Remediation of developmental dyslexia: tackling a basic memory deficit.

Remediation studies of developmental dyslexia are extremely rare. We present a single case study of an 8-year-old German developmental dyslexic boy K.H. who is hypothesized to suffer from a severe memory problem, impeding the development of normal reading functions. The memory problem especially affects the storage and access to letter-sound associations. As a consequence, the boy was initially unable to accurately and quickly name most of the 26 letters of the alphabet. A multiple-baseline across-material design with a high-frequency errorless learning procedure was set up to improve letter-n

Gestural communication in Alzheimer's disease.

Spontaneous communicative hand-arm gestures were evaluated in elderly patients with probable Alzheimer's disease (AD) and healthy controls (NC). Based on the notion that speech and gestures arise from common semantic-conceptual representations, qualitatively similar linguistic and gestural communicative impairments were expected in association with semantic memory impairment in AD. Despite equal quantity and rate of gesturing, AD and NC groups produced qualitatively different types of gestures. Patients with AD produced proportionately more referentially ambiguous gestures, fewer gestures refe

Objects and properties: a study of the breakdown of semantic memory.

This paper reports a study of the breakdown of semantic memory in the case of a subject with semantic dementia. The first experiment shows that the subject failed to comprehend words of low familiarity and word frequency, even though the spoken word forms were recognised as familiar. Experiments 2 and 3 showed (a) that the recall of word meanings in definition tasks did not vary with the generality of the word meaning (e.g. category, basic level, or subordinate property) but varied instead with the concept familiarity and frequency of the name; (b) that the ability to verify properties of basi

The relationship of phonological awareness, rapid naming, and verbal memory to severe reading and spelling disability.

The present study examined the relationship of phonological awareness, naming speed, and verbal memory to the scores obtained from five tests assessing word attack, word identification, reading comprehension, and spelling skills in 54 children with severe reading disabilities (48 boys and 6 girls; M age = 9 years, 7 months). Multiple regression analyses indicated that the best predictor of achievement across the five academic tests was the Verbal Comprehension factor from the Wechsler Intelligence Scale for Children-Revised. Age, socioeconomic status (SES), and externalizing behavior problems

Neuropsychological impairment of face recognition units.

Despite the absence of "conscious", overt identification, some patients with face recognition impairments continue covertly to process information regarding face familiarity. The fact that by no means all patients show these covert effects has led to the suggestion that indirect recognition tasks may help in identifying different types of face recognition impairment. The present report describes a number of experiments with the patient NR, who, after a closed head injury, has been severely impaired at recognizing familiar faces. Investigations mostly failed to show overt or covert face recogni

Developmental dyslexia and word retrieval deficits.

Developmental dyslexics, selected on the basis of very slow naming rates on the Rapid Automatic Naming Tasks (RAN), were compared to normal readers on oral language, picture categorization, and reading tasks. Findings indicated that the dyslexics' word retrieval deficits were one symptom of a more generalized, however subtle, oral language deficit which involved both receptive and expressive oral language functioning. The dyslexics' word retrieval problem also seemed chiefly related to language processing and not to deficits in semantic memory as there were no significant differences between d

[A clinicopathological case of progressive aphasia].

We report the case of a 59-year old right-handed woman presenting with progressive aphasia without any other neurological deficit and characterized by anomia, agramatism and auditory comprehension difficulties. CT scan showed no abnormalities. Four years later, aphasia was complete but neurological examination was normal. CT scan disclosed a mild cortico-subcortical cerebral atrophy with slight widening of both sylvian fissures. The results obtained with the Wisconsin Card Sorting Test and the Raven's Matrices showed only minor deficits. The IQ (WAIS) was 90. During the following 6 years the p…

Referential communication in Alzheimer's type dementia.

This paper investigates factors that underlie reduced informative content and lack of reference in the discourse of patients with Alzheimer's Type Dementia (DAT). Patients with DAT, fluent aphasics and normal controls were given a referential communication task structured to assess lexical encoding of information, pragmatic/conceptual elaboration of information and effectiveness in establishing reference. The subjects also received standardised aphasia tests (CADL, Holland, 1980; and Cookie Theft Picture Description, Nicholas and Brookshire, 1993). Comparable reduction of lexical encoding of i

The Gerstmann syndrome in Alzheimer's disease.

BACKGROUND: It remains unclear from lesion studies whether the four signs of the Gerstmann syndrome (finger agnosia, acalculia, agraphia, and right-left confusion) cluster because the neuronal nets that mediate these activities have anatomical proximity, or because these four functions share a common network. If there is a common network, with degeneration, as may occur in Alzheimer's disease, each of the signs associated with Gerstmann's syndrome should correlate with the other three signs more closely than they correlate with other cognitive deficits. METHODS: Thirty eight patients with pro

When a rose is a rose in speech but a tulip in writing.

We report the pattern of performance on language tasks by a neurologically impaired patient, RCM, who makes semantic errors in writing to dictation and in written naming, but makes very few errors at all (and no semantic errors) in spoken naming, oral reading, or spontaneous speech. RCM also shows a significant effect of concreteness on spelling accuracy and other features of so-called "deep dysgraphia." However, it is shown that, unlike previously reported patients described as deep dysgraphic, RCM has intact semantic processing but impairment in accessing lexical-orthographic representations

Impaired activation of the phonological lexicon: effects upon oral reading.

The role of the phonological lexicon in oral reading is examined in a patient with a small focal left hemisphere lesion. Impaired access to the patient's phonological lexicon is suggested by a number of findings, including the production of phonemic errors across a variety of tasks; increasing difficulty in word production with increasing word length; and difficulty on tests of homophone and rhyme judgments. Two competing models of reading are tested: the nonlexical ("rules") and the lexical ("no-rules") models. The rules model predicts that a disturbance in the phonological lexicon will resul

[Serial reactions in aphasic patients].

A symptom is reported that has hitherto not been described in neuropsychological literature: Not infrequently, severe aphasics mix automatized verbal series or parts of them not only into their spontaneous speech but also into other modalities of the expressive language. Sometimes this form of automatism replaces a demanded answer. In the speech data of 134 aphasics this symptom was observed in half of the cases. Its incidence was analysed in relation to aetiology, type of aphasia and the modalities in which it was observed. 6 cases are described in detail. The serial reaction is a neuropsycho

Selective crossed aphasia in a trilingual aphasic patient followed by reciprocal antagonism.

After surgical removal of a parasitic cyst in the right prerolandic area, an educated 25-year-old male exhibited obvious deficits in one of his three languages (Gujarati), with no measurable deficits in the other two (French and Malagasy). The patient spoke all three languages fluently before the operation. Gujarati and Malagasy had been acquired in infancy. Gujarati was the language of his parents and relatives. Malagasy the language of the local population. All the patient's schooling had been in French, the only language in which he was literate. He used French daily at work. Since the acqu

Construction of a Prediction Model for Naming Recovery in Subacute Poststroke Aphasia Based on Multivariate Analysis: Evaluation of Accuracy and Reliability.

PURPOSE: Anomia, a common dysfunction in poststroke aphasia (PSA), impacts daily communication, and its prognosis remains challenging. This study aimed to develop a model to predict naming rehabilitation in patients with subacute PSA. METHOD: Data of PSA were collected retrospectively. Logistic regression (LR) analyses by a nested fivefold cross-validation were performed to identify predictors and construct a predictive model. The efficacy of the model was evaluated using receiver operating characteristic (ROC) curves, calibration curves, and decision curve analysis (DCA). Additionally, the n…

Covert reading of letters in a case of global alexia.

This study describes the case of a global alexic patient with a severe reading deficit affecting words, letters and Arabic numbers, following a left posterior lesion. The patient (VA) could not match spoken letters to their graphic form. A preserved ability to recognize shape and canonical orientation of letters indicates intact access to the representation of letters and numbers as visual objects. A relatively preserved ability to match lowercase to uppercase letters suggests partially spared access to abstract letter identities independently of their visual forms. The patient was also unable

Age at onset and language disturbances in Alzheimer's disease.

This study examined the effect of age at symptom onset of Alzheimer's disease (AD) on the pattern of language disturbance. We assessed 150 consecutive patients with a clinical diagnosis of mild-to-moderate AD using the Western Aphasia Battery and a 100-item picture-naming test. A multivariate linear regression analysis examined the effect of age at onset after controlling for gender, education, severity of dementia and duration of the disease. Patients with early onset performed significantly worse than did patients with late onset on the word comprehension and sequential commands subtests. On

[Verbal ability of Alzheimer's disease patients and very elderly control patients using the Aachen aphasia test].

We assessed language function, using the Aachen aphasia test (AAT), in 14 subjects with Alzheimer's disease and 6 control patients. Aphasia did not occur in control patients, but in 11 of 14 Alzheimer's disease patients. Impairment of object naming was the most prevalent abnormality. Performance in AAT subtests repetition, written language, object naming, comprehension and token test correlated strongly with Mini-Mental State scores.
