Hemodynamic effects of digoxin on congestive heart failure.

To assess the hemodynamic effects of digoxin (0.01 mg/kg) on congestive heart failure, we evaluated 19 patients with decreased contraction force of left ventricle (old myocardial infarction n = 9, and dilated cardiomyopathy n = 10, group 1) and 8 patients with mechanical impaired left ventricular filing (mitral stenosis n = 8, group 2). In groups 1 and 2, heart rate and pulmonary capillary pressure significantly decreased (p less than 0.05). In group 1, stroke volume increased, but not significantly. In group 2, stroke volume increased significantly (p less than 0.05). There were no significan

Structural Microvascular Dysfunction: Remodeling and Capillary Rarefaction in Ischemia with Nonobstructive Coronary Arteries.

Structural coronary microvascular dysfunction (CMD) is an important endotype of ischemia with nonobstructive coronary arteries (INOCA) and is characterized by adverse arteriolar remodeling and capillary rarefaction. These alterations impair coronary vasodilator capacity and increase microcirculatory resistance, all of which contribute to impaired myocardial perfusion. In this article, we aim to review the anatomic and pathophysiological basis of structural CMD, its clinical presentation and prognostic relevance, and the role of invasive physiologic indices in its identification. We also highli

Exercise and insulin-mediated capillary recruitment in muscle.

Blood flow to contracting muscles is increased rapidly and is redistributed within the muscle. Recent research has highlighted that insulin causes similar changes, but these are absent in insulin resistance, resulting in impaired insulin-mediated muscle glucose uptake. Understanding the mechanisms and interactions between contraction- and insulin-mediated alterations in blood flow within muscle may reveal new approaches to overcome insulin resistance.

Effects of blood flow reduction and lymphatic ligation on coronary transport in dogs.

We studied the effects of reduced-flow coronary ischemia and coronary lymphatic ligation on transcapillary movement of albumin. We infused 125I-albumin intravenously into four groups of five anesthetized, open-chest dogs in which external carotid-to-coronary arterial cannulas had been placed. One group was subjected to both lymphatic ligation and ischemia; a second group to lymphatic ligation only; a third group ischemia only; and the fourth was a control group. After 1 hr, 85Sr-microspheres were injected into the cannula. After 2 hr, the animals were sacrificed. Extravascular albumin activiti

Role of the vascular factor in hypertension.

A survey is given of experimental results in both man and hypertensive rats, suggesting a rapid and hemodynamically important structural 'resetting' of the systemic precapillary resistance vessels, of the renal 'long-term' and carotid-aortic 'short-term barostats', as well as of the left heart, which together greatly affect both the initiation and maintenance of hypertension. Particularly the adaptation in design of the precapillary resistance vessels, being a per se normal, local response to more sustained changes in transmural pressure ('structural autoregulation'), implies the introduction …

Clenbuterol administration does not enhance the efficacy of furosemide in attenuating the exercise-induced pulmonary capillary hypertension in Thoroughbred horses.

The stimulation of pulmonary beta2-adrenergic receptors causes a decrease in vascular resistance. Thus, the present study was carried out to examine whether concomitant administration of clenbuterol-a beta2-adrenergic receptor agonist, to horses premedicated with furosemide would attenuate the exercise-induced pulmonary capillary hypertension to a greater extent than furosemide alone, and in turn, affect the occurrence of exercise-induced pulmonary hemorrhage (EIPH). Experiments were carried out on six healthy, sound, exercise-trained Thoroughbred horses. All horses were studied in the control

Microvascular dysfunction and cardiac fibrosis in heart failure with preserved ejection fraction: a case report.

We report the case of a 55-year-old woman with heart failure with preserved ejection fraction (HFpEF), who presented with hypertensive urgency and pulmonary oedema. The patient was medically optimized and underwent cardiac catheterization revealing pulmonary hypertension, elevated pulmonary capillary wedge pressure, normal cardiac index, and non-obstructive coronary disease. Invasive evaluation of coronary flow revealed blunted coronary flow reserve and increased index of microvascular resistance. Cardiac magnetic resonance imaging demonstrated reduced global myocardial perfusion and diffuse i

A simple Computational Model-Based Validation of Guyton's closed circuit analysis of the heart and the peripheral circulatory system.

A computer model of the human cardiovascular system has been created based on Guyton's closed circuit analysis of the heart and the peripheral circulatory system. This model was checked against Guyton's experiments performed in anesthetised dogs regarding the normal venous return curve, the effect of mean circulatory filling pressure (MCFP) and the importance of arterial venous and capillary resistance. These comparisons indicate that this simple model is valid for the study of Guyton's experimental work and also forms the bases of a more complex model of the cardiovascular model with specific…

[Restriction of pulmonary capillary volume, delayed sequelae of immersion in fresh water with respiratory distress syndrome].

Lung mechanics and pulmonary gas exchange studies were performed 6 and 12 months after the accident on 2 survivors of adult respiratory distress syndrome (ARDS) following near drowning in fresh water. The patients (25 and 29 years of age) were nonsmokers and neither had a history of lung disease. Immersion lasted 10 and 20 minutes, and assisted ventilation 19 and 34 days, respectively. The results show that lung mechanics returned to normal within 6 months after the onset of ARDS. Conversely, the ratio of dead space to tidal volume was increased significantly at rest and during exercise, and t

Fluid and Electrolyte Management in Critically Ill Patients with Infections.

Fluid and electrolyte management is a crucial component of care in critically ill patients with severe infections. Patients with sepsis, septic shock, or multiorgan dysfunction syndrome struggle to maintain fluid balance due to increased capillary permeability, inflammation, vasodilation, and organ dysfunction. Inadequate fluid balance can increase complications, morbidity, and mortality. Fluid management is complex because sepsis disrupts vascular permeability, tissue perfusion, and cellular function. Infectious disease care is further challenged by antimicrobial resistance, drug stewardship,

Pulmonary Hypertension in Chronic Kidney Disease.

There is a high prevalence of pulmonary hypertension in chronic kidney disease (CKD), with rates increasing as glomerular filtration rate declines. Pulmonary hypertension is associated with a higher risk of cardiovascular events and mortality in non-dialysis-dependent CKD stages 3 to 5, dialysis-dependent CKD, as well as kidney transplant recipients. The pathophysiology of pulmonary hypertension in CKD is multifactorial and includes higher pulmonary capillary wedge pressure caused by ischemic heart disease and cardiomyopathy, higher cardiac output caused by anemia and arteriovenous access used

Updates on Management for Acute and Chronic Phenotypes of Neonatal Pulmonary Hypertension.

Neonatal pulmonary hypertension is a heterogeneous disease in term and preterm neonates. It is characterized by persistent increase of pulmonary artery pressures after birth (acute) or an increase in pulmonary artery pressures after approximately 4 weeks of age (chronic); both phenotypes result in exposure of the right ventricle to sustained high afterload. In-depth clinical assessment plus echocardiographic measures evaluating pulmonary blood flow, pulmonary vascular resistance, pulmonary capillary wedge pressure, and myocardial contractility are needed to determine the cause and provide indi

[Portopulmonary hypertension with recurrent syncope:
a case report and review of literature].

A case of portopulmonary hypertension characterized by repeated syncope was retrospectively analyzed. Intrahepatic or extrahepatic factor-induced portal hypertension complicated with metabolic disorder of vasoactive substances, vascular pressure, inflammation, etc. may result in systolic and diastolic dysfunction of pulmonary arteries and systemic hyperdynamic circulation, the long-term effect of which can induce vascular remodeling and consequently, pulmonary hypertension. The pathogenic process is rather insidious. Pulmonary hypertension is clinically characterized by the raised average pulm

Patent ductus arteriosus and respiratory outcome in premature infants.

A persistent ductus arteriosus is a common event in preterm infants. The systemic-to-pulmonary shunting that occurs as the pulmonary vascular resistance decreases after birth can have significant cardiovascular and respiratory consequences. Acute pulmonary effects include pulmonary edema and hemorrhage, worsened lung mechanics and deterioration in gas exchange with hypoxemia and hypercapnia. The increased pulmonary blood flow can also produce damage to the capillary endothelium and trigger an inflammatory cascade. This, plus the need for longer and more aggressive mechanical ventilation, can e

Aluminium phosphide poisoning: haemodynamic observations.

Haemodynamic measurements were made on 25 patients with aluminium phosphide poisoning. There was severe hypotension (mean arterial pressure 62.7 +/- 19.3 mmHg) and reduction in cardiac output (2.13 +/- 0.9 lit/min/m2). However, the systemic vascular resistance was only moderately elevated (2050 +/- 732 dynes/m2). The right atrial pressure was increased but the pulmonary artery and pulmonary capillary wedge pressures were normal, even in patients with pulmonary oedema. Thirteen patients (52%) who died had a lower cardiac output (p less than 0.05). Saline infusion significantly improved haemodyn

[Histomorphometric characteristic of human brain in acute alcoholic intoxication].

Different brain sections were studied in 20 subjects, who died of ethanol intoxication and in 14 subjects who died of injuries of the heart and main vessels, in order to detect histological changes in the brain and for the purpose of defining spatial and quantitative ratios between cerebral tissue structures in alcoholic intoxication. Different histological, stereometric and morphometric tools were made use of. It was demonstrated that, in alcoholic intoxication, there occur severe disorders of the circulation with affection of vessels in the brain; there are also dystrophic and necrotic chang

Muscle hemodynamics in hereditary myopathy of Labrador retrievers.

Morphologic lesions seen in six 8-month-old Labrador Retrievers with hereditary myopathy were predominantly small- and large-group atrophy of muscle cells of all fiber types. The dogs were intolerant of exercise and fatigued rapidly. An isolated gracilis muscle preparation was used to study the hemodynamic features of the microvasculature. Isogravimetric capillary pressure as well as arterial and venous pressures in the isolated gracilis muscle preparation obtained during maximal vasodilatation were within the range reported for healthy, mixed-breed dogs, as were precapillary, postcapillary, a

[Endoscopic diagnosis of oesophagitis: problems of differentiation from normal (author's transl)].

The diagnostic significance of endoscopically early signs of oesophagitis in the absence of macroscopic evidence of epithelial abnormalities has been studied prospectively. Changes from the usually observed shininess, pale-pink colour, smooth texture, regular capillaries, sharp Z-line and a decreased mechanical resistance of the mucosa to mechanical damage do not indicate oesophagitis. Nor should one infer gastro-oesophageal reflux from these endoscopic findings. They are presumably ageing changes in the oesophagus.
