Alveolar ridge preservation immediately after tooth extraction.

Ridge preservation procedures immediately after tooth extraction, are commonly used with a view to minimising remodelling and shrinkage of the alveolar ridge, associated with socket healing. These procedures may sometimes be effective, but they cannot completely prevent reduction in dimension of the ridge. Certain biomater als used may actually hamper normal deposition of bone within the healing socket, reducing bone trabeculae that can integrate with the implant surface. However, in extraction sockets in alveolar ridges of low bone density, particles of implanted bone substitute incorporated

[Local bone augmentations for the use of implants].

Placement of endosseous implants is often restricted because of lack of supporting bone, limiting placement of implants of adequate length in a prosthodontically optimal position. Several surgical procedures have been described to create sufficient volume of bone for placement of implants. In this paper, various techniques are described for harvesting intraoral bone grafts for augmentation of a localized defect of the alveolar ridge.

Postextraction ridge maintenance using the endosseous ridge maintenance implant (ERMI).

Classically, alveolar bone height in the maxilla and mandible is naturally lost by resorption soon after exodontia; or subsequently by pressure that is exerted on the edentulous ridge by the removable denture. As the alveolar bone is resorbed, the only alternative given to the dental practitioner in the past was to constantly reline the removable prosthesis. With the ERMI, the postextraction edentulous ridge can be sustained; thus reducing the physiological and retentive complications encountered by the patient over time. The intent of this article is to review the physiology of the ERMI in bo

Piezosurgery applied to implant dentistry: clinical and biological aspects.

Piezosurgery is a new and modern technique of bone surgery in implantology. Selective cutting is possible for different ultrasonic frequencies acting only in hard tissues (mineralized), saving vital anatomical structures. With the piezoelectric osteotomy technique, receptor site preparation for implants, autogenous bone graft acquistition (particles and blocks), osteotomy for alveolar bone crest expansion, maxillary sinus lifting, and dental implant removal can be performed accurately and safely, providing excellent clinical and biological results, especially for osteocyte viability. The aim o

Localized vertical maxillary ridge preservation using bone cores and a rotated palatal flap.

The aim of vertical augmentation of the alveolar ridge is to restore resorbed alveolar ridges. This technique is critical to the placement of dental implants in a favorable position and the enhancement of restoration esthetics. The present report describes a technique for surgical preservation of the anterior maxillary process using maxillary bone from the surgical site and raising a soft tissue rotated palatal flap. Maxillary lateral incisor extraction and periapical surgery of the central incisors were first carried out. Two bone cores were harvested from the neighboring buccal vestibular re

Treatment of fully edentulous patients with ITI implants.

The use of ITI implants for rehabilitation of the edentulous patient has shown predictable results not only in the standard sites but also in cases of advanced alveolar ridge atrophy. Preoperative analysis, including clinical and radiological evaluation, implant selection, and determination of implant number and distribution in the edentulous maxilla and mandible, is described in this article. The surgical procedures used in standard sites are presented, the benefits of a nonsubmerged surgical approach in these cases are emphasized, and a protocol for postoperative management is recommended. Maxillary lateral incisor agenesis is associated with maxillary form: a geometric morphometric analysis.

BACKGROUND AND OBJECTIVE: Agenesis of the maxillary lateral incisor occurs in up to 4% of all individuals and requires multidisciplinary treatment. Its developmental origins, however, are not fully understood. Earlier studies documented genetic factors contributing to agenesis but also an association with craniofacial morphology. In this study, we assessed the association between maxillary morphology and lateral incisor agenesis by a geometric morphometric approach to disentangle the roles of developmental plasticity and genetic factors. MATERIALS AND METHODS: We quantified the maxillary alve

Reconstruction of Atrophic Alveolar Process With Xenograft, Fibrin-Rich Plasma, Titanium Mesh, Implant Placement, and Immediate Provisionalization.

Placement of dental implants is often compromised due to alveolar ridge resorption caused by postextraction defects, periodontal disease, traumatic tooth avulsion, or long-term edentulism. During the last 2 decades, various techniques have been proposed for reconstructing atrophic alveolar processes. Different therapeutic modalities have been implemented to achieve bone gain. These techniques require an orderly sequence of maneuvers, which involves handling the soft and hard tissues to minimize the risk of complications. A clinical case of reconstruction of an atrophic alveolar process with xe

[Application of autogenous block bone graft in the reconstruction of severe alveolar crest defect].

Patients with severe alveolar ridge defects cannot be directly implanted and repaired, which seriously affects their quality of life. Onlay bone grafting is the main solution for severe alveolar ridge bone defect reconstruction, among which autogenous block bone grafting is the most widely used and is also the focus of clinical research on bone tissue reconstruction. This article expounds the characteristics and basic principles of autogenous bone block grafting, and comprehensively analyzes the selection of autogenous bone donor site, the principles of surgical operation, and the progress of

Evaluation of poly lactic-co-glycolic acid-coated β-tricalcium phosphate for alveolar ridge preservation: A multicenter randomized controlled trial.

BACKGROUND: Alveolar ridge preservation via socket grafting (ARP-SG) is indicated to attenuate physiologic alveolar bone resorption as a consequence of tooth extraction. However, a specific bone grafting material that is patently superior has not been identified yet. The aim of this randomized controlled trial was to evaluate the efficacy of a moldable alloplastic graft, Poly Lactic-Co-Glycolic Acid-Coated β-Tricalcium Phosphate (PLGA-β-TCP), for ARP purposes [Group A] compared to freeze-dried bone allograft (FDBA) particles covered with a rapidly absorbable collagen dressing (RACD) (Group B)

Maxillary ridge augmentation with custom-made CAD/CAM scaffolds. A 1-year prospective study on 10 patients.

Several procedures have been proposed to achieve maxillary ridge augmentation. These require bone replacement materials to be manually cut, shaped, and formed at the time of implantation, resulting in an expensive and time-consuming process. In the present study, we describe a technique for the design and fabrication of custom-made scaffolds for maxillary ridge augmentation, using three-dimensional computerized tomography (3D CT) and computer-aided design/computer-aided manufacturing (CAD/CAM). CT images of the atrophic maxillary ridge of 10 patients were acquired and modified into 3D reconstr
