An inverted left atrial appendage which fails to revert spontaneously is a rare complication of cardiac surgery. We present a case of an inverted left atrial appendage discovered intraoperatively on transoesophageal echocardiography. This was readily identified and was easily corrected with digital manipulation. Intraoperative transoesophageal echocardiography plus an awareness of the possibility that a newly presenting left atrial mass post-bypass might be an inverted left atrial appendage, facilitates immediate correction. So doing removes any need for further investigation or further cardia…

Radiofrequency catheter ablation (RFCA) at the ostium of the pulmonary veins can potentially cure atrial fibrillation. Left-sided ablation procedures may carry a risk of thrombo-embolic events. Emboli from intracardiac thrombus originate from the left atrial appendage (LAA) in a substantial number of patients. The presence of spontaneous echo contrast in the left atrium (LA) and LAA peak emptying flow < or = 20 cm/s are also significantly related to thrombo-embolic events. Evaluation of the anatomy of the LA and LAA prior to left-sided procedures is therefore mandatory. Intracardiac echocardio…

Fifty-one patients were divided into two groups: 20 patients with proven rheumatic mitral valve disease (RMVD) and 31 patients with left atrial enlargement (LAE) of a nonrheumatic etiology. The latter group included patients with ischemic papillary muscle dysfunction, mitral valve prolapse, and congestive cardiomyopathy. Radiographic studies showed that enlargement of the left atrial appendage (LAAE) was present in 18 of 20 rheumatics but in only one of 31 non-rheumatics. There was no direct relationship between enlargement of the LAA and radiographic or echocardiographic left atrial size, deg…

INTRODUCTION: Typical atrial flutter (AFL) is a macroreentrant tachycardia in which intracardiac conduction rotates counterclockwise around … AFL. Electroanatomical mapping was performed both during typical AFL and entrainment from the right atrial appendage (RAA). The 12-lead electrocardiogram (ECG) and three-dimensional (3D) electroanatomical maps were analyzed.
RESULTS: The positive F wave in lead V1 changed during entrainment from the RAA in all the cases. The 3D map during entrainment from the RAA …

EuroIntervention : journal of EuroPCR in collaboration with the Working Group on Interventional Cardiology of the European Society of Cardiology2026eng
Left atrial appendage closure (LAAC) is an established alternative for stroke prevention in patients with non-valvular atrial fibrillation who are not suitable candidates for long-term oral anticoagulation. Imaging is central to procedural success and safety, yet variability in acquisition, measurement, and reporting contributes to inconsistent workflows and outcomes among centres. This expert consensus document from the European Left Atrial Appendage Closure Club provides a unified framework for pre-, intra-, and postprocedural imaging in LAAC with the aim of reducing variability, standardisi…

BACKGROUND: Epicardial and endocardial left atrial appendage occlusion (LAAO) is commonly performed for stroke prevention for atrial fibrillation (AF) patients at increased risk for stroke who cannot tolerate long-term oral anticoagulation. However, there is limited data on the outcomes of unsuccessful LAAO procedures. This study aims to assess the incidence and outcomes of unsuccessful LAAO procedures.
METHODS: A retrospective analysis was performed on patients undergoing LAAO at our institution from 2020 to 2023. The primary endpoint was a failure to deploy the LAAO device, which was define…

OBJECTIVE: We report our initial cases of robotic left atrial appendage occlusion (LAAO) device deployment.
METHODS: From June 2021 to December 2024, all consecutive patients undergoing robotic-assisted LAAO (isolated or with robotic-assisted CABG) using an epicardial clip were enroled. The study protocol was approved by the Institutional Review Board (IRB 45CFR164.512).
RESULTS: A total of 28 patients were included, 22 of which underwent a concomitant CABG. Transesophageal echocardiogram confirmed excellent exclusion of the LA appendage in all but one patient. There were no postoperative st…

Peri-device leak and device-related thrombus1 remain key challenges of current left atrial appendage occlusion (LAAO)2 owing to the incompatibility between the solid occluder and the left atrial appendage (LAA). Here we propose a personalized and complete LAAO using magnetofluids that is suitable for all types of LAAs. Magnetofluids can be injected into LAAs from cardiac catheters. In the presence of a sufficient magnetic field, magnetofluids can resist high-speed blood flow. Magnetofluids can precipitate into magnetogels in contact with water in the blood within only a few minutes. We further…

Percutaneous left atrial appendage occlusion (pLAAO) is increasingly being adopted as an alternative to non-vitamin K antagonist oral anticoagulants (NOACs) for patients with atrial fibrillation. However, the current evidence does not justify this enthusiasm. Key limitations include wide non-inferiority margins in some of the trials, inclusion of components in the primary efficacy endpoint that are not directly influenced by pLAAO, making non-inferiority easier to achieve, limited statistical power to detect differences in ischemic stroke or systemic embolism, and an overstated bleeding advant…

Atrial fibrillation is a frequent cardiac arrhythmia and is associated with an increased risk of cardioembolic stroke. The left atrial appendage is a finger-like extension originating from the main body of the left atrium and the main location of thrombus formation in patients with atrial fibrillation. Surgical or percutaneous left atrial appendage occlusion (LAAO) aims at preventing clot formation in the left atrial appendage. Here, we describe available surgical and percutaneous approaches to achieve LAAO and discuss the available evidence for LAAO in patients with atrial fibrillation. We di…

BACKGROUND: Procedure volumes are associated with outcomes for many cardiovascular procedures, leading to guidelines on minimum volume thresholds for certain procedures; however, the volume-outcome relationship with left atrial appendage occlusion is poorly understood. As such, we sought to determine the relationship between hospital and physician volume and WATCHMAN left atrial appendage occlusion procedural success overall and with the new generation WATCHMAN FLX device.
METHODS: We performed an analysis of WATCHMAN procedures (January 2019 to October 2021) from the National Cardiovascular …

Percutaneous left atrial appendage closure (LAAC) is a valid alternative to oral anticoagulation to prevent ischemic stroke in patients with atrial fibrillation. The devices approved in Europe and United States for percutaneous LAAC contain metal and temporary antithrombotic therapy is strongly recommended following implantation to prevent thrombus formation on the atrial device surface. There is still uncertainty regarding to the optimal antithrombotic drug regimen after device implantation for several reasons. Thus, this review aims at summarizing the available evidence and the remaining chal…

59-year-old women with permanent atrial fibrillation and previous haemorrhagic stroke was admitted for percutaneous left atrial appendage occlusion. Shortly after, AmplatzerTM device migration into the left ventricular outflow tract was confirmed. Unsuccessful transcatheter mobilization led to an emergent surgery for device retrieval and LAA closure. No events at 6-month follow-up.

Atrial fibrillation is an increasingly prevalent entity faced by cardiac surgeons. While oral anticoagulation therapy aims to reduce the risk of thromboembolic events patients may desire to discontinue these medications or develop contraindications to their use. Left atrial appendage ligation permits stoppage of oral anticoagulation while also reducing the risk of cerebrovascular events. This manuscript describes the techniques employed in the first reported successful uniportal/single-incision, thoracoscopic epicardial left atrial appendage ligation.
