Investigation of prevalence of MRSA in referrals to neurosurgery: implications for antibiotic prophylaxis.

In order to establish the appropriateness of our current prophylactic antibiotic regimen we analysed the prevalence of MRSA in emergency referrals to our unit. MRSA screening records for all emergency admissions for a 3-month period were analysed. One-hundred-and-seventy-five patients were admitted as transfers from another hospital. Evidence of screening was found in 61% (107 patients). Of the screened patients, 15% (16) were MRSA positive. Source of referral or length of inpatient stay after referral to the time of transfer were not predictive for MRSA status. Gentamicin is active against mo

[Treatment of bites to the hand and wrist--is the primary antibiotic prophylaxis necessary?].

Animal bites make up a large proportion of the injuries treated in an emergency department. Due to the type of injury, the variety of wounds and the contamination with aerobic and anaerobic organisms, they deserve special attention. In this study, we reviewed 98 patients (55 male/43 female) with bite wounds to the hand and wrist treated between 1995 and 2000. They were either treated conservatively (n = 65) or surgically (n = 33) depending on the clinical findings. In 18 of 33 cases, the reason for surgical treatment was an infection. A primary antibiotic prophylaxis, usually with cephalospori

Review of antibiotic prophylaxis for bacterial endocarditis in relation to radiological procedures.

Although the risk of endocarditis from radiological instrumentation is low, it is recommended that antibiotic cover is used for certain invasive procedures in patients at high risk. The barium enema is, in this context, included in procedures requiring cover. Prosthetic heart valves are particularly liable to infection.

[Prompt antibiotic prophylaxis in resection of the prostate and removal of the bladder catheter].

Several regimens of antibiotic prophylaxis have been used in transurethral prostatic resection. In this study 50 patients received 1 g of cefotaxime during resection and thereafter were divided at random into two groups: one group received another 1 g dose of cefotaxime when the bladder catheter was removed, while the other group was left untreated at that stage. The administration of cefotaxime during both prostatic resection and catheter removal resulted in a highly significant reduction of infection risk, duration of hospitalisation and cost of antibacterial treatment. The most important ri

The efficacy of antibiotic prophylaxis in the prevention of post-cesarean section endometritis.

Endometritis is an infectious complication in 9% to 65% of patients delivered by cesarean section. The risk of developing endometritis is greater in the high-risk emergent patient as compared to routine repeat abdominal deliveries. This study describes the incidence of endometritis following cesarean section delivery in different patient groups at the University of Virginia Hospital during a 1-year period and reviews the literature on the efficacy and risks of prophylactic antibiotics in this setting. Of patients not receiving antibiotic prophylaxis, 11 (less than 1%) of 1,461 normal spontaneo

Antibiotic prophylaxis in pediatric surgery: when more is not better-or safer.

PURPOSE: Antibiotic resistance is a major public health concern. While surgical antibiotic prophylaxis (SAP) reduces surgical site infections (SSIs), inappropriate use may cause harm. This study evaluates the impact of SAP appropriateness and compares beta-lactam versus non-beta-lactam antibiotic use in pediatric surgery. METHODS: A retrospective cohort analysis was conducted using ACS NSQIP Pediatric data (2021-2023). Exclusions included preexisting infections, malignancy, immunodeficiencies, and emergent surgeries. SAP appropriateness was classified according to NSQIP guidelines. Logistic r

Infections in noninstrumented spine surgery: is the discontinuation of antibiotic prophylaxis to blame?

BACKGROUND CONTEXT: Noninstrumented spine surgery (NISS) remains a common procedure. Surgical site infections (SSIs) are common complications. Since 2024, the official French guidelines recommend discontinuing preoperative antibiotic prophylaxis. Prior to 2024, preoperative antibiotics were routinely administered. PURPOSE: To compare the rate of SSIs after NISS in patients operated with vs without antibiotic prophylaxis. STUDY DESIGN: Single-center retrospective comparative series. PATIENT SAMPLE: A total of 651 patients underwent elective NISS between January 2022 and June 2025. OUTCOME M

Evaluation of Antibiotic Prophylaxis and Discharge Prescriptions in the General Surgery Department.

Introduction Although there are international guidelines for surgical antibiotic prophylaxis (SP), the use of inappropriate SP is still a common problem. Most studies investigated SP applications in clean and clean-contaminated cases. However, antibiotics in the discharge prescriptions of these cases have not been adequately investigated. In this study, we aimed to examine the antibiotics in SP applications and discharged prescriptions together and to find out the causes of inappropriate use. Materials and methods We retrospectively evaluated the data of patients admitted to our general surger

Continuous antibiotic prophylaxis in the setting of prenatal hydronephrosis and vesicoureteral reflux.

Continuous antibiotic prophylaxis (CAP) has traditionally been offered for children with recurrent urinary tract infections (UTIs) or those at risk, including children diagnosed with prenatal hydronephrosis (HN) and vesicoureteral reflux (VUR). However, indications for antibiotic prophylaxis are controversial, data on who should benefit from this therapy is conflicting and, thus, guidelines are unable to provide conclusive recommendations. In the setting of prenatal HN, although randomized trials are currently underway, most evidence is derived from low- to moderate-quality observational studi

Current trends in antibiotic prophylaxis for laryngectomy in the UK - a national survey.

BACKGROUND: With the increasing use of chemoradiotherapy protocols, total laryngectomy carries increasing risks such as pharyngocutaneous fistula. There is little reference to the use of antibiotic prophylaxis in salvage surgery. This study aimed to determine the current practice in antibiotic prophylaxis for total laryngectomy in the UK. METHOD: A questionnaire was designed using SurveyMonkey software, and distributed to all ENT-UK registered head and neck surgeons. RESULTS: The survey revealed that 19 surgeons (51 per cent) follow a protocol for antibiotic prophylaxis in primary total lary

Antibiotic prophylaxis in the management of complex midface and frontal sinus trauma.

OBJECTIVES/HYPOTHESIS: Although mandible trauma has been studied extensively, there is no standard for use of pre- and postoperative antibiotics in other facial trauma. We sought to determine whether antibiotic strategies have an effect on infection rates. STUDY DESIGN: Retrospective chart review and cohort analysis. METHODS: Patients seen by the otolaryngology service for traumatic facial injuries between January 1, 2003 and January 1, 2009, were included in a retrospective cohort analysis (N = 223). All patients received perioperative antibiotic coverage. Isolated mandible fractures were e

Antibiotic prophylaxis for group A streptococcal burn wound infection is not necessary.

BACKGROUND: Historically, group A beta-hemolytic streptococci (GAS) burn wound infection has been a major source of morbidity and mortality in burn patients and has prompted the prophylactic administration of antibiotics to children with burns. Wound monitoring, surveillance cultures, and early excision of deep wounds may have changed this. Our objective in this project was to determine the efficacy of routine antibiotic prophylaxis in the era of early excision and closure of deep burn wounds. METHODS: Two cohorts of burned children were compared: all children admitted during calendar years 1

Surgical antibiotic prophylaxis utilization for pediatric thyroid operations: A NSQIP-Pediatric analysis.

INTRODUCTION: Thyroid operations are generally clean procedures. However, there are no pediatric-specific guidelines regarding surgical antibiotic prophylaxis (SAP) for thyroid operations. This study aims to understand SAP utilization for thyroid operations in children and its association with surgical site infection (SSI). METHODS: Patients <18 years old who underwent clean thyroid lobectomy and total thyroidectomy between 2021 and 2023 were identified in the National Surgical Quality Improvement Program-Pediatric (NSQIP-P) database. Patients with pre-existing infection and non-clean wound c

Genomic adaptation in group B Streptococcus following intrapartum antibiotic prophylaxis and childbirth.

Through vaginal colonization, group B Streptococcus (GBS) causes severe outcomes including neonatal sepsis and meningitis. Although intrapartum antibiotic prophylaxis (IAP) has reduced neonatal disease rates, GBS can persist in the genitourinary tract even after antibiotic administration. To determine if IAP selects for genomic signatures that enhance GBS survival and persistence, we compared the isolates from individuals before (prenatal) and after (postpartum) IAP/childbirth. Among 34 of the paired strains from participants with persistent colonization, 31 (91.2%) clustered together in a cor

Antibiotic Prophylaxis in Maxillofacial Surgery and Evidence-Based Practice: A Systematic Review.

Antibiotic prophylaxis is defined as the intentional administration of antibiotics with the goal of preventing infection prior to the occurrence of contamination by surgical incision. So, it can either be given before the procedure or after the surgery to prevent infection. The aim of this study was to review the current literature and evaluate whether the use of preoperative, perioperative, or postoperative antibiotic prophylaxis influences outcomes in patients undergoing oral and maxillofacial surgeries. This systematic review was conducted in line with the Preferred Reporting Items for Syst
