BACKGROUND: Blast injury has been the most common cause of morbidity and mortality encountered by UK forces during recent conflicts. Injuries sustained by blast are categorised by the injuring component of the explosion and depend upon physical surroundings. Previous work has established that head injuries and intra cavity haemorrhage are the major causes of death following exposure to under body (mounted) blast but has yet to explore the precise nature of these torso injuries nor the effect of particular injuries upon survival. This study examines the patterns of torso injury within the mount

During the Iraq and Afghanistan wars, blasts were the most common cause of combat injuries, including traumatic brain injury (TBI). Prior to 2007, service members were not systematically screened for TBI, and estimates suggest that tens of thousands of mild TBIs went undiagnosed. This study sought to understand post-acute "injury careers," documenting the life- and health-related narratives of veterans who were at high risk of undocumented TBI due to being blast-exposed before 2007. Researchers conducted 38 in-depth interviews between May 2013 and August 2014 with Army veterans who served in c

Little is known about the effects of blast exposure on the human brain in the absence of head impact. Clinical reports, experimental animal studies, and computational modeling of blast exposure have suggested effects on the cerebellum and brainstem. In US military personnel with isolated, primary blast-related 'mild' traumatic brain injury and no other known insult, we found diffusion tensor MRI abnormalities consistent with cerebellar white matter injury in 3 of 4 subjects. No abnormalities in other brain regions were detected. These findings add to the evidence supporting the hypothesis that

Objective: Current treatments for blast-induced lung injury are limited to supportive procedures including mechanical ventilation. The study aimed to investigate the role of post-trauma-induced oedema generation in the function of time and trauma intensity and the probable role of beta 2-adrenergic receptors (β2-ARs) agonists on pulmonary oedema. The study is conducted using an ex vivo model after an experimental in vivo blast-induced thorax trauma in rats. Methods: Rats were randomised and divided into two groups, blast and sham. The blast group were anaesthetised and exposed to the blast wav

We investigated whether pharmacological increase of "M-type" (KCNQ, Kv7) K + channel currents by the M-channel opener, retigabine (RTG), acutely after repetitive traumatic brain injuries (rTBIs), prevents or reduces their long-term detrimental effects. rTBIs were studied using a blast shock air wave mouse model. Animals were monitored by video and electroencephalogram (EEG) records for nine months after the last injury to assess the occurrence of post-traumatic seizures (PTS), post-traumatic epilepsy (PTE), sleep-wake cycle architecture alterations, and the power of the EEG signals. CASE: Blast injuries to the upper extremity can be devastating and emotionally stressful injuries. We describe a case of a high-energy blast injury to an upper extremity from an explosive. The transfer of energy caused severe soft tissue/bony damage to the hand, but also led to associated Essex Lopresti and terrible triad injuries. The patient required emergent transradial amputation by hand surgery as well as definitive fixation by our orthopaedic team.
CONCLUSION: We describe a unique salvage operation that established forearm pronosupination, elbow flexion, and proper prosthetic fitting. W…

On January 31, 2018, a 21-year-old male patient with extremely severe burn-blast combined injury complicated with acute kidney injury caused by dust explosion of crospovidone was admitted to the Second Affiliated Hospital of Zhejiang University School of Medicine. The following nursing measures were carried out, aiming at the condition of the patient. We strengthened the management of fluid intake and output volume, prevented hypothermia and unplanned disembarkation during the period of continuous renal replacement therapy with burn shock and acute kidney injury. For traumatic pneumothorax, we

Abstract Explosive blast-induced traumatic brain injury (TBI) is the signature insult in modern combat casualty care and has been linked to post-traumatic stress disorder, memory loss, and chronic traumatic encephalopathy. In this article we report on blast-induced mild TBI (mTBI) characterized by fiber-tract degeneration and axonal injury revealed by cupric silver staining in adult male rats after head-only exposure to 35 psi in a helium-driven shock tube with head restraint. We now explore pathways of secondary injury and repair using biochemical/molecular strategies. Injury produced ∼25% mo

Diffusion tensor imaging (DTI) has emerged as a promising neuroimaging tool for detecting blast-induced mild traumatic brain injury (bmTBI). However, lack of refined acute-phase monitoring and reliable imaging biomarkers hindered its clinical application in early diagnosis of bmTBI, leading to potential long-term disability of patients. In this study, we used DTI in a rat model of bmTBI generated by exposing to single lateral blast waves (151.16 and 349.75 kPa, lasting 47.48 ms) released in a confined bioshock tube, to investigate whole-brain DTI changes at 1, 3, and 7 days after injury. The neuropathology of mild traumatic brain injury in humans resulting from exposure to explosive blast is poorly understood as this condition is rarely fatal. A large animal model may better reflect the injury patterns in humans. We investigated the effect of explosive blasts on the constrained head minimizing the effects of whole head motion. Anesthetized Yucatan minipigs, with body and head restrained, were placed in a 3-walled test structure and exposed to 1, 2, or 3 explosive blast shock waves of the same intensity. Axonal injury was studied 3 weeks to 8 months postblast using β-amyloid pr

Blast injuries to the perineum have become prevalent on battlefields with the use of improvized explosive devices. Such injuries were commonly encountered during the height of the internal conflict in Sri Lanka from 2007 to 2009. These were associated with unique complications as well as challenges in their management. We present a case of a complex perineal injury which was initially reconstructed with a Gracilis neosphincter, presenting a decade later as a giant implantation dermoid incorporating the rectum.

BACKGROUND: High-energy blast injuries produce severe anatomical distortion that may compromise ultrasound-guided peripheral nerve blocks (PNBs). However, large-scale data identifying predictors of block failure in this population remain limited.
OBJECTIVE: To determine the incidence and independent predictors of PNB failure in patients with blast-related limb trauma.
METHODS: Prospective observational cohort study conducted between March 2023 and February 2026 at a tertiary war trauma center. Adult patients undergoing ultrasound-guided PNB for blast-related limb injuries were included. Prim…

Blast injuries produce complex multi-system trauma through primary barotrauma, secondary penetrating projectiles, tertiary displacement forces, and quaternary mechanisms. Among the most challenging complications is the concurrent presentation of traumatic intracranial hemorrhage and cerebral venous sinus thrombosis (CVST), which creates a profound therapeutic dilemma regarding anticoagulation management. We present a 26-year-old male who sustained injuries from a high-order explosive device that resulted in 19 fatalities. He presented 16 h post-injury with a Glasgow Coma Scale score of 10/15 a…

This cross-sectional study investigated the factors influencing injury characteristics and outcomes following the 2020 Beirut Blast. Blast victims (n = 310) were identified from hospital registries in Beirut. Soft tissue injuries predominated (N = 276; 88.2%), with most patients struck by objects (N = 261, 84.2%). Minor injuries were common (70.3%), with significant associations noted for eye and internal injuries. Victims were primarily situated 500-1700 m from the blast epicentre (71%), indoors (81%), often in residential dwellings (48%), near windows. Severe injuries, especially musculoskel

Traumatic brain injury (TBI) is the leading environmental risk factor for neurodegenerative diseases, yet its molecular link to chronic neurodegeneration is unclear. While animal models of TBI are commonly used, emerging research suggests that induced pluripotent stem cell (iPSC)-derived brain organoids offer a promising human-specific alternative, particularly for studying processes like cryptic exon splicing. However, widespread use has been limited by methodological variability and the need for expensive and specialized equipment. To address these challenges, we developed a tabletop blast d
