Introduction to the Site-specific Etiologic Results From the Pneumonia Etiology Research for Child Health (PERCH) Study.

The Pneumonia Etiology Research for Child Health (PERCH) study evaluated the etiology of severe and very severe pneumonia in children hospitalized in 7 African and Asian countries. Here, we summarize the highlights of in-depth site-specific etiology analyses published separately in this issue, including how etiology varies by age, mortality status, malnutrition, severity, HIV status, and more. These site-specific results impart important lessons that can inform disease control policy implications.

Intravenous fluid management for the acutely ill child.

PURPOSE OF REVIEW: To review the principles of prescribing intravenous fluids (IVFs) to the acutely ill child and of adjusting sodium composition and fluid rate to prevent disorders in serum sodium or volume status from occurring. RECENT FINDINGS: Recent data have revealed that the historic approach of administering hypotonic IVFs results in a high incidence of hospital-acquired hyponatremia in children. The majority of hospitalized children requiring IVFs are at risk for developing hyponatremia from numerous stimuli for arginine vasopressin (AVP) production, such as volume depletion, pain, s

A four-year-old boy with fever, rash, and arthritis.

The triad of fever, rash, and arthritis in a hospitalized child suggests an inflammatory, infectious, or postinfectious process in most cases; however, malignancy must be considered. The most common causes in this age group are inflammatory conditions, including Kawasaki disease, Henoch-Schönlein Purpura, serum sickness-like reaction, and juvenile idiopathic arthritis. Other rarer inflammatory processes can present with this triad of symptoms such as Cryopyrin-related diseases (autoinflammatory disorders), urticarial vasculitis, and systemic lupus erythematosus. We will discuss the differentia

Community-acquired infection due to Stenotrophomonas maltophilia: a rare cause of septic arthritis.

Stenotrophomonas maltophilia is an important nosocomial pathogen in hospitalized patients, particularly those with prior broad-spectrum antibacterial therapy. The microorganism mainly infects severely ill, debilitated patients and is most frequent in immunocompromised hosts. A prominent feature of this organism is its resistance to multiple antibiotics including beta-lactam agents, carbapenems and aminoglycosides. Community-acquired infection with Stenotrophomonas maltophilia is reported rarely. This is the first report of a child patient diagnosed with septic arthritis due to Stenotrophomonas

[The HELLP syndrome--a rare form of preeclampsia. Anesthesiologic and obstetric aspects].

A 34-year-old quadripara was hospitalized in the 33rd gestational week due to an acute hypertensive crisis, headache, upper abdominal pain, icterus, and proteinuria. Laboratory testing revealed hemolysis, hepatic dysfunction, and thrombopenia. The values returned toward normal after delivery by cesarean section. Diffuse bleeding in the surgical wound and acute renal failure necessitated two relaparotomies, intensive hemotherapy, and hemodialysis. Mother and child were released in good condition. Anesthesiological and obstetrical aspects of the HELLP syndrome are discussed.

Association of cirrhosis severity with outcomes after hip fracture repairs: A propensity-score matched analysis using a large inpatient database.

BACKGROUND: Advanced cirrhosis is associated with increased mortality in certain surgeries, but the impact of cirrhosis severity on outcomes in patients with hip fractures remains unclear. METHODS: In a large nationwide administrative database of hospitalized patients, we compared postoperative outcomes in patients with hip fractures across different Child-Pugh classes of cirrhosis in Japan. Using the Japanese Diagnosis Procedure Combination Database, we identified 833,648 eligible patients diagnosed with hip fractures and underwent surgery between July 2010 and March 2021. Three sets of 1:1

[Interstitial nephropathy, tapeto-retinal degeneration and cataract. Apropos of a new case].

A case of chronic interstitial renal disease is reported. Onset was manifested at the age of three by polyuria and polydipsia. The child was hospitalized at the age of eleven for renal failure and tapetoretinal degenerescence with cataract were found. The simultaneous occurrence of interstitial renal disease and tapetoretinal degenerescence is well-known. However, this case where cataract was also present illustrates the fact that tapetoretinal degenerescence is not the only ocular abnormality found in this interstitial nephropathies.

Quality of Life Impact of Varicella on Children and Their Families in the UK: The QoLPoX Study.

BACKGROUND: Varicella (chickenpox) is a ubiquitous disease of childhood in the UK, yet highly effective and safe vaccines have been routinely used in other countries for years. Previous UK assessments of varicella vaccines did not meet the National Institute of Health and Care Excellence £20-30k/quality-adjusted life year (QALY) cost utility threshold, in part due to limited data quantifying the quality of life (QoL) lost. METHODS: We carried out a prospective multicenter observational study recruiting families while their child had acute varicella or secondary complications. We recruited sev
