Surgical options in the treatment of interhemispheric arachnoid cysts.

BACKGROUND: Arachnoid cysts located within the interhemispheric fissure are a matter of discussion as to the surgical indication and the choice of the most appropriate surgical procedure. In fact, in spite of the large dimensions that they can reach, the clinical manifestations may remain subtle and apparently stable over the years. On the other hand, the larger utilization of prenatal ultrasound investigations has increased the number of lesions detected before they can provoke clinical manifestations. As far as surgery is concerned, two main options are available-an extrathecal shunting proc

Optic nerve hypoplasia with hypopituitarism and an arachnoid cyst.

We describe a 5-month-old boy with bilateral optic nerve hypoplasia associated with visual impairment, growth hormone deficiency and an arachnoid cyst in the right temporal lobe on MRI. Optic nerve hypoplasia is sometimes associated with agenesis of the septum pellucidum, commonly known as septo-optic dysplasia, and/or hypopituitarism. There have been a few reports about its association with other abnormalities of the central nervous system, but none about that with an arachnoid cyst.

[Osteoporosis, arachnoid cyst and conus medullaris compression].

The authors report the case of a 75-year-old woman with an osteoporotic vertebral crush fracture of L1, without detectable vertebral necrosis, with signs of conus medullaris compression. The outcome at 2 years was satisfactory with complete recovery of neurological impairment after laminectomy from T12 to L2 and evacuation of a compressive arachnoid cyst overlying L1. This rare case is remarkable because of the presence of an arachnoid cyst. The role of the latter in the pathogenesis and progression of the neurological lesions is discussed.

[Ultrastructural findings of arachnoid cysts and epithelial cysts].

There may be several kinds of pathological conditions in the cystic lesion which are clinically diagnosed as benign intracranial cysts on CT scan. Light and electron microscopic studies on cyst walls were important in the differential diagnosis of benign intracranial cysts. We have studied 5 cases of intracranial arachnoid cysts and two epithelial cysts using the light and electron microscopy. Five cases of intracranial arachnoid cysts included two children and three adults (three females and two males). Three cases of them were localized in the middle cranial fossa, one case in the anterior a

Ultrastructural comparison of arachnoid villi and meningiomas in man.

An ultrastructural comparison of arachnoid villi and meningiomas was made in man. Human arachnoid villi basically consisted of four portions: fibrous capsule; arachnoid cell layer; cap cell cluster; and central core. Arachnoid cells had many characteristic structures in common with meningioma cells: plasmalemmal interdigitations; desmosomes; hemidesmosome-like junctions; and cytoplasmic filaments. Both arachnoid villi and meningiomas were characterized by the numerous extracellular cisterns which appeared to form outflow channels of cerebrospinal or serum fluid. Differences between arachnoid v

Postsurgical arachnoid cyst: report of two cases.

We present two cases that demonstrate the occurrence of symptomatic arachnoid cysts after craniotomy. Both patients presented 1 year postoperatively with generalized seizures as the only symptom. Focal or localizing neurological signs were not present. Both patients responded well to shunting procedures.

Microstructural damage progression in the pia-arachnoid complex.

Traumatic brain injury is a leading cause of death and disability worldwide, with mild traumatic brain injury comprising the majority of cases. The pia-arachnoid complex (PAC), located at the brain-skull interface, plays a critical role in brain mechanics during head impacts, yet its response to mechanical loading is understudied. This study investigates the progression of microstructural damage in the PAC under sub-failure cyclic mechanical loading. Using a newly developed inflation technique, we measured the displacement of the arachnoid membrane in piglets, adult pigs, and adult sheep durin

Regulation of folate transport at the mouse arachnoid barrier.

BACKGROUND: Folates are a family of B9 vitamins essential for normal growth and development in the central nervous system (CNS). Transport of folates is mediated by three major transport proteins: folate receptor alpha (FRα), proton-coupled folate transporter (PCFT), and reduced folate carrier (RFC). Brain folate uptake occurs at the choroid plexus (CP) epithelium through coordinated actions of FRα and PCFT, or directly into brain parenchyma at the vascular blood-brain barrier (BBB), mediated by RFC. Impaired folate transport can occur due to loss of function mutations in FRα or PCFT, resultin

Giant intracranial arachnoid cyst in an outstanding soldier.

This article reports a case of a large intracranial arachnoid cyst in a soldier who had performed well. This is a very marvelous and interesting case. Although the cyst pressed the entire brain into the contralateral cranial cavity, the patient did not show any significant clinical symptoms and had excellent motor and cognitive abilities, reflecting the excellent plasticity of the brain tissue.

Coexistence of Trigonocephaly and Sylvian Arachnoid Cysts: A Coincidence?

INTRODUCTION: The association between trigonocephaly and Sylvian fissure arachnoid cysts (ACs) has been occasionally reported in the literature. However, the real incidence of this association and its clinical relevance remain unknown. METHODS: The authors collected and retrospectively reviewed all clinical charts and CT scans of patients surgically treated for trigonocephaly at the Pediatric Neurosurgical Department of Fondazione Policlinico Universitario "Agostino Gemelli" IRCCS from January 2014 to June 2023. RESULTS: During the study period, 136 patients with trigonocephaly underwent sur

The role of arachnoid granulations in idiopathic intracranial hypertension.

OBJECTIVES: Idiopathic intracranial hypertension (IIH) is a disease defined by elevated intracranial pressure (ICP) without an established etiology. Arachnoid granulations (AG) are conduits for CSF resorption from the subarachnoid space to the venous system. AG have been implicated to play a central role in maintaining CSF homeostasis. We tested the hypothesis that patients with fewer visible AG on MRI are more likely to present with IIH. METHODS: In this institutional review board (Institutional Review Board)-approved retrospective chart review study, 65 patients with a clinical diagnosis of

Giant Arachnoid Granulations: A Systematic Literature Review.

Giant arachnoid granulations (GAGs) are minimally investigated. Here, we systematically review the available data in published reports to better understand their etiologies, nomenclature, and clinical significance. In the literature, 195 GAGs have been documented in 169 persons of varied ages (range, 0.33 to 91 years; mean, 43 ± 20 years; 54% female). Prior reports depict intrasinus (i.e., dural venous sinus, DVS) (84%), extrasinus (i.e., diploic or calvarial) (15%), and mixed (1%) GAG types that exhibit pedunculated, sessile, or vermiform morphologies. GAG size ranged from 0.4 to 6 cm in maxi

Ophthalmic manifestations of ruptured arachnoid cysts in children.

BACKGROUND: Intracranial arachnoid cysts are relatively common in the pediatric population. Rarely, they rupture, leading to acute subdural fluid collections, which can cause a sudden increase in intracranial pressure. The purpose of this study was to characterize ophthalmic sequelae in a large cohort of these patients. METHODS: The medical records of all children treated for ruptured arachnoid cysts who presented at a single tertiary pediatric hospital for initial assessment between 2009 and 2021 were reviewed retrospectively. RESULTS: Of 35 children treated for ruptured arachnoid cysts dur

Aging-associated inflammation and fibrosis in arachnoid membrane.

BACKGROUND: The physiological and pathological significance of the arachnoid membrane (AM) is still unknown. In this study, we investigated various characteristics of the AM, focusing on the influence of inflammation and fibrosis. METHODS: Small pieces of AM sample were obtained during neurosurgical procedures from 74 cases. The clinical and pathological characteristics of the hyperplastic AM group (≥ 50 μm) and the non-hyperplastic AM group (< 50 μm) were compared. Then, potential correlations between AM thickness and clinical characteristics were analyzed. Moreover, VEGFα, TGFβ, and TGFα le
