Secretory carcinoma of the breast with a cystically dilated intraductal component: report of a case.

A case of secretory carcinoma of the breast in a 61-year-old woman is described. She came to our hospital complaining of a bloody nipple discharge. The physical examination revealed a 2.8 x 2.1-cm, firm, irregular lump with bloody nipple discharge in her left breast. Mammography demonstrated an irregular mass and ultrasonography showed an irregular hypoechoic mass accompanied with a cyst. Ductgraphy was done. It showed a dilated mammary duct leading to the cyst which was partially occupied with carcinoma. A cytologic smear of the nipple discharge and fine-needle aspiration cytology revealed si

Antibiotic-refractory unilateral breast abscess with contralateral cystic lesion in a pubertal girl: a case report.

Pediatric breast abscesses require timely infection control while minimizing injury to the developing breast. We report an 11-year-old pubertal girl with persistent left breast pain, swelling, and erythema after 7 days of oral antibiotic therapy. Physical examination revealed a fluctuant left breast mass and a tender contralateral cystic lesion. Inflammatory markers were elevated. Ultrasonography demonstrated a heterogeneous left-sided collection with peripheral vascularity, consistent with abscess formation, whereas the right breast showed a well-circumscribed anechoic cyst without inflammato

Cystic carcinoma of the breast: a trap for the unwary.

Cystic breast masses are a common presentation to breast clinics. While the majority of cysts can be managed by simple aspiration, a small proportion are malignant. Histology records for a 10-year period have been examined to identify patients with cystic breast carcinomas. In all, 31 patients were identified. Of these, 18 had cystic degeneration of high-grade tumours, while 13 had intracystic papillary carcinoma. Both of these tumour types were diagnosed by a combination of cyst fluid cytology and breast imaging. The prognosis of high-grade tumours was poor, while that of intracystic papillar

CONSERVATIVE TREATMENT OF BREAST C YSTS: POSSIBILITIES OF USING INDINOLE FORTO

Among the drugs used in the conservative therapy of breast cysts and fibrocystic mastopathy, phytopreparations are mainly used in clinical practice. The drug Indinol Forto, obtained from plants of the cruciferous family, contains indolecarbinol, the effect of which is associated with the regulation of estrogen metabolism, inhibition of pathological proliferation, blockade of the formation of inflammatory cytokines, activation of apoptosis of transformed cells, reduction of breast cysts. The paper presents the results of a study of the pharmacological effect of the drug Indinol Forto on cystic …

Is cystic disease related to breast cancer?

Human breast cystic disease is a common premenopausal benign breast condition. Apocrine metaplasia of normal breast epithelium is the lesion that allows cysts to develop. Apocrine metaplasia and breast cysts occur frequently in association with other proliferative changes in breast epithelium, especially breast epithelial hyperplasia. Clinical follow-up studies of women with breast cystic disease indicate an increased risk of subsequent development of breast carcinoma. This risk is enhanced when multiple cysts occur. A positive family history of breast carcinoma adds to the increased risk that…

Intracystic epidermal growth factor level is predictive of breast-cancer risk in women with gross cystic disease of the breast.

Women affected by gross cystic disease of the breast have an increased risk of breast cancer. We report here the incidence of breast cancer by cyst type and intracystic epidermal growth factor (EGF) concentration. Our retrospective study included 504 women who had at least 1 cyst aspiration between 1985 and 1993. Cyst fluids were processed for electrolyte concentration (n = 378), EGF concentration (n = 347) or both (n = 337). Age-standardized incidence ratios (SIRs) were estimated using the population of the Genoa Cancer Registry. A multivariate Poisson regression model was used to estimate re

Seat belt injuries of the breast: findings on mammography and sonography.

OBJECTIVE: The purpose of this study was to determine the mammographic and sonographic findings of injuries of the breast caused by seat belts in automobile accidents. The imaging findings of such injuries must be differentiated from those of carcinoma, so that inappropriate treatment is avoided. MATERIALS AND METHODS: Five patients who had breast injuries caused by seat belts with shoulder restraints between 1988 and 1993 and who underwent mammography were included in this study. Four of these women had imaging studies done in our department, and one was examined elsewhere. The initial mammo

Cystic breast lesions: sonographic findings and clinical management.

OBJECTIVE: This article reviews basic sonographic findings for distinguishing cystic lesions of the breast. METHODS: We describe sonographic features of simple and complicated cysts in comparison with complex masses and intracystic carcinomas. RESULTS: We correlate cystic lesion appearances with histologic patterns and illustrate the diagnostic and therapeutic management of cystic breast lesions. CONCLUSIONS: Sonography is a useful tool in distinguishing simple cysts from complicated cysts and complex masses of the breast.

Sonographic findings in a patient with cystic hypersecretory duct carcinoma of the breast.

In this report, we describe a case of a rare form of intraductal carcinoma of the breast known as cystic hypersecretory duct carcinoma in a 49-year-old woman with a palpable mass and no history of breast disease. Mammography showed heterogeneous dense breast tissue with no definite abnormally increased density or microcalcifications. Gray-scale sonography detected multiple small aggregated, anechoic cysts with good through-transmission in the upper outer quadrant of the left breast, corresponding to the location of the palpable mass. The patient underwent an excision biopsy, and histopathologi

Cystic disease and fibroadenoma of the breast: natural history and relation to breast cancer risk.

Approximately 25% of all 'discrete' breast lesions are fibroadenomas or breast cysts and they more commonly cause a breast lump than breast cancer. Despite their frequency, their natural history and relationship to subsequent breast cancer have not been clearly defined, although it would appear that palpable breast cysts, but not fibroadenomas, are associated with some increased risk of breast cancer. The diagnosis of these two entities is now possible by fine needle aspiration and excision of these lesions is only indicated in certain circumstances.

Ruptured epidermal inclusion cysts in the subareolar area: sonographic findings in two cases.

Epidermal inclusion cyst of the breast is an uncommon benign lesion and it is usually located in the skin layer. We report here on two cases of ruptured epidermal inclusion cysts in the subareolar area, which is a very unusual location for these cysts and these lesions can be mistaken for breast malignancies.

The treatment of symptomatic benign breast disease with danazol.

A prospective randomized double blind trial, using danazol in a dose of 200 mg twice a day, was carried out on 80 women presenting with severe cyclical symptomatic benign mammary dysplasia. Standardized scores for 4 parameters of disease severity (breast pain, breast tenderness, breast nodularity and breast cysts) were defined and measured monthly on all patients admitted to the trial. Using nonparametric statistical analysis, significant improvement in breast pain, tenderness and nodularity, was found in both the double blind and crossover arms of the trial. An insufficient number of patients…

[A Case of Pure Squamous Cell Carcinoma of the Breast in an Elderly Woman Diagnosed by Cytology].

A n 85-year-old woman presented with a mass in the left breast. A3 7mm lobulated mass including enhancement of a cyst of 37mm was detected by sonography. An axillary lymph node had increased to 16 mm. Atypical cells dyed by light green and orange G were identified by fine needle aspiration and cytology. She was diagnosed with left breast cancer(cT2N1M0, Stage II B), histologically suspected to be squamous cell carcinoma. She underwent a left-breast-conserving surgery and axillary lymph node dissection. On pathology, a cyst of 34×30mm was noted. The tumor grew from inside the cyst to the surrou

Risk of breast cancer subsequent to proven gross cystic disease.

3809 women in whom breast cysts were aspirated were followed up to evaluate the observed/expected ratio of subsequent breast cancer. Breast cancer at cyst aspiration was excluded by physical examination and mammography. The first year of follow-up was censored to avoid a prevalence screening effect. Subsequent breast cancers were found either directly or by means of a cancer registry which also provided the expected age and residence specific incidence rates. The number of expected cancers was assessed in person-years (15,915 in the total series). The observed/expected subsequent breast cancer…

Intracystic lipidic profile in fibrocystic breast disease.

This study was designed to investigate the lipidic profile of fibrocystic breast disease. The study entailed measuring total cholesterol, high-density lipoprotein-cholesterol (HDL-C), low-density lipoprotein-cholesterol (LDL-C), very low-density lipoprotein-cholesterol (VLDL-C) and triglyceride levels in the serum of 50 women with fibrocystic breast disease without macrocysts, and in the serum and breast cyst fluid of 60 women with fibrocystic breast disease and macrocysts. The relationships between the lipids analyzed in the cyst fluid and intracystic estradiol, progesterone, dehydroepiandros…

Cyst fluid proteases.

The precise origin of breast cyst fluid remains obscure. Molina has presented evidence that type II cysts (high Na/K ratio) may be transudative, that is, partly derived from plasma elements which enter through gap junctions, while Type I cysts (high K/Na ratio) are primarily secretory. In transudative cysts, plasma protease inhibitors may be present, but the balance between protease and its inhibitors may fluctuate as a result of as yet undetermined circumstances. An imbalance between the protease activity of cyst fluid and its inhibitors may be involved in the pathogenesis of breast gross cys…

Natural history of cystic disease: the importance of cyst type.

All breast cysts aspirated from a series of 100 patients followed for a minimum period of 2 years were classified on the basis of electrolyte composition as apocrine or flattened, this being the nature of the epithelium lining the two populations of breast cysts. Patients with a single cyst were more than 3 times as likely to have a flattened rather than an apocrine cyst. Multiple cysts, whether simultaneous or sequential in any individual patient, were usually all of the same type, and were more commonly apocrine than flattened. A comparison of the frequency of subsequent cysts in patients wh…
