Branchial cleft anomalies make up 30% of all pediatric neck masses, but complete second branchial cleft anomalies are extremely rare. We report an unusual case of a complete second branchial cleft anomaly that presented as a draining neck fistula and a tonsillar cyst in an otherwise healthy 3-month-old girl. At the age of 7 months, the patient had been experiencing feeding difficulties, and there was increasing concern about the risk of persistent infections. At that point, the anomaly was excised in its entirety. Our suspicion that the patient had a complete second branchial cleft anomaly was

The elongated stylohyoid process presents with considerable anatomic variability. We report here oil an ancient cranium with bilateral elongated styloid process, 3.3 cm on the right side and 5.1 cm on the left side, found during the examination of excavated bones of 2000 (years ago (first half of 1st century AD) from the old Greek-Roman city-of Leodikya in Turkey. We determined the gender as female, from the examination of the skeleton (especially skull and pelvic bones). No other variation was observed. On the basis of embryology, the reason for this variation may be partial ossification of t

Branchial abnormalities constitute 32% to 45% of all neck pathologies in children. They may be a part of branchiootorenal (BOR) syndrome, which is characterized by branchial arch anomalies, preauricular pits, hearing impairment, and renal malformations. Typically, the management of a branchial fistula does not necessarily require an extensive diagnostic workup. However, in patients with a branchial fistula associated with external ear anomalies on physical examination and/or a history of hearing loss and a similar history and findings in other family members, an additional workup should be per

A thirty-year-old male patient with bilateral second branchial cleft cysts is presented, in whom no association was found with any other congenital abnormality, in particular branchio-oto-renal syndrome. He had a complaint of painless swelling on both sides of the neck. Physical and radiological examinations showed bilateral branchial cleft cysts. A coexisting congenital syndrome could not be documented despite investigations including physical examination, blood biochemistry and complete blood count, audiologic tests, temporal bone computed tomography, and abdominal and renal ultrasonography.

Neck abscess located in or around the thyroid gland should raise the suspicion of acute suppurative thyroiditis, pyriform sinus fistula, a 3rd or 4th branchial cleft anomaly. Differentiating between these entities on a clinical basis is difficult. After treating the initial infection, computed tomography, barium esophagography, ultrasound, and endoscopy can be used in search of the fistulous tract that can be associated with a pyriform sinus fistula or a 3rd or 4th branchial anomaly. We present a case of a pyriform sinus fistula involving the use of oral contrast, combined with computed tomogr

First branchial cleft anomalies are extremely rare and account for less than 8% of all branchial anomalies. They are generally found in infancy although they arise during the embryonal period because of an incomplete closure of the first branchial cleft. In 1972 Work classified first branchial cleft anomalies into two types: Type I, ectodermal cysts, is a duplication of the external auditory duct; Type II, originate in both ectodermal and endodermal components and contain cartilage. The present case report describes a Type II branchial malformation. G.M., a 2-year-old male, presented painful t

First branchial cleft anomalies account for only 1-8% of all branchial anomalies. Several classification systems have been proposed to aid in the diagnosis of these defects. We present a series of 12 patients with first branchial cleft anomalies classified according to anatomic criteria. The preferred treatment is complete excision. For patients with acutely inflamed masses, surgery should be performed only after resolution of the infection has been achieved with adequate drainage in combination with antibiotic therapy.

BACKGROUND: The literature is ambiguous as to the question of the developmental background of systemic-pulmonary collateral arteries. These are found in combination with various congenital heart malformations such as pulmonary atresia. From a clinical point of view, it is of interest to know whether we are dealing with the persistence of transient embryological vessels such as ventral segmental arteries or parts of pharyngeal arch arteries or with the prenatal or postnatal recruitment of the bronchial vasculature that normally supplies the lung. This study of the embryology of the extrapulmona

These are cysts and fistulae which closely adherent to the external auditory canal and drain through the high lateral cervical suprahyoid site. Misdiagnosis is common with these lesions and inadequate removal leads to surgical failure. A review of the embryology of this region is necessary since we intend to explain the clinical aspects and the variable relationship to the facial nerve. Eight cases are described and the literature pertaining to first branchial cleft syndrome is reviewed. Diagnostic and management problems will be discussed. A full exposure of the parotid gland and facial nerve

An unusual combination of first and second arch anomlies is presented. This has resulted in absent middle and external ears with gross naso- and oropharyngeal defects. A brief reference is made to the embryology of this region to explain the anatomical and functional defects. Vascular abnormality during the peri-gestational period is suggested as a possible cause and a conservative line of treatment has been followed with good result.

Cervical congenital cystic masses constitute an uncommon group of lesions usually diagnosed in infancy and childhood. The most common congenital neck mass is the thyroglossal duct cyst. The diagnosis is easily established from the presence of a cystic lesion in the anterior midline portion of the neck. The vast majority of branchial cleft cysts arise from the second branchial cleft. They can occur anywhere from the oropharyngeal tonsillar fossa to the supraclavicular region of the neck. Cystic hygroma is the most common form of lymphangioma. In the neck, cystic hygromas are most commonly found

Branchio-oto-renal (BOR) syndrome is an autosomal dominant disease characterized by varying combinations of branchial, otic and renal anomalies. By positional cloning, a candidate gene, EYA1, homologous to the drosophila eyes absent gene, has recently been identified at 8q13.3 and shown to underlie this syndrome. The name branchio-oto (BO) syndrome has been used to describe a similar combination of branchial and otic anomalies, without the association of renal anomalies. Whether BOR and BO syndromes involve the same gene was unknown. To address this question, we analyzed two large independent

Branchial cleft anomalies are congenital malformations resulting from the abnormal persistence of branchial clefts during embryogenesis, often presenting as cysts, sinuses, or fistulae. These anomalies account for approximately 20% of pediatric head and neck masses, with bilateral cases being particularly rare. This report discusses a case of an 11-year-old boy with complete bilateral second branchial cleft fistulae, characterized by intermittent mucopurulent discharge and recurrent inflammation, primarily following upper respiratory infections. Diagnostic imaging, including CT scans, confirme

Disruption of cardiac neural crest cells (CNCCs) results in congenital heart disease, yet we do not understand the cell fate dynamics as these cells differentiate to vascular smooth muscle cells. Here we performed single-cell RNA-sequencing of NCCs from the pharyngeal apparatus with the heart in control mouse embryos and when Tbx1, the gene for 22q11.2 deletion syndrome, is inactivated. We uncover three dynamic transitions of pharyngeal NCCs expressing Tbx2 and Tbx3 through differentiated CNCCs expressing cardiac transcription factors with smooth muscle genes. These transitions are altered non

Background: Branchial cleft cysts or fistulae are common in pediatric surgical pathology and are cured by surgery. Lesions in this area may not show the classical features of a cyst or duct lined by squamous or respiratory epithelium and other differential diagnoses should be considered. Case report: A seven-year-old otherwise healthy boy presented with bilateral swelling of the lower neck and reported intermittent secretion of clear fluid on the right side. Excision of the right sided lesion revealed an ectopic salivary gland, the excision of the left showed only subtle fibrosis. Conclusion:

BACKGROUND: Asymmetries in craniofacial anomalies are commonly observed. In the facial skeleton, the left side is more commonly and/or severely affected than the right. Such asymmetries complicate treatment options. Mechanisms underlying variation in disease severity between individuals as well as within individuals (asymmetries) are still relatively unknown.
RESULTS: Developmental reductions in fibroblast growth factor 8 (Fgf8) have a dosage dependent effect on jaw size, shape, and symmetry. Further, Fgf8 mutants have directionally asymmetric jaws with the left side being more affected than
