Effect of the stellate ganglion block on symptoms of ulcerative colitis: A case report.

RATIONALE: Chronic ulcerative colitis is an autoimmune disease in which epithelial injury continuously occurs in the colonic mucosa. While mesalazine (5-aminosalicylic acid) is used to treat ulcerative colitis, it can also cause liver failure, headaches, and abdominal pain; therefore, an alternative treatment is required. The purpose of this study was to evaluate the effectiveness of 80 stellate ganglion blocks in reducing pain and other symptoms in a patient with chronic ulcerative colitis. PATIENT CONCERNS: A 54-year-old female patient with a history of ulcerative colitis was concerned with

Celiac plexus block with botulinum toxin in severe chronic pancreatitis-A case report.

WHAT IS KNOWN AND OBJECTIVE: The botulinum toxin (BoNT) has been widely used for various conditions associated with pain. CASE DESCRIPTION: Here, we report a case where celiac plexus block (CPB) with BoNT relieved intractable chronic pancreatic pain without complications. CPB was performed at the L1 level under fluoroscopic guidance, and 50 IU BoNT was injected on each side. After 15 weeks, pain was decreased to 0/10 on a visual analogue scale, without opioids or tramadol. WHAT IS NEW AND CONCLUSION: Our case demonstrates the efficacy of CPB with BoNT in intractable pain due to severe chroni

Harlequin Syndrome After Thoracic Paravertebral Block.

Harlequin syndrome is characterized by the sudden onset of unilateral facial flushing and sweating, often preceded by exercise, excessive heat, or, rarely, regional anesthesia. Although the exact mechanism remains unclear, it is often referred to as transient or permanent interruption of the sympathetic nervous system. We present a case of Harlequin syndrome without Horner syndrome in a patient with unilateral right-sided facial flushing that started shortly after a left-sided thoracic paravertebral nerve block for a mastectomy. We discuss the interruption of the sympathetic and parasympatheti

Ultrasound guided greater occipital nerve blocks for post-traumatic occipital neuralgia.

Chronic headaches can be debilitating for many patients. They often have a nebulous etiology, unpredictable course, and can be difficult to manage. We describe a post-traumatic headache that began after a motor vehicle collision. The patient sustained multiple injuries including a scalp laceration and bilateral occipital condyle fractures. Oral agents were unable to quell this patient's headaches. The diagnosis of occipital neuralgia was suspected based on history and presentation. Our patient received dramatic relief after ultrasound guided bilateral greater occipital nerve blocks.

The role of neurolytic celiac plexus block in the treatment of pancreatic cancer pain.

Pancreatic carcinoma, an important leading cause of cancer death, has increased steadily in incidence and still has a poor prognosis. Pain is one of the most frequent symptoms, affecting more than 75% of patients. It is often present in the early stages of disease and may be severe and difficult to treat. Abdominal viscera, including pancreas, liver, gallbladder, adrenal, kidney, and the gastrointestinal tract from the level of the gastroesophageal junction to the splenic flexure of the colon are innervated, at least in part, via the celiac plexus. Thus, painful tumors in these viscera may hav

Reflex sympathetic dystrophy involving three limbs: a case study.

Reflex sympathetic dystrophy (RSD) is a symptom complex characterized by pain, hyperesthesia, and vasomotor instability. RSD has long been considered a manifestation of sympathetic nervous system dysfunction because of characteristic edema and vasomotor instability in addition to frequent alleviation of RSD symptoms by sympathetic blockade. RSD generally only affects one limb although it is recognized that the contralateral limb is also abnormal and at risk of also developing clinical RSD without the advent of a new injury. An unusual case of three-limb RSD is presented where classical RSD was

Transient locked-in syndrome after vascular injection during stellate ganglion block.

Intravertebral artery injection of local anesthetics is a rare and alarming complication of neural blocks of the neck. Although seizures, apnea, and loss of consciousness occur, an unusual syndrome with sparing of eye movement might also occur. In transient locked-in syndrome, the patient remains conscious despite inability to breathe or move. The authors report a patient with apparent transient locked-in syndrome that resulted during a stellate ganglion block. This complication is different from frequently reported stroke-like complications. Needle position in the vertebral artery is a possib

[Continuous retroperitoneal infusion of a novocaine cocktail in the combined treatment of severe forms of acute pancreatitis].

Severe forms of acute pancreatitis were treated in 53 patients (25 with a destructive form, 28 with an edematous form) with using permanent infusions of a novocaine cocktail into the retroperitoneal space. General lethality was reduced to 5.7%. In the group of patients with a destructive form of acute pancreatitis lethality was 12%. Nobody died in the group of patients with the edematous form. No complications were noted resulting from the method.

Right vagal nerve stimulation during minimally invasive direct coronary artery bypass grafting in dogs. A preliminary study.

BACKGROUND: Minimal access coronary artery bypass grafting without cardiopulmonary bypass has been used with increasing frequency. However, some surgeons are still unwilling to perform MIDCAB because the anastomosis is technically demanding. The aim of this study was to assess whether autonomic activation via right vagal nerve stimulation will be an effective support technique on heart rate during MIDCAB in dogs. EXPERIMENTAL DESIGN: Preliminary study. SETTING: Cardiothoracic Surgery Unit, University Clinic. METHODS: The right cervical vagal nerve was stimulated in five dogs. In one dog, ri

[Potential Mechanisms Underlying Acupuncture Regulation on Hypertensive Myocardial Hypertrophy and Cardiac Function Damage].

Long-lasting hypertension results in ventricular remodeling characteristic of myocardial hypertrophy including myocyte hypertrophy and myocardial fibrosis, a pathological process failed to be blocked by the current biomedical means and could eventually lead to heart failure. It has been shown that acupuncture can excite the parasympathetic nerve to counter the overtonic activity of sympathetic nerve, elicit antihypertensive effect and produce cardioprotection via the regulation of calcium signaling-related proteins in myocytes, suggesting its potential role in the management of these disorders

A randomized controlled trial of femoral nerve blockade administered preclinically for pain relief in femoral trauma.

BACKGROUND: Analgesia at the location of the accident and on transport for femoral trauma is often delayed or insufficient. In this prospective, randomized, controlled study, we evaluated the preclinical use of femoral nerve blockade for reducing pain and anxiety compared with IV analgesia using metamizol. METHODS: Patients with painful femoral trauma, such as fracture or severe contusion, were randomized to receive at the site of the accident a femoral nerve blockade (n = 31) or IV analgesia with metamizol (n = 31). A visual analog scale (VAS) was used to assess pain and anxiety. Variables w

Severe bradycardia in a teenager as the initial manifestation for Guillain Barré syndrome: a case report.

BACKGROUND: Guillain-Barré syndrome is the most common cause of flaccid paralysis, with multiple known clinical variants. Autonomic dysfunction, although frequently reported in the clinical course, is often overlooked in the pediatric population and is usually not the initial presenting symptom in this age group CASE PRESENTATION: We present the case of a previously healthy 17-year-old who arrived at the Emergency Department complaining of gastrointestinal symptoms associated with lipothymia. An initial electrocardiogram (ECG) showed sustained sinus bradycardia subsequently associated with art

Effect of Lambert-Eaton myasthenic syndrome antibodies on autonomic neurons in the mouse.

Somatic muscle weakness and autonomic symptoms characterize the autoimmune Lambert-Eaton myasthenic syndrome (LEMS). The former results from IgG autoantibody-mediated down-regulation of P/Q-type voltage-gated calcium channels at motor nerve terminals and consequent reduction in acetylcholine release; the basis for the autonomic symptoms is unknown. Using omega-conotoxins GVIA and MVIIC and omega-agatoxin IVA that block N-, Q-, and P-type channels, we investigated ex vivo the calcium channels subserving transmitter release from postganglionic parasympathetic neurons in the bladder and from post
