Does light pose a hazard to the macula in aphakia?

The changes produced within the retina by ultraviolet and blue-violet light are discussed in relation to the increased sensitivity of aphakic and pseudophakic eyes to actinic damage.

Experimental radial keratectomy in rabbits for the correction of aphakia.

The effect on central corneal curvature of radial keratectomy was examined. Ten rabbits had six equally spaced radial wedges removed from the cornea of one eye. Preoperative and serial postoperative keratometry readings were performed to follow the change in refractive power. The follow-up period was 3 months. Radial keratectomy produced an initial steepening of the central cornea, but this effect had disappeared by the end of the follow-up period. It is suggested that this could well have been because the 10/0 prolene sutures pulled through the rabbit cornea, which does not possess a Bowman's

Epikeratophakia: the surgical correction of aphakia. Update: 1982.

Sixty-five epikeratophakia procedures have been performed in 63 patients; visual acuity data have been tabulated on 31 patients with 4 to 30 months follow-up. Patients with more than a year of follow-up showed stable keratometry readings. Early patients achieved 70% of the predicted dioptric correction; more recent patients have achieved 87% with improvements in the lathing procedure, tissue handling, and surgical technique. Visual acuities improve with time. At any given time after surgery, acuities measured with a hard contact lens are better than those measured with spectacles; the decrease

Aphakia: perceptual and refractive problems of spectacle correction.

The perceptual variations due to removal of the crystalline lens and the employment of powerful plus spectacle lenses are described. These include refractive change, distortion, magnification, limitation of the visual field, peripheral vision, sensitivity of light and changes in the required convergence. The technique for satisfactory refraction of the aphakic eye is discussed and the elements leading to error such as vertex distance and pantoscopic tilt are considered. Other factors necessary for understanding the refraction such as the apparent accommodative range and the limitations affecti

Experience with extended wear hydrogel lenses for aphakia.

Nineteen of 23 aphakic eyes were able to wear a hydrogel soft lens for 3 to 14 months (average, 8.32 months). About half of the patients in the study had failed to tolerate all other types of contact lenses. The most annoying problem with the hydrogel lens was the development of surface deposits, which required cleaning or replacement of the lenses. Despite this drawback, the lenses were enthusiastically accepted and provided satisfactory vision for all the patients, several for their only functioning eye.

Clinical experience with hydrophilic lenses in monocular aphakia.

Fifty monocular aphakes were fitted with hydrophilic aphakic contact lenses. Twenty-seven (54%) could wear the lenses for one week or more without removal. Eighteen (35%) could wear the lenses from 1 to 7 days. Five (10%) could not be fitted. No serious complications were noted although corneal epithelial problems and neovascularization needed careful observation. No case of clinical infection, conjunctival or corneal was noted.
