Background: Obstructive sleep apnea (OSA) pathogenesis, clinical manifestations, and consequences vary markedly among patients. Yet OSA heterogeneity goes largely unaddressed in current management pathways, which are characterized by high treatment failure rates of ∼50%. Growing knowledge of OSA pathogenesis has stimulated new lines of investigation for therapies targeted to specific underlying mechanisms or "endotypes." OSA endotypes include the primary anatomic predisposition to pharyngeal collapse, as well as "nonanatomic" mechanisms of low arousal threshold, unstable control of breathing (

Bronchiectasis, particularly in children, is an increasingly recognised yet neglected chronic lung disorder affecting individuals in both low-to-middle and high-income countries. It has a high disease burden and there is substantial inequity within and between settings. Furthermore, compared with other chronic lung diseases, considerably fewer resources are available for children with bronchiectasis. The need to prevent bronchiectasis and to reduce its burden also synchronously aligns with its high prevalence and economic costs to health services and society. Like many chronic lung diseases, b

BACKGROUND: Uganda experiences a high disease burden of malaria, infectious and non-communicable diseases. Recent data shows that malaria is the leading cause of morbidity and mortality among all age groups, while HIV prevalence is on the increase and there is re-emergence of viral haemorrhagic fevers and cholera epidemics. In order to respond to the above situation, a team of researchers, policy makers, civil society and the media was formed in order to build a collaboration that would help in discussing appropriate strategies to mitigate the high disease burden in Uganda.
METHODS: A prepara

Over the past 50 years, we have seen dramatic changes in cardiovascular science and clinical care, accompanied by marked declines in the morbidity and mortality. Nonetheless, cardiovascular disease remains the leading cause of death and disability in the world, and its nature is changing as Americans become older, fatter, and ethnically more diverse. Instead of young or middle-aged men with ST-segment elevation myocardial infarction, the "typical" cardiac patient now presents with acute coronary syndrome or with complications related to chronic hypertension or ischemic heart disease, including

Ensuring equitable access to research funding is crucial for fostering diversity, innovation and excellence in science. Despite progress, significant disparities remain, with underrepresented researchers-including women, racial and ethnic minorities, LGBTQIA+ individuals and those with disabilities-continuing to receive disproportionately less funding. These disparities not only hinder individual careers but also limit the breadth of perspectives that drive scientific discovery. Through discussions with major funding agencies, including the Dana Foundation, European Research Council (ERC) and

The history of human subjects research and the abuses that led to the creation of the United States' (US) regulatory schema for research, which includes institutional review boards (IRBs), is long and complex. However, when one understands the history, one can see direct links between those abuses and the regulations, as well as the ethical principles, contained in the Belmont Report. To that end, we provide a brief analysis of the history of human subjects research and its abuses, including a lesser known case (the atrocities committed by Nazi scientists and physicians during World War II) an

This Review underscores qualitative research as an indispensable facet of scientific inquiry to improve provision of safe, effective, and affordable care for patients with cancer worldwide. Representing a global working group of applied qualitative researchers, our multidisciplinary authorship team defines qualitative research, characterises its purpose and value, and describes specific ways in which qualitative inquiry is central to advancing cures, treatment, and quality of life in oncology. In this Review, we put forward an imperative for the use of qualitative methods to assess context, un

Open science initiatives, intended to democratize access to research products, have made steady progress in shifting the global science culture toward practices like preregistration and data sharing. However, current open science efforts have not yet addressed the special obligation to ensure that research results are made accessible to the portion of the general population who contribute most directly to scientific advances: research participants. In this article, we explore the ethical obligation to communicate aggregate results to research participants and consider elements of open science

In Australia, research priority setting (RPS) may be misclassified as research requiring ethics approval, despite its purpose as a collaborative, pre-research involvement activity. Treating RPS as research creates delays, diverts resources and undermines partnership principles central to inclusive, community-centred priority setting. Recent revisions to the National Statement on Ethical Conduct in Human Research clarify that pre-research consultation does not require ethics review, creating an opportunity for consistent national practice. We call for institutions to recognise RPS as involvemen

BACKGROUND: Treatment mechanisms of force-based manipulation (manual therapies) aim to explain not just "whether" the treatment works but "why" and "how" a treatment works. To date, a majority of studies have failed to address whether treatment mechanisms are causal, signifying that they are the steps or processes through which a manual therapy treatment leads to measurable changes in clinical outcomes. The objectives of this commentary were: 1) to outline various research designs capable of investigating causal treatment mechanisms, and 2) to illustrate each design type with examples drawn fr

Inadequacies in the conduct and quality of research are well established across many research domains, including sport science and medicine. Metaresearch-the practice of performing research on research-is presented as a practical vehicle for improving research quality through evaluating the research processes. This article introduces the concept of metaresearch to sport as a new sub-field of sport science. The broad types of metaresearch are introduced, with a mapping of current sports metaresearch activity across these areas. Interdisciplinary centres aimed at improving scientific quality acr

Ethics in clinical research are indispensable for participant protection and ensuring research quality, and constitute essential knowledge for all healthcare professionals involved in clinical practice. Learning from historical case studies of unethical research, such as the Nazi human experiments and the Tuskegee Syphilis Study has led to progressive development of ethical guidelines like the Nuremberg Code, the Declaration of Helsinki, and the Belmont Report-which presents the three core principles of respect for persons, beneficence, and justice. In Japan, incidents including the Diovan cas

This article introduces the Journal of Speech, Language, and Hearing Research Special Issue: Selected Papers From the 2022 Apraxia Kids Research Symposium. The field of childhood apraxia of speech (CAS) has developed significantly in the past 15 years, with key improvements in understanding of basic biology including genetics, neuroscience, and computational modelling; development of diagnostic tools and methods; diversity of evidence-based interventions with increasingly rigorous experimental designs; and understanding of impacts beyond impairment-level measures. Papers in this special issue

BACKGROUND: Millennium Development Goal 4 (MDGs) mobilised countries to reduce child mortality by two thirds the 1990 rate in 2015. While India did not reach MDG 4, it considerably reduced child mortality in the MDG-era. Efficient and targeted interventions and adequate monitoring are necessary to further progress in improvements to child health. Looking forward to the Sustainable Development Goal (SDG)-era, the Indian Council of Medical Research and The INCLEN Trust International conducted a national research priority setting exercise for maternal, child, newborn health, and maternal and chil
