Occurrence of giant cell arteritis...suddenly.

PURPOSE: To define the kinetics and mechanisms of frank arteritis onset in patients with giant cell arteritis. METHODS: Cytokines were analyzed from tissue of a patient before and after the development of arteritis. RESULTS: A temporal artery biopsy specimen from a patient with giant cell arteritis showed no pathologic changes on microscopic examination, but there was evidence of early tissue activation of inflammatory markers. A specimen from the contralateral artery 12 days later had high levels of IL-18 transcripts and abundant transcripts for CCL19. Also, CD83 and IL-1 were present, conf…

[Therapeutic approach to progressive Takayasu's arteritis].

BACKGROUND: Takayasu's arteritis (TA) is a nonspecific autoimmune inflammation manifested with systemic large vessel vasculitis which affects predominantly the aorta and its main branches. CASE REPORT: We presented a 40-year-old woman with a two-year history of the symptoms of compromised circulation in the head, the neck, the upper extremities and the right leg, followed by claudications in the left leg, numbness in the left hand and stenocardia. Aortic arch angyography revealed occlusion of the right subclavian artery stenosis of the right vertebral artery (20-30%) and the right iliac arter…

Annuloaortic ectasia and giant cell arteritis.

BACKGROUND: Thoracic aortic aneurysm, aortic dissection and aortic valve regurgitation have been widely described in patients with Horton disease, also known as giant cell arteritis. We present our midterm experience with patients with these features. METHODS: A total of 386 cases of ascending aorta and aortic valve replacement performed for thoracic aortic aneurysm and aortic insufficiency between 1998 and 2004 were reviewed. Among them 10 cases of histopathologically confirmed GAA were identified. Patients were predominantly female (90%); the mean age was 74.5 +/- 4.6 years. RESULTS: Eight…

[Polymyalgia rheumatica and giant-cell arteritis].

The subjects of the study were 112 patients with rheumatic polymyalgia (RPM) and giant-cell arteritis (GSA). The study shows that RPM and GSA are complicated by cerebral flow disturbances and myocardial infarction. Therapy with prednisolone and non-steroid antiinflammatory drugs reduces the risk of these complications and lethal outcome in patients with RPM and GSA.

Pyoderma gangrenosum associated with Takayasu's arteritis.

Pyoderma gangrenosum (PG) is a neutrophilic dermatosis characterized by destructive, necrotizing and noninfective ulceration of the skin mostly on lower extremities. PG is well known as a complication of Takayasu's arteritis in Japan. However, this association is not commonly observed in North American and European patients. We describe a case of PG that was associated with Takayasu's arteritis who was successfully treated with systemic cyclosporin. We have reviewed 35 well-documented PG cases with Takayasu's arteritis in comparison to 106 PG cases without Takayasu's arteritis. The results dem…

Pleural effusion revealing giant cell arteritis.

Pulmonary involvement is considered to be rare in giant cell arteritis (GCA), usually occurring in the course of the disease. We describe the case of a patient who developed left pleural effusion revealing GCA. Thoracic CT scan demonstrated an abundant left pleural effusion and a thickening of the aortic wall. The patient's condition improved rapidly, with regression of pulmonary clinical features and complete clearance of pleural effusion, after institution of steroid therapy. Our case report reinforces the possibility of unusual presentation of GCA; such a diagnosis should, therefore, be con…

Inflammatory diseases associated with Takayasu's arteritis.

The etiology of Takayasu's arteritis is unknown; however, abnormality of the immune system plays an important role. To clarify a possible association between inflammatory diseases and Takayasu's arteritis, the authors retrospectively reviewed the medical records of 36 patients (2 men and 34 women), admitted to their department with this diagnosis. They found that 11 of the 36 patients had at least 1 other chronic or subacute inflammatory disease: Chronic tonsillitis, tuberculous lymphadenitis of the neck, chronic hepatitis B (infectious diseases); pyoderma gangrenosum, erythema nodosum (skin l…

Polymyalgia rheumatica/temporal arteritis: recent advances.

Polymyalgia rheumatica (PMR)/temporal arteritis (TA) frequently causes significant morbidity in patients older than 50 years of age. This review highlights recent trends in clinical findings, epidemiology, pathogenesis, and laboratory and radiologic assessment of the disease. Although steroids are the mainstay of therapy because of their effectiveness and ease of administration, they have numerous side effects, particularly in an aging population. Furthermore, recent evidence suggests that steroids only suppress clinical symptoms, while a smoldering level of damaging vascular inflammation pers…

Sex differences in giant cell arteritis.

OBJECTIVE: Although it has been suggested that sex differences underlie the varying presentation of giant cell arteritis (GCA), this has not been proven. We compared medical history, symptoms, and signs in patients with GCA and polymyalgia rheumatica (PMR). METHODS: We performed a retrospective study in the Hadassah University Hospitals in Jerusalem, Israel. We evaluated medical data of 88 patients (59 women, 29 men) admitted with the diagnosis of GCA or PMR between 1980 and 1998. RESULTS: Comparison of comorbidities among patients showed that non-insulin dependent diabetes mellitus, cerebro…

Microscopic haematuria in giant cell arteritis.

Giant cell arteritis (GCA) is traditionally considered to spare the kidney, although an uncontrolled study reported microscopic haematuria in 10 out of 30 patients with GCA. To study the frequency and the characteristics of microscopic haematuria in GCA, we retrospectively studied 42 patients with biopsy-proven GCA, 39 patients with polymyalgia rheumatica (PMR) and 62 control patients >or=60 years of age, admitted to the general internal medicine unit. Patients with pyuria, significant bacteriuria or a known haematuric disorder were excluded. Microscopic haematuria was defined as the presence …

[Takayasu's arteritis: underestimated cause of hypertension].

UNLABELLED: Takaysu's arteritis (TA) is regarded to be a frequent cause of hypertension in Asian population. However some reports have recently informed about increasing frequency of its also in non Asian people. The aim of this study was to evaluated own experiences on diagnosis and treatment of TA as well as prevalence of TA in hypertensive patients in Caucasian population. MATERIALS AND METHODS: During last 15 years we examined 4190 hypertensive patients hospitalized in our department. Renovascular hypertension was recognized in 139 cases. The diagnosis of TA was based on angiographically …

Polymyalgia rheumatica and giant cell arteritis.

PMR and GCA are related conditions that seem to represent a continuum of disease. These conditions are relatively common and seem to be mediated by a cellular inflammatory response. Increasing evidence suggests an infectious cause (or causes) precipitating this immune response in genetically susceptible individuals. Whereas previously thought to affect primarily branch vessels of the aortic arch, GCA is now thought of as a disease in which proximal aortic involvement is frequent. Despite the potential for serious, even fatal complications, overall prognosis for patients with GCA or PMR is exce

Dysarthria associated with giant cell arteritis.

Over a one month period, a 74-year-old man developed typical features of giant cell arteritis (GCA) including visual changes, headache, scalp tenderness, and an elevated erythrocyte sedimentation rate. In addition, he had reproducible painless dysarthria that was precipitated by chewing or prolonged talking and was relieved by resting the jaw. The dysarthria with chewing along with the other classical symptoms of GCA subsided with treatment. To our knowledge this is the first report of painless dysarthria associated with GCA.

Ocular manifestations of giant cell arteritis.

PURPOSE: To report the ocular manifestations of giant cell arteritis using the strict criterion of a positive temporal artery biopsy for diagnosis of giant cell arteritis. METHODS: In a prospective study from 1973 to 1995, we investigated 170 patients whose diagnosis of giant cell arteritis was confirmed on temporal artery biopsy. At the initial visit, all patients were questioned regarding systemic and ocular signs and symptoms of giant cell arteritis and underwent ophthalmic, erythrocyte sedimentation rate (Westergren), and C-reactive protein evaluations. Any patient with a high index of su…
