A simplified levator palpebrae superioris (LPS) muscle recession was performed to correct eyelid retraction in 56 eyelids of 48 patients. Improvement of preoperative keratopathy and cosmetically acceptable results were achieved in 93% of the eyelids. In six patients affected by congenital ptosis, a disposable epidural anesthesia needle was used instead of a Wright needle for frontalis suspension surgery. For each of the patients in this study, the use of a disposable needle permitted smaller supra-brow area incisions with a minimal scar. The needle also created a smaller breach in the plane of levator aponeurosis and preaponeurotic fat, causing less tissue edema and bleeding than with a Wright needle.

The deep fascia of the foot lies beneath the subcutaneous tissue and surrounds the intrinsic foot muscles. Depending on its location, the composition of the deep fascia varies. In some areas it is thin, while in other areas it is greatly thickened to form retinacula and the plantar aponeurosis. Selected clinical considerations that relate to the deep fascia of the foot are described. These include the following: plantar fasciitis, infection, compartment syndrome, calcaneal fracture, and neuroma.

Delayed subaponeurotic fluid collections (DSFCs) are benign, focal points of fluid collection beneath the scalp's aponeurosis. We present an infant with a complicated cesarian delivery followed by a persistent, 4-cm effusion on the patient's posterior cranium that remained for 9 months. The exact etiology of this clinical manifestation remains uncertain, but some distinctive characteristics are important for early recognition to ensure proper interventions are taken if necessary.

Skoog and others have concluded that the Skoog fibres, or transverse ligament of the palmar aponeurosis (TLPA), are never involved in the Dupuytren's disease process. Here we present a case in which this clearly occurs. Whilst for the vast majority of Dupuytren's cases the Skoog's fibres delineate the required deep extent of dissection for pretendinous cords, this case proves an exception, highlighting an occasional need to beware of accepting the conventional wisdom.

In a patient requiring tendon transfer after radial nerve palsy, the flexor carpi radialis, a bipennate muscle, was split longitudinally into 2 compartments along the length of the aponeurosis extending proximally from the distal tendon to provide independent finger and thumb extension. This case report shows that the 2 compartments of a bipennate muscle in the forearm may have separate innervation, allowing transfer for independent functions.

Debate continues about the origin of Dupuytren's disease, which is usually in the palm but is seen elsewhere as ectopic lesions. We describe a young patient with Dupuytren's disease extending proximal to the wrist crease in continuity with the palmar lesion. Our findings support the view that the condition starts within the palmar connective tissue, but there is no palmar aponeurosis in the forearm and the proximal extension probably started in the deep layer of the superficial fascia.

A report on compression of the median nerve in distal forearm and wrist by an accessory muscle (the palmaris bitendinous) is described. Its position, deep to the palmaris longus, and distal insertion on the deep surface of the superficial aponeurosis were similar to palmaris profundus. Nevertheless, both its tendinous-musculo-tendinous structure and the origin from the dorsal epimysium of the palmaris longus muscle belly were unlike palmaris profundus.

The palmar aponeurosis typical of Dupuytren's disease has often been encountered in people with diabetes mellitus. A well-tested technique that is highly sensitive and specific was used to identify the signs of Dupuytren's disease in a group of Diabetic Clinic patients and revealed a statistically significant connection between the two conditions. For this reason, the hands should be examined carefully and systematically during periodic check-ups on diabetic patients.
