OBJECTIVE: The purpose of the present work is to determine whether bulimia nervosa (BN) and eating disorder not otherwise specified, BN type (EDNOS-BN) were qualitatively distinct in terms of eating and general psychopathology.
METHOD: This study presents a comparison of 138 women with BN and 57 with EDNOS-BN from a multisite study on eating-related and general psychopathology measures.
RESULTS: Although women with BN reported higher lifetime history rates of anorexia nervosa, greater binge eating and vomiting frequency, and more eating concerns, no significant differences were observed betw…

Abnormal serotonergic regulation in bulimia nervosa is thought to contribute to recurrent binge eating, depressed mood, and impulsivity. To follow-up on previous studies showing decreased neuroendocrine responses in symptomatic patients, this study assessed serotonin-mediated prolactin responses in individuals who had remitted from bulimia nervosa. Subjects included 21 women with a history of bulimia nervosa and 21 healthy female controls, as well as an additional comparison group of 19 women with current bulimia nervosa. Placebo-controlled neuroendocrine response studies utilized a single ora…

OBJECTIVE: This study examined pain sensitivity and its relationship to arterial blood pressure in bulimia nervosa (BN).
METHODS: Fourteen women who met Diagnostic and Statistical Manual of Mental Disorders, 4th Edition, criteria for BN, purging subtype, and 14 controls were tested for ischemic pain sensitivity after an extended baseline period. Blood pressure, cardiac output, stroke volume, and total peripheral resistance were assessed during baseline, during ischemic pain testing, and at the point of voluntary tolerance.
RESULTS: Women with BN had significantly greater ischemic pain tolera…

Controlled studies have established manual-based cognitive-behavioral therapy (CBT) is the first-line treatment of choice for bulimia nervosa. Nevertheless, its effectiveness is limited. On average, only 50% of patients cease being eating and purging. Of the remainder, some show partial improvement, whereas a small number derive no benefit at all. In treating nonresponders to CBT one option would be to use antidepressant medication. A second would be to adopt interpersonal psychotherapy (IPT), an alternative psychological therapy with empirical support. However, both options have failed to red…

OBJECTIVE: The results of past research suggest the possible existence of a distinct subgroup of bulimic individuals who display multiple behaviors indicative of impulsivity (e.g., stealing, self-injury, attempted suicide, drug abuse). We further investigated potential relationships between multi-impulsivity and other clinical variables.
METHOD: We compared women with bulimia nervosa (purging type) who displayed "multi-impulsivity" (n = 40) to those who did not (n = 177) with regard to symptom history and presentation, eating-disordered attitudes, and sexual experience.
RESULTS: The two grou…

OBJECTIVE: The authors' purpose in this study was to further delineate the character of cerebral metabolism in bulimia nervosa and to determine if functional links could be made between regional cerebral metabolism and the symptoms of depression, obsessive-compulsive disorder, and bulimia nervosa.
METHOD: Regional cerebral glucose metabolism was measured by using positron emission tomography in 11 inpatients with bulimia nervosa and 18 normal comparison subjects matched in sex (all were women), age, and educational level. The bulimic patients were also tested for symptoms of major depression …

Parotid gland enlargement occurs in about 25% of patients with the binge eating syndrome of bulimia nervosa. The parotid salivary secretory patterns in 28 bulimics were determined in order to investigate the functional abnormality in the glands. Bulimia patients had a reduced resting flow rate. Bulimics who developed sialadenosis (4 patients) had reduced resting and stimulated flow rates. The salivary amylase activity was increased in both the resting and stimulated states in bulimics and the sialadenosis group. The resting total protein levels were greater in the bulimics. The electrolyte and…

Erosion of the dental hard tissues and enlarged parotid and submandibular saliva glands are commonly associated with bulimia nervosa. In 15 patients and controls, no significant difference was detected in the concentrations of potassium, chloride, calcium, urea nitrogen or albumin. There was also no evidence of olfactory dysfunction.

Methylphenidate (MPT) was prescribed four days after an uncomplicated appendectomy in a 27 year old woman who had suffered from bulimia nervosa (BN) for at least nine years. Before the onset of appendicitis, her bingeing and self-induced vomiting had occurred several times daily. With MPT the patient reported a calm emotional state and an absence of temptation to binge or to induce vomiting. Previously published reports of treatment of BN with MPT could not be found. This may be the first. Vulnerability to surgical disorders and to postoperative complications as well as the safety and efficacy…

Fifty-six bulimia nervosa patients were treated by means of a behavioural approach and followed for at least one year after completion or dropping out of treatment. Outcome was encouraging in about half of the patients and several psychiatric indicators, such as duration of illness and response to treatment, were identified. The significance of the findings and unresolved methodological issues are discussed.

Bulimia nervosa (BN) shares central features with substance-related and addictive disorders. The metabotropic glutamate receptor subtype 5 (mGlu5) plays an important role in addiction. Based on similarities between binge eating and substance-related and addictive disorders, we investigated mGlu5 in vivo in 15 female subjects with BN and 15 matched controls. We measured mGlu5 distribution volume ratio (DVR) with positron emission tomography (PET) using [11 C]ABP688. In BN mGlu5 DVR was higher in the anterior cingulate cortex (ACC), subgenual prefrontal cortex, and straight gyrus (p < 0.05). In …

PURPOSE: The current study aimed to determine which particular eating disorder (ED) symptoms and related features, such as BMI and psychological distress, uniquely predict impairment in bulimia nervosa (BN).
METHODS: Two hundred and twenty-two adults with BN completed questionnaires assessing ED symptoms, general psychological distress, and psychosocial impairment. Regression analyses were used to determine predictors which account for variance in impairment.
RESULTS: Four variables emerged as significant predictors of psychosocial impairment: concerns with eating; concerns with weight and s…

Several approaches to the treatment of bulimia nervosa have proved effective, including cognitive-behavioral therapy; however, not all patients improve. It is therefore necessary to explore the possibilities of increasing the efficacy of such treatments. One way to attempt this is to incorporate new technologies. This review explores the possibility of developing a new, empirically validated procedure for the treatment of bulimia nervosa patients that involves cue exposure via virtual reality.

BACKGROUND: The neurobiology of bulimia nervosa (BN) is poorly understood. Recent animal literature suggests that binge eating is associated with altered brain dopamine (DA) reward function. In this study, we wanted to investigate DA-related brain reward learning in BN.
METHODS: Ill BN (n = 20, age: mean = 25.2, SD = 5.3 years) and healthy control women (CW) (n = 23, age: mean = 27.2, SD = 6.4 years) underwent functional magnetic resonance brain imaging together with application of a DA-related reward learning paradigm, the temporal difference (TD) model. That task involves association learni…

OBJECTIVE: This study aimed to examine sex differences in young people's beliefs about the causes and characteristics of, as well as treatment for, bulimia nervosa.
METHOD: Participants (100 male, 102 female) completed a four part questionnaire adapted from other studies.
RESULTS: There were sex differences on about a third of the attitude statements particularly about the cure for bulimia nervosa. A factor analysis of each section revealed six to seven interpretable factors per issue. An analysis of the factor scores showed few significant sex differences but most for the characteristics di…
