How chamber-specific hemodynamic shear forces coordinate the spatial and temporal patterning of cardiac valves remains poorly understood. The zebrafish atrioventricular (AV) valve transitions from 2-cusp to 4-cusp at 14 days post-fertilization (dpf) through commissural remodeling, differential cell proliferation, and chondrogenic matrix secretion. Orthogonal valvular axes are present at 25 hpf (hours), preceding endocardial cushion formation. AV canal remodeling aligns the shear stress with leaflet formation at 48 hpf and 14 dpf. At 28 hpf, the ventricle develops functional syncytium, while th

Severe aortic stenosis typically presents with reduced exercise tolerance, exertional chest pains, or syncope. We report on a case of a young female on therapeutic anticoagulation and a history of nephrotic syndrome, who presented with subacute limb ischaemia resulting from axillary artery thrombus. Urgent echocardiogram demonstrated a bicuspid aortic valve with critical stenosis, and she underwent surgical aortic valve replacement. Her presenting symptoms resolved after three months of warfarin therapy. This case highlights the importance of systemic evaluation of unexplained arterial thrombo

Here, we present a case of infective endocarditis (IE) caused by Gemella sanguinis, a pathogen that rarely causes human infection. The patient in this case was an otherwise healthy 50-year-old man who presented with gradually worsening shortness of breath, weight loss, fatigue, chills, and leg swelling for six weeks. He had no prior history of cardiac disease and had never used intravenous drugs, but he had undergone extensive dental work following a military-associated injury. A workup for IE was done, and echocardiography revealed a bicuspid aortic valve (BAV) with vegetations causing modera

BACKGROUND: The anatomy of bicuspid aortic valves (BAV) varies considerably and is broadly classified into two main types: two-sinus and fused BAV. Possible prognostic implications of these two main types remain unclear. This study aimed to assess potential associations between BAV morphology and the timing of surgery of the aortic valve or ascending aorta.
METHODS: A multi-center cohort study including 1004 adult outpatients with BAV. BAV morphology was classified as either two-sinus or fused type. The primary outcome was a composite of surgical or endovascular intervention on the aortic val

Marfan syndrome is a genetic condition that causes connective tissue abnormalities that can ultimately lead to cardiovascular complications such as aneurysms. When a bicuspid aortic valve is also present, which is a congenital heart defect characterized by the aortic valve having two cusps instead of three, a combined effect of serious cardiovascular complications can result. There is an additive and increased risk of aortic complications due to the combined impact on aortic integrity that can lead to aneurysm formation and/or dissections. A severe case of aortic stenosis with a thoracic aorti

BACKGROUND: Bicuspidization repair is typically applied to severe congenital aortic valve lesions. Increasingly, this repair is recognized as an effective and durable surgical approach. The consequences of postoperative leaflet geometry in this repair are relatively unstudied, especially compared with current understanding of optimal geometry for a trileaflet valve. In this work, a computational approach was systematically applied to study the effect of changes in leaflet geometric height in symmetric bicuspidization repair.
METHODS: Eight model valves with 4 geometric heights spanning a rang

BACKGROUND: Aortic valve stenosis (AS) occurs in bicuspid aortic valve (BAV) patients at a relatively young age compared to tricuspid aortic valve (TAV) patients. However, the underlying cause of this phenomenon remains unknown. Neopterin, which is a by-product of the guanosine triphosphate (GTP) pathway, enhances the oxidative potential of reactive oxygen species. To clarify the role of neopterin in the aortic valve, we immunohistochemically studied the presence of neopterin in aortic valve specimens from patients with AS harboring either TAV or BAV.
METHODS: Frozen aortic valve samples were

BACKGROUND: Identifying bicuspid aortic valve (BAV) patients at risk for cardiac events remains challenging and the role of the electrocardiogram (ECG) has not yet been described. Therefore, this study aims to describe ECG parameters in BAV patients, and investigate their prognostic value.
METHODS: In this single-center prospective study patients with BAV without a prior aortic valve replacement (AVR) were included. Transthoracic echocardiogram and 12‑lead resting-ECG were obtained. Associations between ECG parameters and the composite endpoint of all-cause mortality and AVR were assessed usi

BACKGROUND: Neonatal truncal valve plasty remains a significant challenge. Several surgical techniques have been reported, but there is no standard procedure. We herein report a novel operative technique for the quadricuspid truncal valve with axisymmetric morphology.
CASE PRESENTATION: A 17-day-old neonate with severe truncal valve insufficiency developed persistent congestive heart failure and underwent urgent truncal valve plasty. The truncal valve was quadricuspid with four equal sinuses, and all four cusps were hypoplastic, resulting in a complete lack of coaptation. We performed bicuspi

A 49-year-old male with a history of left ventricular systolic function dilated cardiomyopathy and severe symptomatic bicuspid aortic stenosis recently diagnosed (syncope, chest pain and dyspnea) was admitted to the coronary care unit. During hospitalization, he developed cardiogenic shock requiring double inotropic support. High surgical risk and an elevated chance of graft rejection contraindicated surgical replacement or heart transplant. We performed a transcatheter aortic valve replacement with a favorable evolution.

Aortic fibrous strands are considered residual tissue from aortic valve development. Rupture of these strands is an important albeit uncommon cause of aortic regurgitation (AR). The authors describe a 67-year-old man who was admitted to the authors' hospital with sudden onset shortness of breath and diagnosed with severe AR. The patient was scheduled for Bentall surgery. The transesophageal echocardiogram (TEE) found multiple fibrous strands that were present in multiple locations of the aortic valve, some of which were ruptured. Ruptured fibrous strands are in the differential diagnosis in pa

UNLABELLED: We describe a 58-year-old Caucasian male weightlifter who presented with acute shortness of breath after finishing his extensive exercise routine. Acute aortic valve regurgitation, due to spontaneous rupture of a bicuspid aortic valve, was diagnosed. Urgent surgical intervention was carried out, during which the bicuspid aortic valve was resected and replaced with an On-X bileaflet mechanical valve. The patient remains asymptomatic and is treated with warfarin, being in excellent physical condition 4 years after aortic valve replacement.
LEARNING POINTS: Spontaneous rupture of a b

BACKGROUND: Bicuspid aortic valve (BAV) is associated with an asymmetric (not circular) aortic root, resulting in variability in the aortic root diameter measurements obtained using different techniques. The objective of this study was to describe aortic root asymmetry, including its orientation in the thorax, in relation to the various phenotypes of BAV and its impact on aortic root diameter measurements obtained using transthoracic echocardiography.
METHODS: Aortic root asymmetry, orientation of the largest root diameter, and orientation of the valve opening were studied using computed tomo

OBJECTIVE: To determine the long-term survival and rate of reoperation after surgical treatment of infective endocarditis (IE) in patients with a bicuspid aortic valve (BAV) and patients with a tricuspid aortic valve (TAV).
METHODS: Between 1997 and 2017, 210 patients underwent surgical treatment for native aortic valve endocarditis, including 51 patients with BAV (24%) and 159 patients with TAV (76%). Data were obtained from the Society of Thoracic Surgeons data warehouse and hospital medical record review, supplemented with surveys and national death index data for more complete follow-up.

OBJECTIVE: Aortic size-based criteria are of limited value in the prediction of aortic events, while most aortic events occur in patients with proximal aortic diameters < 50 mm. Serological biomarkers and especially circulating microRNAs (miRNAs) have been proposed as an elegant tool to improve risk stratification in patients with different aortopathies. Therefore, we aimed to evaluate the levels of circulating miRNAs in a surgical cohort of patients presenting with bicuspid aortic valve disease and distinct valvulo-aortic phenotypes.
METHODS: We prospectively examined a consecutive cohort of

BACKGROUND: Valve-related hemodynamics and intrinsically regulated matrix proteases are 2 determined pathogenetic factors associated with medial elastin degeneration in bicuspid aortopathy. This study analyzed the association between elastic fiber deterioration and the 2 pathogenetic factors in ascending and root morphotypes, aiming to elucidate the etiological heterogeneity between the 2 morphotypes.
METHODS: Four-dimensional flow cardiac magnetic resonance was used to measure the regional wall shear stress (WSS) on the ascending aorta, and matrix metalloproteinase (MMP) expression was asses
