The neuropsychology of ventral prefrontal cortex: decision-making and reversal learning.

Converging evidence from human lesion, animal lesion, and human functional neuroimaging studies implicates overlapping neural circuitry in ventral prefrontal cortex in decision-making and reversal learning. The ascending 5-HT and dopamine neurotransmitter systems have a modulatory role in both processes. There is accumulating evidence that measures of decision-making and reversal learning may be useful as functional markers of ventral prefrontal cortex integrity in psychiatric and neurological disorders. Whilst existing measures of decision-making may have superior sensitivity, reversal learni

Induced hypothermia in critical care medicine: a review.

BACKGROUND: Clinical trials of induced hypothermia have suggested that this treatment may be beneficial in selected patients with neurologic injury. OBJECTIVES: To review the topic of induced hypothermia as a treatment of patients with neurologic and other disorders. DESIGN: Review article. INTERVENTIONS: None. MAIN RESULTS: Improved outcome was demonstrated in two prospective, randomized, controlled trials in which induced hypothermia (33 degrees C for 12-24 hrs) was used in patients with anoxic brain injury following resuscitation from prehospital cardiac arrest. In addition, prospective

[Clinical application of functional magnetic resonance imaging].

Three types of researches have been carried out on brain-mind relationships: 1. researches on anatomical correlates of special talents (for example, perfect pitch) or deficits (for example, dyslexia), 2. researches to examine the relationship between a given cognitive syndrome and the site of brain damage, 3. researches to localize human cognitive function in the brain in vivo using functional magnetic resonance imaging (fMRI) and positron emission tomography (PET). fMRI is a particularly important because it is noninvasive. A tutorial covering basic aspects of this methodology is presented, a

Acute disseminated encehalomyelitis with typhoid fever.

A 45 year old male came with fever, headache, altered sensorium pallor and lower gastrointestinal bleeding. Laboratory investigations confirmed typhoid fever. Magnetic resonance imaging (MRI) was suggestive of acute disseminated encephalomyelitis.

A historical review of topographical disorientation and its neuroanatomical correlates.

Topographical disorientation (TD) refers to impaired orientation and navigation in real-world environments. Although numerous cases have been reported, disagreement over neuroanatomical correlates remains. This has been contributed to by concern with the "essential defect," ambiguous terminology, and incomplete assessments. Attention to three factors permits a coherent understanding of the widely divergent descriptions of cognitive deficits and neuroanatomical findings: point in course (acute vs. chronic), characteristics of the environment, and a patient's specific cognitive deficits. Defects

Traumatic brain tissue acidosis: experimental and clinical studies.

We have been focusing on potential metabolic derangement associated with severe head injury and a clinical trail directed toward treating brain tissue acidosis is currently underway. More specifically, we based this study on the hypothesis that following brain trauma brain tissue acidosis develops which may contribute to the prolongation of coma and neurologic deficit. Tromethamine (THAM), a safe and low toxicity agent which buffers in major part by causing a hypocapnic alkalosis, was selected for trial. Patients admitted with GCS < 8 were randomized into one of three arms: control: THAM plus

Management of intracranial hypertension in head injury: matching treatment with cause.

Raised intracranial pressure (ICP) is common after head injury and strongly associated with mortality and morbidity. Empirical and prophylactic therapy with steroids and barbiturates has proved unsuccessful. Ideally, therapy should be targeted at the predominant cause of the increase in ICP. In head injury these may be (1) an increase in cerebral blood volume best treated by hyperventilation and hypnotic drugs. (2) an increase in brain water content best treated by osmotherapy and (3) increased CSF outflow resistance best treated by CSF drainage. This last cause seldom predominates in head inj

Birth stress, autoimmune diseases, and handedness.

A large nonclinical sample of left-handed and right-handed subjects completed laterality and medical health questionnaires. The group of left-handed subjects showed a raised incidence of birth complications and learning disorders whereas no differences were found for birth order and maternal age. Left-handed and right-handed subjects showed similar incidences of autoimmune diseases, allergies, migraine, and stuttering. These results suggest that an association between early pathology and left-handedness may be found in a subset of the nonclinical left-handed population. The underlying patholog

Reptilian behavioural patterns in childhood autism.

Childhood autism may be caused by damage to three phylogenetically distinct regions of the brain, or their major pathways and connections. Injury to the neocortex results in loss of language and cognitive function, while injury to the limbic cortex results in autistic withdrawal and abolition of play behaviour. Injury to the more primitive striatal complex, mammalian counterpart of the brain of reptiles, results in a bizarre and truncated form of stereotyped and ritualistic behaviour. The causes of brain injury in childhood autism could be those common in the perinatal period including cerebra

Brain disorders don't disqualify workers from job.

A variety of neurological disorders afflict people in the workforce and the occupational physician faces tough decisions about them. Sometimes the afflicted individual is an applicant for a job. In other cases, he or she suffers a neurological disorder, ranging from epilepsy to senility, while an employee, often after long and valued service. Whether the decision pertains to hiring or to approval of a return to work, it's usually up to the occupational physician to make it. Often he or she is under strong pressure to make an expedient decision.

Anesthesia for patients with head trauma.

Patients undergoing anesthesia soon after head trauma are at great risk for further neural damage during the anesthetic, especially if the head injury is severe or the anesthetic technique is suboptimal. Secondary complications of the anesthetic that are often lethal include hypoventilation, increases in ICP, airway obstruction, and brain-stem herniation. Anesthetic management of patients with head injury must include intravenous induction with barbiturates or narcotics, smooth endotracheal intubation, controlled ventilation with oxygen, and minimal amounts of inhalational agents. It is import

Does hyperbilirubinemia damage the brain of healthy full-term infants?

In the 1950s, exchange transfusion to keep the total serum bilirubin below 20 mg/dl was shown to be an effective way of preventing kernicterus in babies with erythroblastosis fetalis. For the last 15 to 20 years this level has also been used to determine the need for intervention in healthy full-term infants who do not have hemolytic disease. A critical review of all the available data including six studies from the collaborative perinatal project (more than 30,000 infants) and several smaller studies of term infants without hemolysis reveals essentially no evidence of adverse effects of bilir

Boxing injuries: neurologic, radiologic, and neuropsychologic evaluation.

Boxing is an endeavor that may have to be re-evaluated in the coming years as to whether it should be designated as a sport. It is the only "sport" in which victory is determined by the amount of physical damage done to the opponent. We have presented the largest number of professional and amateur boxers (58) evaluated by various modern diagnostic modalities and have unequivocally demonstrated the deleterious effects of boxing upon the brain. There have been few, if any, meaningful actions taken by the promoters of boxing to correct the conditions under which boxers are subjected to physical a
