Coarctation of aorta with complete aortic occlusion.

Survival to advanced age is exceptional in patients with unrepaired aortic coarctation. We report the case of an 81-year-old man with aortic coarctation and total occlusion who was otherwise asymptomatic. Coarctation was suspected when a femoral-radial pulse delay was noted during his routine physical examination. A 70-mmHg systolic blood pressure gradient between the upper and lower extremities was detected. Subsequent magnetic resonance angiography, aortography, and coronary angiography revealed severe coarctation of the aorta, well-developed collateral vessels, and severe coronary artery di…

Ruptured pseudoaneurysm of aortic root graft into the pulmonary artery as a cause of severe heart failure.

A 52-year-old man presented with heart failure of 1 month duration. He had undergone aortic valve and root replacement 30 months before admission. A continuous murmur was heard in the second intercostal space at the parasternal border. Aortography showed a pseudoaneurysm surrounding the aorta, whereas color Doppler study revealed flow from the central aorta to the pseudoaneurysm and flow from the pseudoaneurysm to the pulmonary artery trunk through a fistulous communication between them. Thus, fistulous communication with pulmonary artery causing heart failure is a complication of pseudoaneury…

Systolic and diastolic mitral regurgitation in a patient with annulo-aortic ectasia demonstrated by color Doppler flow imaging.

A rare case of annulo-aortic ectasia is reported in a 65-year-old man who had aortic and mitral regurgitation during systole and diastole. He was hospitalized for further examination of the heart due to cardiomegaly and heart murmurs. Aortography revealed severe aortic regurgitation. On color Doppler flow imaging, we could detect red aortic regurgitant signals in the left ventricular cavity during diastole, and mosaic and blue mitral regurgitant signals in the left atrial cavity during systole and diastole associated with a relatively long R-R interval, respectively. The unique observation of …

Renovascular hypertension as a complication of umbilical arterial catheterization.

Renovascular hypertension (RVH) in the neonatal period is frequently associated with thromboembolic complications of umbilical artery catheterization. Seven newborn infants with RVH were studied by angiography and/or radionuclide examination. Aortography and, in one case, selective angiography showed variable degrees of renal artery occlusion or attenuation. Thromboembolic defects were frequently present in other vessels. Radionuclide flow studies, renograms, and computer analysis of data (ADAC) demonstrated defects in renal function, indicative of renal ischemia. There was a high degree of co…

Radiation exposure during uterine artery embolization: effective measures to minimize dose to the patient.

PURPOSE: Evaluation of patient radiation exposure during uterine artery embolization (UAE) and literature review to identify techniques minimizing required dose. METHODS: A total of 224 of all included 286 (78 %) women underwent UAE according to a standard UAE-protocol (bilateral UAE from unilateral approach using a Rösch inferior mesenteric and a microcatheter, no aortography, no ovarian artery catheterization or embolization) and were analyzed for radiation exposure. Treatment was performed on three different generations of angiography systems: (I) new generation flat-panel detector (N = 10…

Bedside placement of an aortic occlusion balloon to control a ruptured aorto-esophageal fistula in a small child.

A child had a percutaneous endoscopic gastrostomy tube removed by transecting it at skin level. The internal portion ("the bolster") was all … edside without fluoroscopic guidance. This temporized the exsanguination. Subsequent intraoperative aortography confirmed an aortoesophageal fistula. A stent-graft was deployed with immediate hemodynamic stabilization. She later underwent esophageal resection and recovered well.

Repair of a ruptured aortic arch aneurysm complicated by postoperative paraplegia: report of a case.

We report herein the rare case of a 79-year-old man who suffered permanent paraplegia after undergoing an otherwise successful total arch replacement for a ruptured aortic arch aneurysm. During cardiopulmonary bypass, perfusion to the distal aorta was maintained from the femoral artery, and postoperative aortography showed intact tributaries from the aorta including the intercostal arteries. Postoperative paraplegia is an extremely rare complication of operations on the aortic arch; however, we speculate that the paraplegia in this patient could be attributed either to a steal phenomenon invol…

Subadventitial rupture of the splanchnic arteries as the result of blunt abdominal trauma presenting with acute gastric dilatation.

Two patients are reported who presented with intestinal ischaemia caused by a subadventitial rupture of the origin of the coeliac trunk and superior and inferior mesenteric arteries after blunt trauma from deceleration injury. In both cases the initial clinical examination revealed a painful abdomen without any 'peritonism'. Abdominal ultrasonographic examination showed no abnormality. Plain abdominal radiography showed gastric dilatation in both patients. In the first, the diagnosis was made by laparatomy but only after 2 days. In the second, diagnosis was made by aortography performed becaus…

[Atresia of the right coronary ostium associated with annulo-aortic ectasia--a case of successful surgical repair].

A case of a 51-year-old male with atresia of the right coronary artery and annulo-aortic ectasia is described. He presented with heart failure and underwent open-heart surgery. Preoperative aortography did not show the ostium of the right coronary artery. At surgery the right coronary ostium was not found on the intimal surface of the aneurismal aortic wall. A fine cord was attached to the adventitial surface of the anterior aortic root. A modified Bentall's procedure was performed to reconstruct the left coronary artery alone. The post-operative course was uneventful. It is speculated that th…

[Histopathological examination of a case of Behçet's disease with aortic regurgitation].

Cardiac involvement in Behçet's disease is quite uncommon. We report a case of Behçet's disease with aortic regurgitation of which histology was examined. A 56-year-old male came to our hospital with a complaint of right cervical pain. Diagnosis of aortic regurgitation was made by echocardiography, cardiac catheterization and aortography. As the cardiac lesion worsened, the operation of the aortic valve replacement was performed. On pathological examination, there was marked fibrous thickening on the aortic valve and the rupture of elastic tissue in the media of aortic wall. There was no histo…

[Diagnosis and treatment of post-traumatic aneurysm of the thoracic aorta (author's transl)].

A post-traumatic false aneurysm of the thoracic aorta is generally the result of a severe closed chest trauma in which deceleration and compression mechanisms operate on the aortic isthmus. Early diagnosis is difficult except where the lesion progresses to rupture. A not inconsiderable number of incomplete and initially closed tears in the wall of the aortic isthmus escape detection and in due course lead to dilatation of the vessel and false aneurysm. The condition is diagnosed, often accidentally, during aortography. The indications for surgical intervention are given by the aneurysm-specifi…

Endovascular aneurysm repair with mesenteric artery bypass for an abdominal aortic aneurysm with occlusion of celiac and superior mesenteric arteries.

A 67-year-old male patient required surgical management of an abdominal aortic aneurysm. A contrast-enhanced computed tomography showed a saccular infrarenal abdominal aortic aneurysm, with occlusion of the origins of the celiac artery, superior mesenteric artery, and inferior mesenteric artery. An aortography revealed large amounts of blood flow from capillaries around the abdominal aorta to the inferior mesenteric artery and retrograde blood flow to the meandering mesenteric artery through the superior rectal artery. Considering the risk of bowel ischemia, we performed endovascular aneurysm …

Low-dose coronary artery computed tomography angiogram of an infant with tetralogy of fallot using a 256-slice multidetector computed tomography scanner.

The reported incidence of anomalous coronary arteries among children is 0.17%, but it is much greater (8%) among children with tetralogy of Fallot (TOF). The diagnosis of coronary artery anomalies can be difficult using invasive aortography and is not without risk. Technical improvements with multidetector computed tomography (MDCT), including a reduced effective radiation dose, make cardiac CT angiography an attractive alternative not only because the risks of an invasive examination are avoided, but also because sedation risks also are eliminated in most cases. This report highlights 256-sli…

[Valvuloplasty for aortic regurgitation with bicuspid valve: report of a case].

A 32-year-old male patient with bicuspid aortic valve underwent valvuloplasty for the treatment of aortic regurgitation. The aortic regurgitation was estimated to be grade III based on the transthoracic echocardiography and aortography findings. Transesophageal echocardiography showed prolapse of the conjoined leaflet. At surgery, the valvuloplasty consisted of triangular resection and re-suture of the prolapsed larger leaflet, and subcommissural annuloplasty. The degree of the aortic regurgitation decreased to very slight degree after the repair. The peak pressure gradient across the repaired…

A case report of surgical treatment of quadricuspid aortic valve associated with regurgitation.

A case of a 65-year-old woman who had a quadricuspid aortic valve associated with aortic regurgitation is reported. The patient had severe aortic regurgitation and four equally divided aortic cusps. The valve abnormality was detected by a transesophageal echo and an aortography. The incomplete aortic valve was excised and replaced by a St. Jude Medical prosthesis. Although this case had no coronary abnormality, a coronary displacement is often reported in quadricuspid aortic valve cases. In order to perform an operation safely, accurate information which is obtained by a non-invasive examinati…

[A successful surgical case of supravalvular aortic stenosis with Williams syndrome: the investigations of the procedures].

We experienced a successful surgical case in which a 3-year-old boy with a characteristic elfin face, heart murmur, and mental retardation, underwent extended patch aortoplasty using equine pericardium for congenital supravalvular aortic stenosis. The aortography performed before operation, and demonstrated diffuse stenosis just above the aortic valve, which was a typical hour-glass type. The preoperative peak systolic pressure gradient between the left ventricle and ascending aorta was 120 mmHg, and was improved postoperatively. In this procedure only one cusp was incised, resulting in no def…

Detection of anomalous origin of left coronary artery from pulmonary artery by real-time Doppler color flow mapping in a 53-year-old asymptomatic female.

The anomalous origin of the left coronary artery from the pulmonary artery in a 53-year-old asymptomatic female was diagnosed by the color Doppler technique. Doppler color flow mapping on a modified high parasternal short axis view clearly demonstrated the blood flow from the anomalous left coronary artery into the pulmonary trunk. Aortography confirmed this finding. The anatomical factors of marked right coronary artery preponderance and a hypoplastic circumflex artery were considered to contribute to the long survival of this patient. In asymptomatic adult patients with this anomaly, Doppler…
