Ipsilateral late stroke after revascularization surgery for patients with Moyamoya disease.

BACKGROUND: Ipsilateral late stroke events occurring after cerebral revascularization for Moyamoya disease (MMD) and their risk factors have not been fully investigated. METHODS: We retrospectively analyzed 123 patients with MMD who underwent 212 revascularizations. We investigated preoperative demographic data, surgical procedures, and ipsilateral stroke events occurring more than 1 month after surgery. The effect of revascularization and the residual Moyamoya vessel (MMV) score were examined using magnetic resonance angiography (MRA). Then, predictive factors for postoperative late stroke o

Treatment of aneurysms in patients with moyamoya disease: a 10-year single-center experience.

OBJECTIVE Moyamoya disease (MMD) is occasionally accompanied by intracranial aneurysms. The purpose of this study was to delineate the efficacy of the authors' current surgical strategy in the management of MMD-associated aneurysms of different types. METHODS Between January 2007 and March 2016, a consecutive cohort of 34 patients with 36 MMD-associated aneurysms was enrolled in this prospective single-center cohort study. The lesions were classified as peripheral (17 aneurysms) or main trunk aneurysms (13 in the anterior circulation and 6 in the posterior circulation). For the peripheral aneu

Atypical postoperative radiographical findings in a patient with moyamoya disease: A case report.

Moyamoya disease is a chronic cerebrovascular disease characterized by spontaneous and progressive stenosis or occlusion of the internal carotid artery and its branches. Revascularization procedures have been shown to improve cerebral hemodynamics and decrease the risk of strokes, but several postoperative complications are known to occur.A 14-year-old girl with moyamoya disease with a history of left-sided revascularization surgery underwent right-sided revascularization. On postoperative day 4, she experienced a transient neurological event (left hemiparesis). Magnetic resonance imaging reve

Metabolism changes during direct revascularization in moyamoya disease: illustrative case.

BACKGROUND: Cerebral revascularization is recommended for patients with moyamoya disease (MMD) with reduced cerebral perfusion reserve and recurrent or progressive ischemic events. The standard surgical treatment for these patients is a low-flow bypass with or without indirect revascularization. The use of intraoperative monitoring of the metabolic profile using analytes such as glucose, lactate, pyruvate, and glycerol has not yet been described during cerebral artery bypass surgery for MMD-induced chronic cerebral ischemia. The authors aimed to describe an illustrative case using intraoperati

[Two-stage contralateral combined microvascular and wide-lumen anastomosis inTwo-stage contralateral combined microvascular and wide-lumen anastomosis in a patient with acute ischemic stroke patient with acute ischemic stroke].

Common carotid artery (CCA) thrombosis is a rare cause of ischemic stroke. In available literature, we found no studies devoted to cerebral revascularization for CCA occlusion in acute period of ischemic stroke. Successful treatment of concomitant occlusion of CCA, internal (ICA) and external carotid arteries (ECA) with microvascular anastomoses is very interesting for various specialists. To demonstrate the possibility of contralateral cerebral revascularization in a patient with CCA, ECA and ICA occlusion in acute period of ischemic stroke. Two-stage intervention was performed in a patient w

The V3 segment of the vertebral artery as a robust donor for intracranial-to-intracranial interpositional bypasses: technique and application in 5 patients.

The V3 segment of the vertebral artery (VA) has been studied in various clinical scenarios, such as in tumors of the craniovertebral junction and dissecting aneurysms. However, its use as a donor artery in cerebral revascularization procedures has not been extensively studied. In this report, the authors summarize their clinical experience in cerebral revascularization procedures using the V3 segment as a donor. A brief anatomical description of the relevant techniques is also provided.

[Surgical Approaches to Pediatric Moyamoya Disease].

Moyamoya disease(MMD)is a chronic, occlusive cerebrovascular disease with unknown etiology, characterized by progressive stenosis of the internal carotid artery terminus and abnormal vascular network formation at the base of the brain. MMD has bimodal age distribution, with peaks in children and young adults, and surgical revascularization is a reasonable management choice for pediatric and adult patients with ischemic MMD. The superficial temporal artery-middle cerebral artery anastomosis with indirect pial synangiosis(combined revascularization)is a preferred surgical procedure for adult and

Occipital Artery to Middle Cerebral Artery Bypass: Operative Nuances.

BACKGROUND: Superficial temporal artery (STA)-middle cerebral artery (MCA) anastomosis is a common procedure for vascular neurosurgeons, and it is used in a variety of diseases. However, there are cases in which the STA is absent or is too hypoplastic to be used as a donor for revascularization. Occipital artery (OA)-MCA bypass may be a treatment option in these cases. METHODS: We encountered 4 cases of symptomatic cerebral ischemia in which the STA was absent or unavailable. These cases were treated by revascularization from the OA to the periphery of the MCA. RESULTS: By meticulous dissect

Improvement in multiple cognitive domains via combined revascularization by 6 months' follow-up: a new potential surgical indication in moyamoya disease.

OBJECTIVE: Moyamoya disease (MMD) is a cerebrovascular condition characterized by progressive stenosis of the main branches of the cerebral artery. Patients with MMD are prone to cognitive decline due to chronic hypoperfusion. Whether revascularization surgery can reverse cognitive dysfunction by improving cerebral blood flow perfusion and what types and levels of cognitive dysfunction can benefit most from the surgery remain unclear. METHODS: Between July 31, 2022, and May 15, 2024, a total of 155 patients were prospectively enrolled in this trial and underwent combined or indirect revascula

The value of 3D pseudo-continuousarterial spin labeling perfusion imaging in moyamoya disease-Comparison with dynamic susceptibility contrast perfusion imaging.

BACKGROUND AND PURPOSE: 3D pseudo-continuous arterial spin labeling (3D pCASL) is commonly used to measure arterial cerebral blood flow (CBF). The aim of this study was to assess the clinical feasibility and accuracy of 3D pCASL in comparison with dynamic susceptibility contrast (DSC) perfusion imaging in moyamoya disease (MMD). MATERIALS AND METHODS: A total of 174 MMD patients underwent 3D pCASL and DSC-MRI for evaluating cerebral blood perfusion. 3D-pCASL with two single post-labeling delay (PLD) times (1,500 and 2,500 ms) was used to measure CBF. The values of DSC-CBF and ASL-CBF were cal

Pial collateral shift after unilateral bypass in bilateral moyamoya disease: illustrative cases.

BACKGROUND: Bilateral revascularization surgery is commonly performed in patients with bilateral moyamoya disease (MMD), even when ischemic symptoms or strokes are confined to one hemisphere. However, in select cases, a unilateral bypass may suffice to revascularize both hemispheres due to the development of post-revascularization pial collateral shift. This report aims to introduce the concept of pial collateral shift and presents 3 cases of bilateral MMD in which unilateral revascularization alone was deemed sufficient. OBSERVATIONS: Three adult patients with ischemic MMD exhibited bilatera
