Subcapsular hemorrhage of the liver in a neonate with extremely low birthweight.

Subcapsular hemorrhage of the liver (SHL) is usually an autopsy finding rather than a clinical diagnosis. Recent literature has noted an increased incidence of SHL in premature infants. A 26-week, 899 g female was born via emergency cesarean section due to excessive vaginal bleeding. At birth and during the first few days of life, the infant presented with anemia which was masked by concomitant maternal antepartum hemorrhage and neonatal intraventricular hemorrhage. SHL was suspected when the infant developed marked anemia, abdominal distension and hemoperitoneum on day 11 of life. Despite sup

Severe fetomaternal hemorrhage. A report of four cases.

Four cases of severe fetomaternal hemorrhage (FMH) presented with severe anemia and signs of circulatory failure. One of the patients developed the syndrome of persistent fetal circulation. The diagnosis of FMH was confirmed in all using Kleihauer-Betke tests, which demonstrated abundant fetal erythrocytes in the maternal circulation. Three infants survived after prompt volume replacement and correction of anemia. The only fatality was related to an underlying chromosomal disorder. The majority of reported neonatal deaths due to FMH have occurred when shock was the presenting manifestation.

Case Report: Severe Plasmodium vivax Malaria Mimicking Sepsis in a Neonate.

Severe congenital malaria associated with Plasmodium vivax is uncommon. In Indonesia, most congenital malaria cases are due to Plasmodium falciparum infections. Most cases of congenital or neonatal malaria in endemic areas are diagnosed from peripheral smear as part of routine sepsis workup. Differentiating congenital and acquired neonatal malaria is very difficult. The case presented in this study describes severe P. vivax malaria with cholestatic jaundice and sepsis-like signs and symptoms in neonates. The mother was asymptomatic and the neonate was successfully treated with intravenous arte

Association Between Attempted External Cephalic Version and Perinatal Morbidity and Mortality.

OBJECTIVE: To examine whether, with fetal malpresentation at term, perinatal morbidity and mortality differ between women who undergo an external cephalic version (ECV) attempt and those who do not and are expectantly managed. METHODS: We conducted a retrospective cohort study of women with nonanomalous singleton gestations in nonvertex presentation delivering at a tertiary care institution from 2006 to 2016. Women undergoing an ECV attempt at 37 weeks of gestation or greater were compared with those with nonvertex fetuses who did not undergo an ECV attempt and delivered at 37 weeks of gestat

Neonatal outcomes among pregnancies with red cell alloimmunization requiring doppler monitoring without intrauterine transfusion: A retrospective cohort study.

INTRODUCTION: Red cell alloimmunization in pregnancy occurs when a pregnant person develops antibodies against red blood cell antigens that are foreign to her. These antibodies can cross the placenta and cause hemolytic disease in the fetus and newborn (HDFN), which can lead to complications ranging from anemia and hyperbilirubinemia to stillbirth. Historically managed with invasive monitoring via amniocentesis, the approach shifted toward non-invasive Doppler ultrasound of the middle cerebral artery-using a middle cerebral artery peak systolic velocity (MCA PSV) threshold of 1.5 multiples of

Risk factors of infant anemia in the perinatal period.

BACKGROUND: Infants are at particular risk of iron-deficiency anemia. We investigated changes in the blood count of the mother and infant as well as the relationship between them and the relationship between infant nutrition method and infant anemia. METHODS: This retrospective cohort study included healthy neonates born between August 2011 and July 2014 at St Luke's International Hospital, Tokyo, Japan. Data from maternal blood samples obtained during late pregnancy and those of infants obtained at birth and at the age of 3, 6, and 9 months were analyzed. Using multivariate logistic regressi

Perinatal asphyxia in monochorionic versus dichorionic twins: incidence, risk factors and outcome.

OBJECTIVE: To estimate the incidence, risk factors, severity and outcome after perinatal asphyxia in monochorionic (MC) versus dichorionic (DC) twins. METHODS: We included all consecutive near-term MC and DC twins with perinatal asphyxia admitted to our neonatal ward between 2004 and 2013 and compared the perinatal characteristics and neonatal outcome between both groups. RESULTS: The incidence of perinatal asphyxia in MC and DC twin infants was 4.0 (11/272) and 4.0% (8/200; p = 1.00). In contrast to DC twins, asphyxia in MC twins was strongly associated with acute exsanguination and anemia

Red cell adenylate kinase deficiency: molecular study of 3 new mutations (118G>A, 190G>A, and GAC deletion) associated with hereditary nonspherocytic hemolytic anemia.

We report here 2 patients with chronic nonspherocytic hemolytic anemia (CNSHA) and severe red blood cell (RBC) adenylate kinase (AK) deficiency. One of these patients, a boy of Spanish origin, exhibited a neonatal icterus and splenomegaly and required blood transfusions until the age of 2 years. The other patient was a white, American infant born to parents who were first cousins; he also presented with neonatal icterus and anemia. In neither case was psychomotor impairment observed. The first patient was found to be a compound heterozygote for 2 different missense mutations, 118G>A(Gly40Arg)

Early treatment with IV iron is associated with improved maternal hemorrhage-related outcomes.

BACKGROUND: Anemia in pregnancy is associated with maternal hemorrhage-related morbidity and mortality, neonatal neurodevelopmental effects, and postnatal neonatal anemia. This study evaluates how the timing of IV iron therapy prior to delivery impacts maternal and fetal outcomes and to better understand how neighborhood context impacts the timing of IV iron administration. STUDY DESIGN: This retrospective cohort study included pregnant patients who received antenatal IV iron therapy for anemia from January 2017 through November 2023. Timing of IV iron administration in relation to the delive

Maternal and neonatal outcomes associated with haemoglobin concentration in pregnancy: a Burden of Proof study.

This study evaluated dose-response relationships between hemoglobin concentration in pregnancy and maternal and neonatal outcomes based on comprehensive systematic reviews using the Burden of Proof meta-analytic framework. Risk estimates accounting for uncertainty and quantitative star ratings were generated to assess evidence strength. Our analysis revealed that even small deviations above or below hemoglobin levels with the lowest predicted risk - particularly on the low end (anemia)-were associated with non-linear increased risks of all-cause maternal and neonatal mortality, postpartum hemo

Red blood cell transfusion in newborn infants.

Red blood cell transfusion is an important and frequent component of neonatal intensive care. The present position statement addresses the methods and indications for red blood cell transfusion of the newborn, based on a review of the current literature. The most frequent indications for blood transfusion in the newborn are the acute treatment of perinatal hemorrhagic shock and the recurrent correction of anemia of prematurity. Perinatal hemorrhagic shock requires immediate treatment with large quantities of red blood cells; the effects of massive transfusion on other blood components must be

Hereditary spherocytosis: Consequences of delayed diagnosis.

OBJECTIVE: To determine whether patients with undiagnosed hereditary spherocytosis hospitalized for transfusions might have avoided hospitalization via earlier diagnosis. STUDY DESIGN: Charts of all (N = 30) patients with hereditary spherocytosis seen in pediatric hematology at West Virginia University-Charleston were reviewed. Family and transfusion history and presence of neonatal jaundice were recorded. Complete blood count and reticulocyte values during infancy were available for 20 of 30 patients, while baseline steady-state values were available for all 30. RESULTS: Transfusions were g

Fetal Brain Injury in Rhesus Isoimmunization Fetus.

This report presents a rare fetal and neonatal complication brain injury (encephalomalacia and ventriculomegaly) as a consequence of severe fetal anemia resulting from Rhesus (Rh) isoimmunization. A 28-year-old gravida 4 para 3 woman was referred at 21+4 weeks of gestation to the fetal medicine clinic as a case of Rh isoimmunization. Fetal ultrasound showed a normal anatomy scan with normal brain structure, but with severe fetal anemia. The patient was treated with multiple intrauterine transfusions, but still developed complications post-transfusions. This case shows that severe cerebral deve

Iron Deficiency and Other Types of Anemia in Infants and Children.

Anemia, defined as a hemoglobin level two standard deviations below the mean for age, is prevalent in infants and children worldwide. The evaluation of a child with anemia should begin with a thorough history and risk assessment. Characterizing the anemia as microcytic, normocytic, or macrocytic based on the mean corpuscular volume will aid in the workup and management. Microcytic anemia due to iron deficiency is the most common type of anemia in children. The American Academy of Pediatrics and the World Health Organization recommend routine screening for anemia at 12 months of age; the U.S. P

Congenital Dyserythropoietic Anemia Type I in Association with CDAN1 c.2605 G>A Homozygous Mutation.

Congenital dyserythropoietic anemia type 1 (CDA-1) is a rare autosomal recessive disorder. We report the case of a two-year-old female who presented at three days of age with neonatal jaundice and anemia. She subsequently developed persistent, moderate normochromic anemia with relative reticulocytopenia. Bone marrow aspirate demonstrated erythroid hyperplasia and mild dyserythropoiesis. Whole-exome sequencing (WES) identified a homozygous c.2605G>A (p.V869M) mutation in the CDAN1 gene, consistent with the diagnosis of CDA-1. Although this variant has been previously documented in a compound he
