The use of flexible intramedullary nails in limb lengthening.

Lower and upper limb length discrepancy and deformity, congenital or acquired, are amongst the most common conditions in children for orthopaedic referral. Over the last twenty years, surgical techniques have evolved in an attempt to decrease Healing Index and minimise these complications. Areas covered: The flexible intramedullary nailing (FIN) is a minimally invasive intramedullary osteosynthesis. In combination with an external fixator, it is appropriated for pediatric bone lengthenings and lengthening of bones of small diameter in adults. In the study the Scopus and/or PubMed indexed publi

[Pédiatrie - Internal lengthening nails: a novel method of lower limb reconstruction in children].

Intramedullary lengthening nails for lower limb reconstruction in children are a rapidly growing field and represent an alternative to external fixators. Today, two types of intramedullary lengthening nails are used: motorized (FITBONE) and magnetically driven (PRECISE) bone lengthening nails. Indications for limb lengthening nails are: 1) Significant lower limb length discrepancy; 2) Short stature and deformities due to musculoskeletal or dysplastic syndromes; 3) Constitutional short stature. Intramedullary lengthening surgery is challenging and represents a safe and reliable mode of treatmen

Compression forces of internal and external ankle fixation devices with simulated bone resorption.

BACKGROUND: Internal and external fixation techniques have been developed to provide rigidity and stability to a fusion site such as in tibiotalocalcaneal (TTC) arthrodesis. Compression of the fusion site plays an integral role in primary bone healing, though little work has been done to quantify the compressive force values of ankle fixation devices. MATERIALS AND METHODS: Using synthetic and cadaveric bone models, a Newdeal/Integra PantaNail and DePuy VersaNail were tested as compressive intramedullary (IM) nails while an Encore True/Lok and an Ace-Fischer frame were tested as external fixa

When Nail Removal Is Not Benign: Iatrogenic Fractures Associated With Titanium Intramedullary Nail Removal.

Intramedullary (IM) nailing is a standard treatment for diaphyseal fractures of long bones. Several factors influence bony ingrowth and osseointegration between the nail and bone. Removal of these nails may be required, especially in younger patients, for multiple indications. Bony ingrowth and osseointegration can cause resistance and complicate nail removal. We report a series of three patients who had bony ingrowth and osseointegration within the nail and proximal metaphyseal bone, which complicated routine nail removal. One patient sustained an intertrochanteric femur fracture during the e

[Internal circular arc osteosynthesis of tibiotalocalcaneal arthrodesis].

Tibiotalocalcaneal arthrodesis has recently become more popular as a form of reconstructive surgery. The precise anatomical orientation and the functional extrinsic musculature of the hindfoot are essential for a satisfactory result. Fixation of the arthrodesis is a mechanical problem. Straight and angulated nails are not anatomically or mechanically ideal. A circular arc nail can fix the tibia, the talus and the calcaneus in anatomical alignment. This is a pure "bone nail", in contrast to the "intramedullary nail," which is driven through an existing opening in long bones. The nail is driven

Treatment of aseptic non-union after intramedullary nailing without removal of the nail.

Failure of bone healing after intramedullary nailing of a diaphyseal long bone fracture is a severe complication that requires an effective management to ensure the best chances for successful bone-union and termination of a long period of incapacity and morbidity for the sufferers. Traditional procedures require removal of the existing nail and re-fixation with wider nail, plate or external fixation constructs. The concept that bone union can be obtained with the existing nail in situ is gaining popularity as its removal adds trauma and potential complications and prolongs the operating time.

Reconstruction of Bony Defects With Motorized Intramedullary Nails After Tumor Resection.

Motorized intramedullary lengthening nails allow for transport of a bone segment for limb lengthening, deformity correction, healing of nonunion, and intercalary distraction osteogenesis. Resection of tumors involving the bone can result in substantial defects that require reconstruction. Use of these nails allows for a biologic reconstruction with the incorporation of allograft or by distraction osteogenesis. Limb lengthening after an internal hemipelvectomy where the hip joint is resected can be performed to improve gait, decrease pain, and prevent the need for a custom shoe or shoe lift. Us

[New principles of using biodegradable implants in the treatment of fractures].

So far the operative treatment of bone fractures has resolved itself to immobilisation and bone fragment joining with the help of metal implants such as nails, screws, plates and wires. Once the bone had healed, they had to be removed. This action bears some risk, e.g. extremity disfunction, wound infection. This requires re-hospitalization and increases social cost of the treatment. These reasons gave rise to research on new, biodegradable materials. One of them is "Biofix" manufactures as threads or nails, employed when immobilising bone fragments. Application of "Biofix" allows early rehabi

Limb lengthening and deformity correction in children with abnormal bone.

Flexible intramedullary nailing (FIN) provides multiple advantages in limb lengthening and progressive deformity correction in combination with external fixation. The article presents brief literature review and authors' experience in limb lengthening of abnormal bone (Ollier's disease, fibrous dysplasia, osteogenesis imperfecta). Titanium and, especially, hydroxyapatite-coated bent elastic nails in combination with external fixator are appropriate in limb lengthening of abnormal bone in children. FIN left in situ after lengthening procedure and external frame removal should be applied for lon

[Experiences with different telescope nails in treatment of pediatric osteogenesis imperfecta].

OBJECTIVE: The present study aims to examine and compare results after intramedullary rodding of long bones with the original Bailey/Dubow-nail and two modifications. PATIENTS AND METHODS: During the period 1985 to 1995 21 patients with an average age of 6.3 years (2-11 yrs) underwent intramedullary rodding of 83 long bones. Therefore 38 original Bailey/Dubow-nails, 36 nails with distal screws and 9 nails with distal K-wires were used. The mean observation period amounted to 5.7 years after insertion. Operation success was judged by bone growth, rod elongation, complications, implantation per

Elastic stable intramedullary nailing (ESIN) in paediatric femur and tibia shaft fractures: Comparison between titanium and stainless steel nails.

INTRODUCTION: Elastic stable intramedullary nailing (ESIN) is currently considered the gold standard in surgical treatment of femur and tibial shaft fractures in school age paediatric patients. Although elastic intramedullary nails are available in both titanium (Ti) and stainless steel (SS) alloy, titanium nails are most commonly used. Nevertheless, there is still contrasting evidence as to whether the use of Ti nails can offer better outcomes in terms of fracture healing and stability over SS nails. The aim of this study was to compare outcomes and complications of Ti and SS ESIN for femur a

Treatment of Proximal Femur Osteomyelitis Occurred after Proximal Femoral Nail Antirotation Fixation, with Antibiotic Cement-coated Tibia Intramedullary Nail: A Case Report.

Antibiotic cement-coated intramedullary nails maintain a locally high antibiotic concentration while contributing to bone stability. We present a case of femoral subtrochanteric fracture in a patient with an infected nonunion who was successfully treated for an infection and nonunion using an antibiotic cement-coated tibial intramedullary nail. A 79-year-old woman with a right femoral subtrochanteric fracture underwent internal fixation using proximal femoral nail antirotation (PFNA). She developed osteomyelitis with nonunion at the surgical site 10 months postoperatively. We decided to insert

Heterotopic ossification around the hip with intramedullary nailing of the femur.

Heterotopic ossification (HO) at the proximal end of a reamed intramedullary femoral nail is a complication of the procedure. Kuntscher stated that these "callus caps" were due to the prominence of the nail above the greater trochanter. A study of patients undergoing locked intramedullary nailing of the femur to assess subsequent development of heterotopic ossification at the proximal end of the implant was done. The purpose was to study the clinical significance of the ectopic bone and identify etiologic factors of bone formation. This group consisted of 59 patients with 60 locked intramedull

The Poller Screw Technique: A Method of Fine-Tuning the Reduction in Locked Nailing.

Intramedullary nailing is the treatment of choice for diaphyseal fractures in long bones. However, nailing of long bone fractures at the metaphyseodiaphyseal junction is technically difficult and can cause malalignment because of the mismatch in the diameter of the bone. One of the most common and recently described methods of correcting deformity during nailing is the poller screw technique. We describe a modified technique to correct malreduced fractures with the nail in situ, which we have used successfully in 3 patients.

Biomechanical Consequences of Nail Insertion Point and Anterior Cortical Perforation for Antegrade Femoral Nailing.

This biomechanical study assessed the influence of changing antegrade cephalomedullary nail insertion point from anterior to neutral to posterior locations relative to the tip of the greater trochanter with or without anterior cortical perforation in the distal femur. Artificial osteoporotic femurs and cephalomedullary nails were used to create 5 test groups each with 8 specimens: intact femur without a nail or perforation, anterior nail insertion point without perforation, neutral nail insertion point without perforation, posterior nail insertion point without perforation, and posterior nail

Commercially available antibiotic-laden PMMA-covered locking nails for the treatment of fracture-related infections - A retrospective case analysis of 10 cases.

Introduction: Fracture-related infections (FRIs) are a devastating complication. FRIs are challenging and should be addressed with a multidisciplinary approach. An FRI should be addressed surgically by non-viable bone debridement, local antibiotic deposition, minimization of dead space and fracture stabilization. Antibiotic-laden PMMA-covered nails are a viable option to face these complications. To demonstrate the safety and utility of commercially available antibiotic-laden PMMA-covered nails, we performed a review of the cases operated in our institution and a cost analysis to compare the c

[Systemic complications in intramedullary nailing].

Today intramedullary nailing is the treatment of choice in stabilizing femoral and tibial diaphysial fractures because of its superior bone healing compared to other forms of osteosyntheses. By interlocking, the indication can be extended to all fractures in which interlocking bolts can be fixed in the proximal and distal main fragment. Küntscher's principle of elastic clamp has changed to intramedullary splinting. With that method reaming is limited to a few reaming processes, and unreamed nailing has become possible. Today implants start at a diameter of 9 mm. The diameter of implants of all

Prevention of Staphylococcus aureus biomaterial-associated infections using a polymer-lipid coating containing the antimicrobial peptide OP-145.

The scarcity of current antibiotic-based strategies to prevent biomaterial-associated infections (BAI) and their risk of resistance development prompted us to develop a novel antimicrobial implant-coating to prevent Staphylococcus aureus-induced BAI. We incorporated the antimicrobial peptide OP-145 into a Polymer-Lipid Encapsulation MatriX (PLEX)-coating to obtain high peptide levels for prolonged periods at the implant-tissue interphase. We first confirmed that OP-145 was highly effective in killing S. aureus and inhibiting biofilm formation in vitro. OP-145 injected along S. aureus-inoculate
