A case report is presented of a 36-year-old U.S. Coast Guard aviator who had a single seizure while operating a helicopter on the ground. His seizure activity produced a loss of consciousness during which he pushed the cyclic to the left anterior quadrant that resulted in a ground mishap. No risk factors were identified in an extensive neurological workup. The current guidance for handling seizures in military aviation personnel is reviewed, along with considerations for treatment. Although the military aviation selection process carefully screens applicants for seizure history and potential, …

Arrhythmogenic right ventricular dysplasia (ARVD) is an important cause of sudden death in the young. The classic presentation is with sustained ventricular tachycardia, with an electrocardiographic pattern of a left bundle branch block, but a proportion of the patients may present as sudden cardiac death. Establishing the diagnosis in aviators may be particularly important since it may prevent sudden incapacitation. We present a case of an aviator with ARVD, in whom the diagnosis was delayed despite an abnormal ECG pattern witnessed over 20 yr. The aviator presented with symptomatic sustained

To address the challenges of increasing carbon dioxide (CO2) emissions and climate change caused by the growth of air traffic, accurate prediction of CO2 emissions in civil aviation has become crucial. This study proposes a CO2 emission prediction method based on an improved back propagation (BP) neural network, where the Improved Sparrow Search Algorithm (ISSA) is employed to optimize the hyperparameters of the BP neural network, thereby enhancing the prediction capability for CO2 emissions in civil aviation. To overcome the limitations of the traditional SSA, such as the tendency to fall int…

BACKGROUND/OBJECTIVES: Military aviators can be exposed to extreme physiological stressors, including decompression stress, G-forces, as well as intermittent hypoxia and/or hyperoxia, which may contribute to neurobiological dysfunction/damage. This study aimed to investigate the levels of neurological biomarkers in military aviators to assess the potential risk of long-term brain injury and neurodegeneration.
METHODS: This cross-sectional study involved 48 Canadian Armed Forces (CAF) aviators and 48 non-aviator CAF controls. Plasma samples were analyzed for biomarkers of glial activation (GFA

We present in this case report the return to flying duty of a pilot with vocal cord paralysis secondary to removal of a thymoma. We discuss the importance of glottic function as it pertains to the unique aviation environment. We also discuss the anatomy and physiology of the glottis, the evaluation for vocal cord paralysis, and surgical approaches for paralyzed vocal cords. Although the incidence of recurrent laryngeal nerve paralysis is low in the military aviation community, it is important to recognize that its sequelae can be managed so that the aviator may return to flight duties.

With the rapid expansion of China's civil aviation industry, airport aircraft emissions have raised growing environmental concerns, making it imperative to clarify their emission characteristics and driving mechanisms. In this study, 10 major civil aviation airports in Shandong Province, a highly representative airport cluster in China, were selected to investigate the characteristics of aviation emissions using actual flight activity data from 2023. Our results show that the total emissions of HC, CO, NOx, SO2, PM, and CO2 from the airport aircraft were 144.6, 1890.4, 2571.8, 358.7, 18.2, and…

INTRODUCTION: Obesity represents a growing concern in the aviation environment due to its association with cardiovascular disease, diabetes mellitus, and obstructive sleep apnea, conditions that may compromise aeromedical certification and long-term medical fitness for flight duties. Although bariatric surgery (BS) effectively reduces obesity-related comorbidities, its long-term implications for pilots remain insufficiently characterized from an aeromedical perspective. This systematic review evaluates long-term health outcomes following BS relevant to flight personnel performance, safety, and…

Mathematical modeling suggests that travel restrictions are likely to have only a limited effect on minimizing the spread of disease. Nevert … ajor challenge is to ensure adequate resources are devoted to pandemic preparedness planning in the aviation sector, which may not be fully considered in a national preparedness plan. This is because the prevention of accidents occupies most of the attention of regulatory aviation authorities, and public health authorities do not always see aviation as a p …

Theodore C. Lyster is a familiar name to aerospace medicine physicians. His early recognition of the unique physical requirements of aviators, the specialized training necessary for flight surgeons, and the need for altitude physiology research provided the foundation on which the specialty of aviation medicine was built. Lyster's medical career, however, encompassed much more than aviation medicine. From his earliest assignment as a contract physician in Cuba in 1899 until his entry into private practice in 1921, he was heavily involved with the fight against yellow fever. In the era before m…

INTRODUCTION: Suicide is the 11 th leading cause of death in the United States. The objective of this study is to document the characteristics of aviation-related suicides and suicide attempts.
METHODS: Aviation accidents reported by the National Transportation Safety Board between 1983 and 2003 were screened for cases in which suicide was listed as a probable cause. For each suicide case, two accidents were randomly selected as controls, matched on sex of pilot, type of flight, state, and year of occurrence. Mantel-Haenszel summary Chi-square tests were used to compare cases to controls. Con…

Apical hypertrophic cardiomyopathy (ApHCM), a subtype of hypertrophic cardiomyopathy, may be found incidentally in healthy young adults. Arrhythmias are poor prognostic signs, and are the most frequent cause of sudden cardiac death. We present two cases of military aviators with ApHCM. One was a high-performance jet weapon system operator, who had asymptomatic non-sustained ventricular tachycardia (NSVT) and subsequently a symptomatic episode of paroxysmal atrial fibrillation. The second was a helicopter pilot, who had asymptomatic NSVT. Both aviators continue their aviation duties without exp…

INTRODUCTION: Maintaining spatial orientation in flight remains a critical aspect of aviation safety. Monitoring an aviator's neurophysiological patterns may provide insight and opportunity to mitigate loss of spatial orientation. However, few studies have examined the utility of such measures. The purpose of the current study was to conduct a scoping review to document the research activities that have examined neurophysiological measures in relation to spatial orientation.
METHODS: Four databases were searched for literature using neurophysiological measures in studies assessing disorientat…

Children are vulnerable to environmental hazards and spend significant portions of their days at school. However, just one national-level study has examined school-level environmental inequalities (in air pollution exposures), and none have examined disparate exposures to noise pollution, even though noise impacts children's health and development. We integrated data from 2014-2015 on the locations and socio-demographics of each public school in the contiguous US (n=94,432) with road and aviation transportation noise estimates. Using bivariate and multivariate statistics, we tested for dispari…

During the investigation of fatal aviation accidents, postmortem samples from the pilots/co-pilots are submitted to the Federal Aviation Administration's (FAA) Civil Aerospace Medical Institute (CAMI) for toxicological analysis. Although therapeutic levels for most drugs are typically reported in the scientific literature for blood and plasma, blood specimens are received in only approximately 70% of our cases. Therefore, it is imperative for an accident investigator and forensic toxicologist to be able to estimate drug concentrations in an aviation accident victim's blood from available tissu…

Beginning in 1985, the Federal Aviation Administration (FAA) came to the realization that regulating crew rest for pilots was an overdue and effective risk-management tool. Fatigue was being increasingly recognized as a contributing factor in many emergency medical services (EMS) accidents. Federal aviation regulations were changed to include pilot crew rest and flight hour and duty time limitations.

Crew resource management (CRM) describes a system developed in the late 1970s in response to a series of deadly commercial aviation crashes. This system has been universally adopted in commercial and military aviation and is now an integral part of aviation culture. CRM is an error mitigation strategy developed to reduce human error in situations in which teams operate in complex, high-stakes environments. Over time, the principles of this system have been applied and utilized in other environments, particularly in medical areas dealing with high-stakes outcomes requiring optimal teamwork and …

Climate change is the megatrend that will have the biggest impact on the development of sustainable air transportation in near future. Aviation is expected to triple its proportional share of a Paris compatible 1.5 °C budget, declared by UNFCCC Agreement for global temperature through 2050 under current international policies. Basket of measures proposed by ICAO to keep the temperature change under this limit, including aircraft technology (up to 25%) and operation improvement (up to 9%) for fuel burn reduction by engines and new revolutionary architectures of the aircraft, deployment of susta…
