The case of a 42-year-old latex-sensitive female who experienced urticaria, pruritus, soft tissue swelling, and significant hypotension during an outpatient barium enema is described. These signs and symptoms of anaphylaxis occurred immediately after enema tip insertion and before infusion of contrast material. We report the case of a patient with anaphylaxis after ingestion of millet seeds, a previously unreported allergen. Skin testing with freshly prepared millet seed extract produced positive intradermal responses at a concentration of 100 pg/ml. The reaction was mediated by IgE antibodies to millet, as demonstrated by elevated specific IgE antibody levels by radioallergosorbent test (RAST) and by significant in vitro histamine release from leukocytes incubated with millet seed extract.

In the current issue of Cell Host & Microbe, Sánchez-Martínez et al. demonstrate that oral and small-intestine small intestinal bacteria can modulate IgE-mediated anaphylaxis through metabolic degradation of immunodominant peanut allergens. PURPOSE OF REVIEW: Airborne anaphylaxis is a rare disorder defined by the occurrence of anaphylactic reactions to inhaled allergens, which may arise not only in occupational exposure but also in common settings.
