[Clinical characteristics of post-stroke anxiety disorders].

Authors studied 198 patients with first-ever cerebral stroke. Affective disorders, including depression, generalized anxiety disorder (GAD) and phobias, were identified in 71.2% of patients. The study demonstrated that the anxiety syndrome developed in the post-stroke period may be regarded as relatively independent. Some post-stroke anxiety disorders had different risk and pathogenetic factors. GAD is associated with young age and phobias are associated with female sex and inherited predisposition. In contrast to post stroke depression, anxiety-phobic disorders develop more frequently in pati…

Clinically relevant drug interactions in anxiety disorders.

OBJECTIVE: Certain drugs used in the treatment of patients with anxiety disorders can interact with other psychotropic drugs and with pharmacological treatments for physical illnesses. There is a need for an updated comparative review of clinically relevant drug interactions in this area. DESIGN: Relevant literature on drug interactions with medications used in the treatment of anxiety disorders was identified through a search in MEDLINE and EMBASE. RESULTS: Drug interactions involving medications used to treat anxiety disorders may be pharmacokinetic, such as enzyme inhibition or induction …

Anxiety disorders diagnosis: some history and controversies.

Treatment of and research on anxiety disorders depends on the adequate conceptualization and measurement of these conditions. We review the history of the nosology of anxiety disorders and note that divisions of "neurosis" have inadvertently taken attention away from what is shared among conditions now classified separately. We note the changes in the definition of agoraphobia over time and the striking differences between DSM-IV and ICD-10 definitions. We mention ongoing controversies in the diagnoses of posttraumatic stress disorder, acute stress disorder, and generalized anxiety disorder. F…

The serotonin-1A receptor in anxiety disorders.

The serotonin system plays an important role in the neural processing of anxiety. The involvement of the main inhibitory serotonergic recept … ovides strong evidence for the key role of the 5-HT1A receptor in the serotonergic dysregulation of anxiety disorders.

Pediatric anxiety disorders: management in primary care.

PURPOSE OF REVIEW: Anxiety disorders are common in children and adolescents, with prevalence rates varying from 6 to 20%. These disorders can result in significant academic, social, and familial impairment. Early identification in pediatric primary care and effective management may help improve outcomes. RECENT FINDINGS: Self-report measures of pediatric anxiety can supplement the clinical interview and assist in screening children and adolescents for separation anxiety disorder, generalized anxiety disorder, and social phobia. Substantial evidence supports the use of cognitive behavioral the…

Anxiety disorders and suicidal behaviour: an update.

PURPOSE OF REVIEW: The primary aim of this review is to present the main findings from the literature published between January 2006 and May 2007 on anxiety and suicidal behaviour. The secondary aim is to present critical comments on methodological issues, highlighting areas for future research. RECENT FINDINGS: Traditionally, anxiety disorders have not been viewed as independent risk factors for suicidal behaviour, and therefore assessment of anxiety disorders has not been particularly emphasized in clinical enquiries and suicide screening tools. This review identifies evidence suggesting th…

Anxiety disorders: a comprehensive review of pharmacotherapies.

This article reviews the evidence from randomized, placebo-controlled trials and meta-analyses of pharmacological treatments of the following anxiety disorders: generalized anxiety disorder, panic disorder, social anxiety disorder, and post-traumatic stress disorder. There is evidence from multiple randomized, placebo-controlled trials to support the use of selective serotonin reuptake inhibitors as first-line pharmacotherapy in these disorders, and a number of the selective serotonin reuptake inhibitors have received US Food and Drug Administration approval for these indications. Serotonin-no…

[Biologic base of anxiety disorders: recent progress].

We reviewed recent knowledge and a biologic base of anxiety disorders. As for brain image study, mainly study on PET, fMRI and NIRS has advanced. The neural circuit hypothesis of Gorman still attracts attention. We will think that characteristics of each anxiety disorder disease will become clear from these studies in future. We think that a genetic role will be associated with personality, character and gene study. We mention the connection between neuropeptides and anxiety in animal experiments. In addition, the past sensory stimulation in the amygdala, regarded as the center of anxiety, and…

Anxiety disorders: under-diagnosed and insufficiently treated.

Anxiety disorders have a high prevalence, particularly specific phobia, social anxiety disorder (SAD), post-traumatic stress disorder (PTSD) and generalized anxiety disorder (GAD). The lifetime prevalence for any anxiety disorder is almost 30%. Also, many anxiety disorders have a high rate of comorbid depression, which is generally secondary to the anxiety. Despite high prevalence, anxiety disorders are under-diagnosed, misdiagnosed and inappropriately treated. There is a hierarchy in the lifetime probability of treatment contact, with panic and GAD being diagnosed and treated earlier - possib…

Managing depressive and anxiety disorders with escitalopram.

This article reviews escitalopram, the S-stereoisomer of the racemic compound citalopram, and a highly selective and potent member of the selective serotonin re-uptake inhibitor class of antidepressants. Escitalopram has a straightforward pharmacokinetic profile, little effect on hepatic metabolism, and is relatively safe in overdose. Similar to other members of the selective serotonin re-uptake inhibitor class, escitalopram (10-20 mg/day) is a well-tolerated and effective treatment of major depressive disorder. Although relatively few head-to-head comparative studies with other antidepressant

Treatment of anxiety disorders with venlafaxine XR.

When venlafaxine was introduced in 1994, it was the first of the newer generation antidepressants to be classified as a serotonin norepinephrine reuptake inhibitor (SNRI). An extended release (XR) formulation of venlafaxine, introduced in 1997, subsequently received regulatory approval for treatment of three anxiety disorders: generalized anxiety disorder, social anxiety disorder and panic disorder. Although less extensively studied, venlafaxine XR also appears to have efficacy for two other anxiety disorders, post-traumatic stress disorder and obsessive-compulsive disorder. In contrast to the…

Disorders of mood and anxiety in adolescents.

Mood disorders and anxiety disorders contribute significantly to morbidity and mortality during adolescence. These disorders often persist or recur in adulthood. Clinical presentations in the primary care setting are myriad and often confusing. Early recognition, differentiation from physical and other psychiatric disorders, and accurate diagnosis lead to more appropriate treatment and improved outcome. Collaboration among primary care providers and mental health professionals is key to reducing the suffering from these disorders for adolescents and their families.

Visually induced postural sway in anxiety disorders.

Postural sensitivity to moving visual environments in patients with anxiety disorders was studied. We hypothesized that patients with anxiety disorders would have greater sway in response to a moving visual environment compared to healthy adults, especially if they have space and motion discomfort (SMD). Twenty-one patients with generalized anxiety without panic (NPA) and 38 patients with panic and agoraphobia (PAG) were compared to 22 healthy controls. SMD was evaluated in all subjects via questionnaire. Subjects stood on a force platform that was either fixed or rotating with the subject (i.…

Anxiety disorders in children with epispadias-exstrophy.

OBJECTIVES: To evaluate the hypothesis that anxiety disorders are common comorbid conditions in children with the epispadias-exstrophy complex. METHODS: Twenty consecutive outpatient exstrophy subjects ranging in age from 5 to 22 years were assessed using a formalized semistructured psychiatric evaluation and were categorized according to the Diagnostic and Statistical Manual of Mental Disorders, version IV, criteria. Disorders were identified if subjects endured moderate to severe impairment in their home, academic, and social environments. RESULTS: All 20 subjects met the criteria for at l…

Predictors of pharmacotherapy response in anxiety disorders.

Although treatment with different compounds such as tricyclic antidepressants, selective serotonin reuptake inhibitors, high-potency benzodiazepines, and monoamine oxidase inhibitors has been proven effective in anxiety disorders, 20% to 40% of patients are nonresponders. Given the limited efficacy, the delayed onset of response (it takes several weeks before a clinical effect can be seen for most of these drugs), and the occurrence of side effects associated with pharmacotherapy, predicting response in anxiety disorders would be immensely valuable. This review surveys the literature over the …

Pregnancy complications associated with childhood anxiety disorders.

To determine whether perinatal complications predict childhood anxiety disorders independently of parental psychopathology, we systematically assessed pregnancy and delivery complications and psychopathology in a sample of children (mean age=6.8 years) at high risk for anxiety disorders whose parents had panic disorder with (n=138) or without (n=26) major depression, and in contrast groups of offspring of parents with major depression alone (n=47), or no mood or anxiety disorders (n=95; total N=306). Psychopathology in the children was assessed by structured diagnostic interviews (K-SADS), and…
