Mirizzi syndrome type IV associated with cholecystocolic fistula: a very rare condition--report of a case.

BACKGROUND: Mirizzi syndrome is a rare complication of prolonged cholelithiasis with presence of large, impacted gallstone into the Hartman's pouch, causing chronic extrinsic compression of common bile duct (CBD). Fistula formation between the CBD and the gallbladder may represent an outcome of that condition. According to Mirizzi's classification and Csendes's subclassification, Mirizzi syndrome type IV represents the most uncommon type (4%). Spontaneous biliary-enteric fistulas have also been rarely reported (1.2-5%) in a large series of cholecystectomies. Cholecystocolic fistula is the most

A rare case of biliary leakage after laparoscopic cholecystectomy-diagnostic evaluation and nonsurgical treatment: a case report.

Although laparoscopic cholecystectomy has become a gold standard in treatment of symptomatic cholelithiasis, it is associated with higher risk of intraoperative lesions and primarily lesions of biliary ducts. In small percentage of cases biliary fistulas occur, most commonly after leakage from cystic duct stump or accessory bile ducts - Luschka's duct. We report of a patient who had episodes of abdominal pain following routine laparoscopic cholecystectomy for acute calculous gallbladder. Results of conducted diagnostics verify the presence of biliary fistula caused by obstruction of bile pathw…

Endoscopic retrograde cholangiopancreatography in ruptured liver hydatid cyst.

One of the most common and serious complications of hepatic hydatid cyst disease is communication between the cyst and the biliary tree. Surgical management of biliary fistula is associated with high morbidity and mortality. We retrospectively reviewed the effectiveness of endoscopic treatment of ruptured hydatid cyst into intrahepatic bile ducts. Diagnosis of intrabiliary rupture of hydatid cyst was mostly suspected by acute cholangitis, jaundice, pain, and/or persistent external biliary fistula after surgery. The diagnosis was confirmed by radiology and endoscopic retrograde cholangiopancrea…

[Is spontaneous, internal bilio-digestive fistula a surgical indication?].

Spontaneous enterobiliary fistulas are recognized preoperatively in only about 50 per cent of patients. Ultrasonography and intravenous cholangiogram merely demonstrate alterations of cholelithiasis. In plain abdominal radiograph biliary gas is presumptive evidence of enterobiliary fistula formation. Barium studies or ERC often succeed in outlining fistula preoperatively. Because of additional findings in more than 70 per cent of cases, such as calculous biliary disease, carcinoma of gallbladder and ascending cholangitis surgical repair of enterobiliary fistula always should be performed. This…

[A case of cholecysto-gastro-colonic fistula with upper gastrointestinal bleeding].

Biliary enteric fistula is an abnormal pathway often caused by biliary disease. It is difficult to diagnose the disease because patients have nonspecific symptoms. A 67-year-old woman presented with hematemesis and melena. She was diagnosed with Dieulafoy lesion on the gastric antrum and underwent endoscopic hemostasis using hemoclips. Follow-up upper gastrointestinal endoscopy revealed an abnormal opening on a previous treated site that was suggestive of biliary enteric fistula. Abdomen simple X-ray and abdominal dynamic CT scan showed pneumobilia and cholecysto-gastric fistula. The patient h

Posttransplant bilioportal fistula with portal vein thrombosis: a case report.

An 8-year-old female patient, known to have post-Kasai biliary atresia with mild intrapulmonary shunting, underwent living donor liver transplantation because of recurrent cholangitis. After the treatment of postoperative biliary stricture with percutaneous transhepatic biliary drainage, the patient subsequently developed hematochezia with portal vein thrombosis. The intraoperative findings showed portal vein thrombosis with a bilioportal fistula. We performed closure of the bilioportal fistula and reconstruction of the portal vein with a native internal jugular vein interposition graft. A bil

Extrahepatic Bile Duct Injury Caused by Blunt Abdomen Trauma: A Case Report.

Traumatic injuries to the extra-hepatic biliary tract are uncommon and may be suspected intraoperatively by the presence of bile-stained fluid in the subhepatic area. We present a case of injury to the common bile duct in a polytrauma patient. The initial CT scan did not suggest biliary injury. However, intraoperatively, bile-stained fluid in the subhepatic space raised suspicion of bile duct injury. This was confirmed postoperatively in the development of a biliary fistula after the primary laparotomy. The patient was treated by endoscopic biliary stenting with complete resolution of the fist…

Endoscopic management of liver abscesses and cysts that communicate with intrahepatic bile ducts.

BACKGROUND AND STUDY AIMS: The formation of a communication between liver abscesses or cysts and intrahepatic bile ducts is an uncommon cause of significant bile leak. Surgical management of biliary fistulas is associated with high morbidity and mortality. We performed a prospective study of endoscopic management of this type of biliary fistula. PATIENTS AND METHODS: We studied 26 patients who had either liver abscesses or hepatic cysts that had ruptured into the intrahepatic bile ducts. The presence of a biliary fistula was suspected by jaundice and/or by the appearance of bile in percutaneo…

[Hepatic hydatid cyst with destruction of the biliary confluent. Apropos of 2 cases treated by direct reconstruction].

The authors present two cases of hepatic echinococcus cyst with destruction of the biliary confluent. The severity of this exceptional lesion requires uncommon treatment. Experience with treatment of biliary fistulas when there is a residual cystic cavity, provides arguments for and against primary suture of these fistulas. In these cases, the required treatment consists in reconstruction of the hepatic biliary ducts destroyed (right, left and common) by using the pericystic envelope itself as a plastic neoformative element. The result has demonstrated its efficacy after many years of use, as …

Gallbladder papillomatosis and cholecystocolonic fistula: a rare combination.

BACKGROUND: Biliary papillomatosis (BP) with sole involvement of the gall bladder or gall bladder papillomatosis (GBP) is very rare. Biliary-enteric fistula, particularly the cholecystocolonic fistula (CCF) type, is also very rare. The combination of both types of lesions in a single patient has never previously been reported in the English literature. CASE REPORT: We report herein the case of an 81-year-old woman who was diagnosed with both disease entities, which occurred in a cause-and-effect relationship. She underwent resection of the gall bladder with closure of the fistula, and was dis

[Internal fistulas of bile ducts caused by cholelithiasis].

Operative treatment of 41 patients is analysed. It is shown that internal fistulas of the bile tract are severe complications of cholelithiasis. Late detection and treatment of diseases of the biliary system is the main cause of formation of the fistulas. Removal of the internal biliary fistula as well as correction of abnormal changes in the bile ducts is the object of the operative intervention. The postoperative fatality was 12.2%.

Colonic gallstone ileus.

We present a rare case of colonic gallstone obstruction in a patient with cholecystocolic fistula. Ultrasound was able to detect the Rigler triad whereas plain film initially only showed ileus. Contrast enema and ERCP failed to show a biliary-enteric fistula. Only the combination of colonoscopy and contrast fistulography evidenced the cholecystocolic connection. The difficulties in establishing the correct radiologic diagnosis of gallstone ileus and in visualizing the biliary-enteric fistula are discussed.

Development of a duodenal gallstone ileus with gastric outlet obstruction (Bouveret syndrome) four months after successful treatment of symptomatic gallstone disease with cholecystitis and cholangitis: a case report.

INTRODUCTION: Cases of gallstone ileus account for 1% to 4% of all instances of mechanical bowel obstruction. The majority of obstructing gallstones are located in the terminal ileum. Less than 10% of impacted gallstones are located in the duodenum. A gastric outlet obstruction secondary to a gallstone ileus is known as Bouveret syndrome. Gallstones usually enter the bowel through a biliary enteral fistula. Little is known about the formation of such fistulae in the course of gallstone disease. CASE PRESENTATION: We report the case of a 72-year-old Caucasian woman born in Germany with a gastr

Cholecystocolic fistula secondary to gallbladder carcinoma: a rare case.

Internal biliary fistulae are a well-recognized complication of biliary lithiasis. Among these, the cholecystoduodenal fistulae are the commonest while cholecystocolic fistulae (CCF) occur much less frequently. CCF secondary to gallbladder carcinoma is a rare occurrence and has been reported in very few studies. Here, the author reports a case of cholecystocolic fistula secondary to gallbladder carcinoma. Preoperative diagnosis of this condition requires high index of suspicion and is usually difficult. Computed tomography scan is helpful in establishing a preoperative diagnosis.

Can occult cystobiliary fistulas in hepatic hydatid disease be predicted before surgery?

BACKGROUND: Biliary fistulas because of the cystobiliary communication is the most frequent and undesirable postoperative complication of hepatic hydatid surgery. We aimed to identify the predicting factors of the occult cystobiliary communication in this study. METHODS: The patients who underwent surgical treatment for hepatic hydatid disease between 2003 and 2008 were reviewed retrospectively. The patients who had jaundice history, preoperative high total bilirubin and direct bilirubin levels, dilated bile duct in preoperative radiologic imagings were not included the study. Patients were d…

Cholecystocolic fistula: an unusual presentation and diagnosis by endoscopic retrograde cholangiopancreatography.

This report describes a patient with a cholecystocolic fistula whose presentation was unusual because it lacked the signs and symptoms that suggest biliary disease (abdominal pain, food intolerance, and belching) and because the fistula was not visualized on barium enema but was apparent on endoscopic retrograde cholangiopancreatography after incidental pneumobilia discovered on ultrasound directed our attention to the biliary tree. A previous Billroth II with vagotomy may have predisposed to the development of the fistula.
