[Simultaneous operation for lung tumor and cardiovascular disease].

Successful simultaneous operation for lung tumor and cardiovascular disease was performed in three cases. A 76-year-old man with stage I lung cancer and ischemic heart disease underwent a partial lobectomy following single coronary artery bypass grafting through a median sternotomy. A 62-year-old man with stage I suspected lung cancer and thoracic aortic aneurysm underwent a partial segmentectomy before aneurysmectomy and patch closure using vascular prosthesis through a left posterolateral thoracotomy. These two cases were performed under extracorporeal circulation. A 69-year-old man with bro

[A perineal radical prostatectomy technic for localized prostatic cancer].

In the absence of an indication for pelvic lymph node dissection or when this has already been performed by other procedures, perineal radical prostatectomy provides excellent local control of localized prostatic cancer. By respecting the local and general contraindications (volume of the prostate, suppleness of the hip joint, cardiovascular status), perineal prostatectomy ensures good quality resection strictly identical to that obtained by retropubic prostatectomy. The time to return of continence is unchanged. The only problem consists of the difficulty of preserving the neurovascular pedic

Resuscitative Mean Arterial Pressure Targets in Cardiovascular Disease: A Narrative Review of Clinical Outcomes.

Mean arterial pressure (MAP) is a commonly used hemodynamic proxy for tissue perfusion. Continuous MAP monitoring and maintenance of MAP targets remains a cornerstone for managing shock states. However, co-morbidities and individual variations can complicate the relationship between MAP and organ perfusion. Factors such as fluid balance, microvascular circulation, and endothelial function all influence tissue perfusion. This review examines the nuances of MAP management across various patient populations with cardiovascular disease, including acute myocardial infarction cardiogenic shock, non-

A nomogram for predicting the risk of postoperative delirium in individuals undergoing cardiovascular surgery.

BACKGROUND AND PURPOSE: Delirium is a common mental disorder after adult cardiovascular surgery. Fifteen to 23% of patients undergoing cardiovascular surgery and cardiomyopathy experience delirium, and the efficacy of treatment interventions for delirium has been consistently unsatisfactory. METHODS: A total of 729 patients who underwent cardiovascular surgery were randomly allocated into a training set and a validation set. A nomogram was developed using a logistic regression model to predict the incidence of delirium following cardiovascular surgery. The validity of the model was assessed b

Gasparyan Method of Total Autologous Reconstruction of the Aortic Valve.

In this case report, I describe a new technique for total reconstruction of the aortic valve with autologous pericardium. The parameters of the cusps were calculated using very simple formulas after measurement of the aortic root intercommissural distances. Glutaraldehyde-treated pericardium was trimmed along the marked line, leaving 2 mm of tissue along the fibrous annulus attachment margin for the suture and small wings on both commissural margins to secure the commissural coaptation between right and noncoronary cusps. The annular margin of each pericardial cusp was sutured to the correspon

Prevalence of major complications and procedural mortality in 336 dogs undergoing interventional cardiology procedures in a single academic center.

INTRODUCTION: Overall complication rates associated with a wide range of diagnostic and therapeutic interventional cardiac procedures in a contemporary academic setting have not been reported. ANIMALS, MATERIALS AND METHODS: Consecutive interventional procedures performed for client-owned dogs were retrospectively analyzed to characterize procedural complications and mortality. RESULTS: Three hundred sixty-four procedures were performed on 336 dogs. Interventions included attempted or completed transvenous pacemaker (PM) implantation (n = 134) with subsequent pacing system revision (n = 8),

[A. N. Bakoulev Scientific Center for Cardiovascular Surgery].

OBJECTIVE: to assess dynamics of diastolic function for detection of development of diastolic dysfunction (DD) and it's causes, to evaluate the effect of DD on prognosis in the postoperative period in patients with acquired heart diseases. MATERIALS AND METHODS: We included in this study 112 patients with aortic and mitral valve diseases (90 men, 22 women, median age 51 [35; 57] years). All patients underwent echocardiography (echo), tissue Doppler, speckle tracking echo prior to surgery, in the early postoperative period (8-14 days) and in 12-36 months after surgery. In 28 patients dynamic c

Cardiac Surgery in HIV Patients: State of the Art.

The clinical status of HIV infection has changed dramatically with the introduction of combined antiretroviral therapy. Patients with HIV are now living long enough to be susceptible to chronic illnesses, such as coronary disease and nonischemic cardiomyopathy, which can be consequences of the combined antiretroviral therapy treatment itself. Cardiovascular diseases are a major source of morbidity and mortality in HIV-positive patients. Increasingly, such patients might be candidates for the full range of cardiac surgical interventions, including coronary bypass, valve surgery, and heart trans

Future of transcatheter aortic valve implantation - evolving clinical indications.

Accumulating clinical experience and technological improvements have provided the basis for transcatheter aortic valve implantation (TAVI) to emerge as a well-established means for treating patients with severe symptomatic aortic stenosis at high or prohibitive surgical risk. During this decade, TAVI has emerged as a valid alternative to surgical aortic valve replacement in patients at intermediate surgical risk, and several studies are currently being performed to evaluate the role of TAVI in patients at low surgical risk. Furthermore, promising, but preliminary, data are emerging on the effi

Role of nutrition support in adult cardiac surgery: a consensus statement from an International Multidisciplinary Expert Group on Nutrition in Cardiac Surgery.

Nutrition support is a necessary therapy for critically ill cardiac surgery patients. However, conclusive evidence for this population, consisting of well-conducted clinical trials is lacking. To clarify optimal strategies to improve outcomes, an international multidisciplinary group of 25 experts from different clinical specialties from Germany, Canada, Greece, USA and Russia discussed potential approaches to identify patients who may benefit from nutrition support, when best to initiate nutrition support, and the potential use of pharmaco-nutrition to modulate the inflammatory response to ca

Interventional fellowship in structural and congenital heart disease for adults.

Training for structural and adult congenital heart disease interventions remains undeveloped. With the advent of recent percutaneous interventions for the treatment of structural and valvular heart disease, such as transcatheter aortic and pulmonary valve implantation, mitral valve repair, and the expansion of shunt closure procedures, there is a clear need to define the training requirements for this category of procedures. The training needs to be aligned with the goals and priorities of a basic or advanced level and be categorized into acquired and congenital. This document will define the

Evaluation and management of the adult patient with transposition of the great arteries following atrial-level (Senning or Mustard) repair.

Atrial-level repair for transposition of the great arteries is one of the greatest achievements of modern cardiovascular medicine, transforming a once fatal congenital heart condition into one with excellent long-term survival. Although atrial-level repair has been supplanted by the arterial switch, there remains a population of patients who underwent atrial-level repair as children and now require ongoing care as adults. Survival slowly continues to decline in this population mainly owing to systemic right ventricular dysfunction and sudden death. Other problems include sinus node dysfunction

Valve-sparing reconstruction within the native aortic root: integrating the Yacoub and the David methods.

I propose an integrated valve-sparing aortic root reconstruction that combines the surgical principles of both the Yacoub and David methods. Valve re-suspension is first completed within the native aortic root; then the graft is anchored to the pledget-reinforced, scalloped true aortic annulus.

Partial aortic root remodelling for fistula between the non-coronary sinus and the right atrium.

This report describes a rare case of the fistula between the non-coronary sinus and the right atrium (RA) after ascending aortic replacement for chronic aortic dissection. A 67-year-old lady had been suddenly suffering from severe dyspnoea with general fatigue for a couple of days. Trans-thoracic echocardiogram in the emergency room demonstrated massive shunt flow from the non-coronary sinus to the RA with remarkable dilatation of the RA, right ventricle (RV) and inferior vena cava, similar to the rupture of sinus of Valsalva (Konno-type IV). The fistula was successfully treated by partial rem
