The brachial plexus tension test (BPTT) appears to offer a means of examining the extensibility and mechanosensitivity of the neural tissues related to an upper limb. This test was used to evaluate possible cervical or brachial plexus neural involvement causing arm pain syndromes in 37 patients presenting for assessment following neck injury in motor vehicle accidents. The BPTT was considered positive in 55 of the 61 symptomatic arms. There were no false-positive responses in the 13 asymptomatic arms although a slight loss of extensibility was evident in five arms. Twenty patients without clin

Harvey Cushing is widely regarded as one of the forefathers of neurosurgery, and is primarily associated with his work on intracranial pathology. However, he had a clinical and academic interest in peripheral nerve surgery. Through the courtesy of the Alan Mason Chesney Medical Archives, the surgical records of the Johns Hopkins Hospital from 1896 to 1912 were reviewed. The records of a single patient undergoing brachial plexus exploration and cervical rib resection were selected for detailed review. The operative report and accompanying illustrations demonstrate Cushing's interest in adding a

An experimental model was established to investigate the possibility of repairing cervical nerve roots avulsed from the spinal cord, as occurs in traction injuries of the brachial plexus. In five sheep the C6 root was avulsed and the ventral root was reattached using freeze-thawed muscle as a short graft (0.5 cm). Recovery was assessed after one year by electrophysiology and histology. Stimulation of the root produced muscle contractions in four out of five sheep. Action potentials were recorded distal to the grafts in all five sheep. Histological examination showed regenerated fibres in the v

Studies on nerve fibres within the phrenic nerve and in the diaphragmatic plexus were carried out in one species, the gerbil, a small desert rodent. The resistance of this small mammal to surgical insult enabled experimental intrathoracic transection of the phrenic nerve to be carried out so that the results in 10 animals could be observed within the diaphragmatic plexus, the phrenic nerve, and the phrenic nucleus. The findings in the diaphragmatic plexus and in the nerve trunk strongly resemble corresponding results for other animals with regard to `persisting' fibres. The phrenic nucleus, ho

A 40-year-old woman presented with symptoms compatible with Raeder syndrome. MR demonstrated narrowing of the left cavernous carotid artery. The high cervical portion of the left internal carotid artery was not identified. Angiography confirmed the narrowing of the cavernous carotid artery and showed marked and irregular stenosis of the distal cervical internal carotid artery. Involvement of the left sympathetic plexus and of ipsilateral cavernous sinus arteries is believed to have been the cause of the Raeder syndrome in this patient.

Morphological features of ansa cervicalis and phrenic nerve were studied in 106 cadavers. Ansa cervicalis was located medial to the internal jugular vein in 63% (medial type) and lateral to the vein in 33.7% (lateral type). Ansa cervicalis was derived from a combination of C1-C4 spinal segments, with C1-C3 being the most frequent pattern (87.5%). In >60% the ansa was bilaterally symmetrical. The distribution of medial and lateral types was equal on left and right sides of the body. The segmental composition of the inferior root was higher in the medial type and also on the left side of the bod

Thoracic outlet syndrome (TOS) is an underdiagnosed condition that is frequently misattributed to cervical spine pathology. Neurogenic TOS (nTOS), the most common subtype, often presents nonspecific upper extremity pain and paresthesia, making diagnosis challenging. We report a case of nTOS associated with a cervical rib that was successfully treated with ultrasound-guided hydrodissection (HD). Dynamic ultrasound examination demonstrated brachial plexus tethering with symptom reproduction. Serial HD resulted in immediate and sustained symptom relief, suggesting that this minimally invasive tec

The neck has intricately connected neural structures, including cervical and brachial plexi, the sympathetic system, lower cranial nerves, and their branches. Except for brachial plexus, there has been little research regarding the normal imaging appearance or corresponding pathologies of neural structures in the neck. The development in imaging techniques with better spatial resolution and signal-to-noise ratio has made it possible to see many tiny nerves to predict complications related to image-guided procedures and to better assess treatment response, especially in the management of oncolo

It is now recognized that spinal dural arteriovenous fistula causes myelopathy, resulting from congestive venous hypertension of the spinal cord. We have recently encountered a rare case of high cervical spinal cord dural arteriovenous fistula which caused intracranial subarachnoid hemorrhage. The patient was a 62-year-old female, who had exhibited an abrupt onset of severe headache and loss of consciousness. Plain head CT scan revealed intracranial subarachnoid hemorrhage. No abnormal vascular lesions were found on the first cerebral angiography. The second angiographical examination demonstr

Thoracic outlet syndrome (TOS) is a rare condition (1-3 per 100,000) caused by neurovascular compression at the thoracic outlet and presents with arm pain and swelling, arm fatigue, paresthesias, weakness, and discoloration of the hand. TOS can be classified as neurogenic, arterial, or venous based on the compressed structure(s). Patients develop TOS secondary to congenital abnormalities such as cervical ribs or fibrous bands originating from a cervical rib leading to an objectively verifiable form of TOS. However, the diagnosis of TOS is often made in the presence of symptoms with physical ex

This report highlights the clinical features of two patients who presented with severe neuropathic chest wall pain caused by herniated C6-7 disc, and speculates on the pathophysiology of this syndrome. Worsening of symptoms with neck movement helped localize the process as cervical spine rather than plexus in origin. Both patients had herniated C6-7 disc material compressing the spinal cord and C7 nerve root, and neurological symptoms resolved promptly following surgery. Neuropathic chest wall pain should alert the clinician to consider the diagnosis of cervical disc herniation and prompt defi

We present a case of a 36-year-old pregnant woman with varicose veins of the uterine cervix diagnosed during the second trimester. The cervical tissue was completely replaced with dilated venous plexus. Therapeutic decisions were further complicated by the presence of blood clots within the dilated vessels. Pregnancy proceeded uneventfully, no bleeding occurred and the patient underwent a planned Cesarean delivery. This is the first reported case in the medical literature where the rare phenomenon of cervical varices was accompanied by thrombosis.

We report the case of a young man who presented with atrophy of the musculature of the right forearm and hand. The clinician suspected cervical myelopathy in Hirayama's disease on the basis of the findings at electromyography and recommended an MRI examination in both flexed and neutral position. The typical imaging findings for cervical myelopathy in Hirayama's disease were observed and the diagnosis was confirmed. The key findings for Hirayama's disease consist of asymmetrical medullary atrophy with involvement of the anterior horns, dorsal detachment of the dura mater, and dilatation of the

Identifying the exact cause for persistent and recurrent neurogenic thoracic outlet syndrome (NTOS) is challenging even with high-resolution imaging of the thoracic outlet. Improvement can be achieved with redo first rib resection, although the posterior first rib remnant is one of several potential points of brachial plexus compression. In approaching reoperative surgery for NTOS, the aim is to provide complete thoracic outlet decompression as guided by the patient's history, physical examination, and adjunctive imaging. This may involve resection of the posterior first rib remnant, scar tiss

In the "flexion-adduction" sign, the arm is maintained in a posture of flexion at the elbow and adduction at the shoulder. Abduction and lateral rotation of the arm, with the elbow in extension, may result in radicular pain. These signs may be useful in the evaluation of patients with cervical root or brachial plexus lesions.

Somatosensory evoked neuromagnetic fields are recorded from peripheral nerves in the upper arm, from plexus brachialis at the ventral and dorsal thorax and from the dorsal horn of the cervical spinal cord ('P13m') at the upper lateral neck. Some perspectives for clinical applications are suggested.

The objective was to determine if the cervical vagus or hepatic branch of the vagus nerve is a suitable site to produce functional parasympathetic denervation of the liver in the rat as assessed from the ability to produce insulin resistance. Anterior plexus denervation in both anesthetized rats and cats results in insulin resistance as assessed by the rapid insulin sensitivity test (RIST). This diagnostic test is a modified euglycemic clamp using the amount of glucose required to be infused to maintain euglycemia following a bolus administration of insulin (50 mU kg(-1) over 5 min, 0.1 ml min
