent reported side effects from disulfiram, whereas significant psychiatric complications were not reported. Although 76% of patients reported drinking while on disulfiram, only 28% experie

Autism families with a high incidence of alcoholism.

To determine the significance of neuropsychiatric disorders in autism families, we analyzed 167 pedigrees ascertained through an autistic child; 39% had alcoholism in patterns consistent with transmission of a genetic trait. Children from high alcoholism families were more likely to have the onset of their autistic behavior occur with a loss of language (52.5% vs. 35.8%, p = 0.04). This occurred primarily in families where the mother was alcoholic (80% vs. 40%, p = 0.05), suggesting an association between maternal alcoholism and regressive onset autism. Children from high alcoholism families w
